Runas un balss traucējumu izplatība Rīgas Austrumu klīniskās universitātes slimnīcā ārstētiem Parkinsona slimības pacientiem. Retrospektīvs pētījums

Abstract

Šī pētījuma tēma - Runas un balss traucējumu izplatība Rīgas Austrumu klīniskās universitātes slimnīcā ārstētiem Parkinsona slimības pacientiem – mūsdienās ir ļoti aktuāla un plaši aprakstīta zinātniskajā literatūrā (Amato et al., 2021; Jerkic et al., 2021; Skodda et al., 2013). Parkinsona slimība (PS) ir hroniska, lēni progresējoša neirodeģeneratīva saslimšana, ar kuru pasaulē slimo vairāk nekā 10 miljoni cilvēku (LNDSA, 2018). Apmēram 90% pacientu Parkinsona slimības gadījumā novēro hipokinētisku dizartriju, ko raksturo runas un balss traucējumi (Jerkic et al., 2021). PS klīniskajās vadlīnijās tiek norādīts, ka pacientiem ar šādiem traucējumiem ir nepieciešamas audiologopēda konsultācijas, taču netiek norādīts, kurā slimības stadijā šādi traucējumi varētu rasties un palīdzība nepieciešama (LNDSA, 2018). Pētījuma mērķis ir izpētīt runas un balss traucējumu izplatību RAKUS ārstētiem PS pacientiem dažādās stadijās un noteikt traucējumu saistību ar slimības smaguma pakāpi. Lai to sasniegtu tika skatīta informācija medicīniskajās vēsturēs PS pacientiem, kas ārstējušies RAKUS laika posmā no 2016. līdz 2020. gadam. Pētījumā tika iekļautas 56 personas ar PS vecumā no 38-89 gadiem (Mdn 67). Rezultāti liecināja, ka 50 % (n=28) pētījumā iekļautajiem dalībniekiem tika novēroti runas un/ vai balss traucējumi. Tikai daļai pacientu (n=34) bija norādīta PS smaguma pakāpe, kas medicīnas vēsturēs bija noteikta pēc modificētās Hoēna un Jāra skalas. Pacientiem ar runas traucējumiem PS bija no 2. līdz 5. pakāpei (Mdn 4.), bet ar balss traucējumiem no 3. līdz 5. pakāpei (Mdn 5.). Izmantojot Manna-Vitnija U testu, tika novērotas statistiski nozīmīgas atšķirības starp PS smaguma pakāpi un runas traucējumiem (p=0,003), kā arī balss traucējumiem (p=0,011). Tika secināts, ka pacientiem ar Parkinsona slimību, kas ārstējušies Rīgas Austrumu klīniskās universitātes slimnīcā laika posmā no 2016. līdz 2020. gadam, runas traucējumu izplatība ir 46,4 %, bet balss traucējumu izplatība 21,4 %. Gan runas, gan balss traucējumi statistiski ticami bija saistīti ar Parkinsona slimības smaguma pakāpi pēc Hoēna un Jāra skalas. Pētījumā iekļautajiem pacientiem ar runas traucējumiem audiologopēda palīdzība būtu nepieciešama 4. Parkinsona slimības smaguma pakāpē, bet pacientiem ar balss traucējumiem 5. slimības smaguma pakāpē.

The topic of this study – Prevalence of speech and voice disorders in Riga East University Hospital (REUH) in patients with Parkinson’s disease – nowadays are very relevant and widely described in the scientific literature (Amato et al., 2021; Jerkic et al., 2021; Skodda et al., 2013). Parkinson’s disease (PD) is a chronic, slowly progressing neurodegenerative disease that affects more than 10 million people worldwide (LNDSA, 2018). Hypokinetic dysarthria, characterized by speech and voice disorders, occurs in approximately 90% of patients with Parkinson’s disease (Jerkic et al., 2021). The PD clinical guidelines state that patients with such disorders require the advice of a speech and language pathologist, but do not specify the stage of the disease at which such disorders may occur, and the assistance is needed (LNDSA, 2018). The aim of this study was to investigate the prevalence of speech and voice disorders in PD patients at different stages treated at REUH and to determine the relationship between the disorder and the severity of the disease. To achieve that, medical history was reviewed for PD patients treated at REUH between 2016 and 2020. The study included 56 individuals with PD aged 38-89 years (Mdn 67). The results showed that 50 % (n=28) of the PD patients enrolled in the study had speech and/ or voice impairment. Only a subset of study participants (n=34) had the degree of the disease recorded in medical history and it was measured by the modified Hoehn and Jahr scale. Patients with speech impairment had PD severity between grade 2 and 5 (Mdn 4) and those with voice impairment from grade 3 to 5. Using Mann-Whitney U test, statistically significant differences were observed between PD severity and speech impairment (p=0,003) and voice impairment (p=0,011). It was concluded that the prevalence of speech disorders in patients with PD treated at Riga East university hospital between 2016 and 2020 was 46,4 %, but the prevalence of voice disorders was 21,4 %. Both speech and voice disorders were statistically significantly associated with the severity of Parkinson’s disease measured by Hoehn and Yahr scale. Patients with speech disorders may need the help of a speech and language pathologist at grade 4 , and patients with voice disorders at grade 5.
