Cieto aukslēju šķeltnes ar mīksto aukslēju šķeltni un runas traucējumu saistība

Abstract

Darba tēma: Cieto aukslēju šķeltnes ar mīksto aukslēju šķeltni un runas traucējumu saistība. Pētnieciskais jautājums: Vai cieto aukslēju šķeltnes ar mīksto aukslēju šķeltni esamība saistīta ar runas traucējumiem? Pētījuma mērķis: Izpētīt saistību starp runas traucējumu esamību un cieto aukslēju ar mīksto aukslēju šķeltni. Pētījuma metodes: Veikts kvantitatīvs neeksperimentāls gadījumu pētījums, kurā iekļauti 44 abu dzimumu pacienti, kas Rīgas Stradiņa universitātes Stomatoloģijas institūta Lūpu, aukslēju un sejas šķeltņu centrā reģistrējušies laika posmā no 2010. līdz 2020. gadam, un kuriem diagnosticēta cieto aukslēju šķeltne ar mīksto aukslēju šķeltni, kā arī veikti runas ieraksti. Pētītas pacientu slimības vēstures un veikta runas audioierakstu analīze. Starp iegūtajiem datiem tika noteiktas saistības. Pētījuma rezultāti: Pēc Fišera precīzā testa (Fisher’s Exact Test) konstatēta statistiski nozīmīga saistība starp hipernazalitāti un gaisa emisiju caur degunu pacientiem ar diagnosticētu cieto aukslēju ar mīksto aukslēju šķeltni (p=0,036). Starp cieto aukslēju šķeltni ar mīksto aukslēju šķeltni un ar šķeltni saistītiem skaņu izrunas traucējumiem pastāv statistiski nozīmīga saistība (Pearson Chi-Square Test: p=0,00). Fišera precīzais tests (Fisher’s Exact Test) neuzrādīja statistiski nozīmīgu saistību starp ar šķeltni saistītu un ar šķeltni nesaistītu skaņu izrunas traucējumu esamību (p=0,275), kā arī starp cieto aukslēju šķeltni ar mīksto aukslēju šķeltni un dzirdes traucējumiem saistība nav statistiski nozīmīga. Statistiski nozīmīga saistība netika konstatēta starp šķeltnes veidu (daļēja/pilnīga) un runas traucējumu izpausmēm (p=0,125; p=0,242; p=0,121; p=0,363). Secinājumi: Starp cieto aukslēju šķeltni ar mīksto aukslēju šķeltni, hipernazalitāti un gaisa emisiju caur degunu pastāv cieša saistība (p=0,036). Cieša saistība konstatēta starp cieto aukslēju ar mīksto aukslēju šķeltni un ar šķeltni saistītiem skaņu izrunas traucējumiem (80%). Cieša saistība netika konstatēta, tomēr secināts, ka pacientiem ar pilnīgu aukslēju šķeltni, visas analizējamās runas traucējumu pazīmes tiek konstatētas biežāk, nekā pacientiem ar daļēju cieto aukslēju un mīksto aukslēju šķeltni. Pētījuma dalībniekiem ar diagnosticētu cieto aukslēju šķeltni ar mīksto aukslēju šķeltni netika konstatēta cieša saistība ar dzirdes traucējumiem.

Topic: The commitment between hard cleft palate with soft cleft palate and speech disorders. Main question: Is the existence of a hard cleft palate with a soft cleft palate related to speech disorders? The aim of the study: To explore the commitment between the existence of speech disorders and a hard cleft palate with a soft cleft palate. Methods: A quantitative non-experimental case study was performed, including 44 patients of both sexes who were registered at the Lip, palate and facial cleft center of the Dentistry institute of Riga Stradins university in the period from 2010 to 2020, and who were diagnosed with a cleft hard palate with a cleft soft palate, also have been made voice recordings for all of the patients. Patients medical histories were examined and speech recordings were analyzed. Commitments were made between the obtained data. Results: The Fisher's Exact Test showed a statistically significant relationship between hypernasality and nasal air emission in patients with a diagnosed hard cleft palate with a soft cleft palate (p=0,036). Data indicated that there is a statistically significant relationship between the presence of a hard cleft palate with a soft palate and cleft-relates speech sound disorders (Pearson Chi-Square Test: p=0,00). Fisher's Exact Test showed a statistically insignificant commitment between cleft-related and non-cleft-related speech sound disorders (p=0,275), and also there was no statistically significant relationship between hard cleft palate with soft cleft palate and the hearing impairment. Statistically insignificant relationship was found between cleft palate type (incomplete/complete) and speech disorders (p=0,125; p=0,242; p=0,121; p=0,363). Conclusion: There is a close relationship between hard cleft palate and soft cleft palate, hypernasality and nasal air emission (p=0,036). A close association was found between hard cleft palate with soft cleft palate and cleft-related speech sound disorders (80%). Although no close association was found, it was concluded that in patients with complete hard cleft palate and soft palate, all signs of speech disorders were more common than in patients with incomplete hard cleft palate and soft cleft palate. Hard cleft palate with soft cleft palate do not form a close association with hearing impairments in the study population.
