Ķirurģiskās aprūpes māsu pieredze ķirurģiskās drošības kontrolsarakstu lietošanā

Abstract

Pacientu drošība katru gadu paliek arvien aktuālāks jautājums medicīnas nozarē. It īpaši tas ir attiecināms uz ķirurģiskās aprūpes nodrošināšanu. Māsa ir pirmais posms, kas nodrošina šo aprūpi un var pamanīt iespējamās kļūdas. Pasaules Veselības organizācijas izstrādātais ķirurģiskās drošības kontrolsaraksts ir veids kā samazināt riskus visā ķirurģiskās aprūpes procesā. Arī Latvijas ārstniecības iestādēs tas tiek izmantots. Bakalaura darbs tika izstrādāts laika posmā no 2020. gada janvāra līdz 2020. gada maijam. Pētījuma mērķis ir noskaidrot ķirurģiskās aprūpes māsu pieredzi ķirurģiskās drošības kontrolsarakstu lietošanas praksē. Tika izvirzīts pētījuma jautājums – kāda ir ķirurģiskās aprūpes māsu pieredze ķirurģiskās drošības kontrolsarakstu lietošanas praksē? Lai sasniegtu mērķi pētījuma veikšanai tika izmantota kvalitatīvā pētniecības metode, pielietojot daļēji strukturētu interviju, ar kuru tika intervētas 14 praktizējošas ķirurģiskās aprūpes māsas divās Rīgas ārstniecības iestādēs. Darba pirmajā teorētiskajā daļā tika apskatīts ķirurģiskās drošības kontrolsaraksta ieviešana, tā mērķis un struktūra, otrajā teorētiskajā daļā kontrolsarakstu lietošanas pieredze citās valstīs, to ieguvumi un sarežģījumi, kontrolsaraksta izmantošana bērnu ķirurģijā. Mērķis tika sasniegts un iegūta atbilde uz pētījuma jautājumu, ka aptuvenais ķirurģiskās aprūpes māsu ķirurģiskās drošības kontrolsarakstu lietošanas pieredze ir 2 līdz 3 gadi, kas norāda, ka pieredze māsām Latvijā atšķirsies no pieredzes citur pasaulē. Tika izstrādāti 11 secinājumi. Personāla pieredze pētījumā nav attiecināma uz visu pētījuma populāciju un neļauj spriest par pieredzes plašumu, niansēm citās veselības aprūpes iestādēs. Respondenti uzskata, ka kontrolsaraksts ir izveidots labi un nekas nav jāpilnveido tā lietošanā. Ietekme uz pacientu drošību, izmantojot kontrolsarakstu, veicina kļūdu, pēcoperācijas komplikāciju samazināšanos, uzlabo pacientu apmierinātību. Darba autores ieteikums cita pētījuma ietvaros veikt aizpildīto kontrolsarakstu analīzi, lai izpētītu, vai visas sadaļas tiek aizpildītas un, kurā posmā ir visvairāk pieļautās kļūdas. Darbs sastāv no 36 lappusēm, kur iekļautas 2 teorētiskā pamatojama nodaļas, 13 tabulas, 5 pielikumi. Izmantoti 25 literatūras avoti.

Patient safety in the medical field is becoming a more pressing matter each year. It is particularly notable in surgical care provision. A nurse is the first to provide such care and can notice possible mistakes. A surgical safety checklist issued by the World Health Organisation gives guidelines to reduce possible risks in the whole surgical care process. It is also being used in medical institutions in Latvia. The Bachelor's Paper was developed in the period from January 2020 to May 2020. The aim of the Paper is to determine the experience of the surgical care nurses in using the surgical safety checklist in practice. A research question was raised - what is the experience of surgical care nurses in using the surgical safety checklist in practice? To reach the aim to do the research a quality research method was used by developing a partly structured interview, interviewing 14 practicing surgical care nurses in two different medical institutions in Riga. In the first theoretical part of the Paper was viewed the surgical safety checklist's implementation - the aim and structure; in the second theoretical part of the Paper was viewed the surgical safety checklist's user experience in other countries, the benefits and drawbacks, the surgical safety checklist's usage in pediatric surgery. The aim of the Paper was achieved and the answer to the research question was obtained - the average surgical care nurse's surgical safety checklist user experience is 2 to 3 years - that shows that the experience of nurses in Latvia will differ from the nurses abroad. 11 conclusions were developed. The staff experience is not applicable to the whole population of the research and it does not allow to estimate the breadth of the experience and nuances in other medical care institutions. The respondents believe that the surgical safety checklist is made good and does not need any improvements in its use. The influence on the safety of the patients, using the surgical safety checklist, promotes decrease in after surgery complications and mistakes made, improves patient wellbeing. The author of the Paper suggests, during another research, to do a completed surgical safety checklist analysis, to understand if all the parts are completed and in which part there are the most errors made. The Paper consists of 36 pages that include 2 theoretical parts, 13 tables, 5 annexes. Used 25 literature sources.
