Fizioterapijas integrēšana primārajā veselības aprūpē pacientiem ar muskuloskeletālām problēmām

Abstract

Bakalaura darba izstrādes mērķis bija noskaidrot fizioterapeitu viedokli par fizioterapijas integrēšanu primārajā veselības aprūpē pacientiem ar muskuloskeletālām problēmām. Tika veikts kvalitatīva dizaina pētījums, kas sastāv no individuālām daļēji strukturētām telefonintervijām. Intervijas tika ierakstītas un pēc tam tika veikta interviju transkribēšana. Datu analīze tika veikta pēc tematiskās analīzes metodes, ievērojot Brauna un Klārka sešu soļu principu. Pētījuma ietvaros tika intervēti vienpadsmit Latvijas praktizējošie fizioterapeiti, noskaidrojot viņu viedokli par fizioterapijas integrēšanu primārajā veselības aprūpē pacientiem ar muskuloskeletālām problēmām. No intervijā iegūtiem datiem iezīmējās piecas galvenās tēmas: 1) fizioterapija veselības aprūpes sistēmā Latvijā, 2) fizioterapeita kompetences darbā ar pacientiem ar muskuloskeletālām problēmām, 3) fizioterapijas integrēšana primārajā veselības aprūpē darbā ar pacientiem ar muskuloskeletālām problēmām, 4) primārās veselības aprūpes komanda, 5) fizioterapijas integrēšanu primārajā veselības aprūpē veicinošie faktori. Pētījumā tika iegūti secinājumi: 1) fizioterapijai ir potenciāls kļūt par būtisku primārās veselības aprūpes daļu, taču šobrīd iztrūkst izpratne par fizioterapiju veselības aprūpes speciālistu vidū un fizioterapeitu informētība par fizioterapijas integrēšanu primārajā veselības aprūpē, 2) priekšnoteikums fizioterapijas integrēšanai primārajā veselības aprūpē ir apjomīgs informatīvais/izglītojošais darbs, skaidrojot fizioterapijas un fizioterapeita kompetences, funkcijas un prasmes un potenciālo ieguldījumu primārajā veselības aprūpē pacientiem ar muskuloskeletālām problēmām, 3) fizioterapeitu iekļaušana primārās veselības aprūpes komandā ļautu uzlabot sabiedrības kopējos veselības rādītājus, veidot uz pacientu orientētu veselības aprūpi, veicinātu fizioterapijas pakalpojuma pieejamību un savlaicīgumu un padarīt primāro veselības aprūpi mērķtiecīgāku un efektīvāku pacientiem ar muskuloskeletālām problēmām.

The aim of the bachelor's research was to find out the opinion of physiotherapists about the integration of physiotherapy in primary health care for patients with musculoskeletal problems. A qualitative design study was conducted, consisting of individual semi-structured telephone interviews. The interviews were recorded and then transcribed. Data analysis was performed according to the method of thematic analysis, following the principle of six steps of Brown and Clark. Eleven Latvian physiotherapists were interviewed in the study, finding out their opinion on the integration of physiotherapy in primary health care for patients with musculoskeletal problems. Five main topics emerged from the interview data: 1) physiotherapy in the health care system in Latvia, 2) physiotherapist's competencies in working with patients with musculoskeletal problems, 3) integration of physiotherapy in primary health care in work with patients with musculoskeletal problems, 4) primary health care team, 5) factors contributing to the integration of physiotherapy into primary health care. The study concluded: 1) physiotherapy has the potential to become an important part of primary health care, but currently there is a lack of understanding about physiotherapy among health care professionals and physiotherapists' awareness of integrating physiotherapy in primary health care, 2) educational work explaining the competences, functions and skills of physiotherapy and physiotherapists and their potential contribution to primary health care for patients with musculoskeletal problems, 3) inclusion of physiotherapists in the primary health care team would improve general public health indicators and make primary health care more targeted and effective for patients with musculoskeletal problems.
