Abstract: | Darba tēma: Mīksto aukslēju šķeltnes, slēptas aukslēju šķeltnes un runas traucējumu korelācija.
Pētnieciskais jautājums: Vai mīksto aukslēju šķeltnes un slēptas aukslēju šķeltnes esamība korelē ar runas traucējumiem?
Pētījuma mērķis: Izpētīt mīksto aukslēju šķeltnes un slēptas aukslēju šķeltnes saistību ar runas traucējumiem.
Pētījuma metodes: Tika veikts kvalitatīvs neeksperimentāls gadījumu pētījums, kurā tika iekļauti 34 pacienti, kuri dzimuši laika posmā no 2005. gada līdz 2015. gadam, konsultējušies Rīgas Stradiņa universitātes Stomatoloģijas institūta Lūpu, aukslēju un sejas šķeltņu centrā, un kuriem diagnosticēta mīksto aukslēju šķeltne un/ vai slēpta aukslēju šķeltne, kā arī veikti runas ieraksti. Tika pētītas pacientu slimības vēstures, kā arī analizēti pacientu runas ieraksti. Starp iegūtajiem datiem tika noteikta saistība.
Pētījuma rezultāti: Pēc Pīrsona Hī kvadrāta testa (Pearson Chi-Square test) tika konstatēta statistiski nozīmīga saistība starp balss hipernazalitāti un gaisa emisiju caur degunu (p<0,001). Cieša korelācija tika pierādīta starp mīksto aukslēju šķeltnes, slēptas aukslēju šķeltnes esamību un balss hipernazalitātes un gaisa emisijas caur degunu esamību (Pearson Chi-Square: p<0,001). Statistiski nenozīmīgu saistību uzrādīja Fišera precīzais tests (Fisher’s exact test) starp mīksto aukslēju šķeltnes un slēptas aukslēju šķeltnes esamību un dzirdes uztveres traucējumiem (p=0,46), kā arī starp mīksto aukslēju šķeltni, slēptu aukslēju šķeltni un skaņu izrunas traucējumiem netika konstatēta nozīmīga saistība.
Secinājumi: Starp izolētu mīksto aukslēju šķeltni un mīksto aukslēju šķeltni kombinācijā ar slēptu aukslēju šķeltni, hipernazalitāti un gaisa emisiju caur degunu pastāv cieša saistība (p<0,001). Pacientiem, kuriem diagnosticēta mīksto aukslēju šķeltne kombinācijā ar slēptu aukslēju šķeltni, papildus šķeltnes izraisītiem skaņu izrunas traucējumiem, biežāk dzirdami arī ar šķeltni nesaistīti skaņu izrunas traucējumi, lai gan netika iegūta cieša korelācija. Pozitīvākie rezultāti pētījumā iekļautajai populācijai pēc veiktas aukslēju operācijas novēroti pacientiem, kuriem tā veikta vecumā no trim līdz deviņiem gadiem (87%). Izolētas mīksto aukslēju šķeltnes, mīksto aukslēju šķeltnes kombinācijā ar slēptu aukslēju šķeltni un dzirdes traucējumu saistība nav cieša pētījumā iekļautajai populācijai.

Main question: Does the presence of cleft soft palate and submucous cleft palate correlate with speech disorders? The purpose of the study: To explore the association of cleft soft palate, submucous cleft palate and speech disorders. Methods: A qualitative non-experimental case study was made, including 34 patients born between 2005 and 2015, consulted at the Lūpu, aukslēju un sejas šķeltņu centrs of the Dentistry institute of Riga Stradins university, who have been diagnosed with cleft soft palate and/ or submucous cleft palate, also the voice recordings has been made for all of the patients. Patients medical histories were studied and patients voice recordings were analyzed. Results: The Pearson Chi-Square test showed a statistically significant relationship between voice hypernasality and nasal air emission (p < 0.001). A close correlation was demonstrated between the presence of an isolated cleft soft palate, the presence of a cleft soft palate in combination with submucous cleft palate and the presence of voice hypernasality and air emission through the nose (Pearson Chi-Square: p < 0.001). A statistically insignificant relationship was demonstrated by Fisher's exact test between the presence of an isolated cleft soft palate, the presence of a cleft soft palate in combination with submucous cleft palate and the impairment of hearing perception (p = 0.46), as well as between the cleft soft palate, the submucous cleft palate, and the sound phonetic disorders were not significantly established relationship. Conclusion: There is a close relationship between the isolated cleft soft palate, cleft soft palate in combination with submucous cleft palate, hypernasality and the emission of air through the nose (p < 0.001). In patients who have been diagnosed with the cleft soft palate in combination with the submucous cleft palate, more frequently have speech sound disorders. The most positive results for the population included in the study following palate surgery were observed in patients aged three to nine years (87%). |
