Abstract

ANOTĀCIJA Nosaukums: Novērtēšanas instrumenta “Rokas, pleca un plaukstas nespēja” īsās versijas latviešu valodā psihometrisko īpašību pārbaude pacientiem / klientiem ar karpālā kanāla sindromu. Ievads: Rokas funkciju traucējumi rada ietekmi uz ikdienas aktivitāšu veikšanu, kā arī dzīves kvalitātes samazināšanos. Pacientu ar dažādām saslimšanām un stāvokļiem novērtēšana rehabilitācijā ir ļoti svarīga sastāvdaļa terapijas mērķu izvirzīšanā un efektivitātes noteikšanā. Dažādas saslimšanas un stāvokļi var radīt rokas funkciju traucējumus. Karpālā kanāla sindroms (turpmāk tekstā KKS) ir izplatītākā kompresijas neiropātija. Aptuveni 90% pacientu, kuriem ir diagnosticēta neiropātija, diagnosticēts tieši karpālā kanāla sindroms. Tā kā ar katru gadu pieaug saslimstības ar KKS rādītāji, ir vērts pārbaudīt instrumenta “Rokas, pleca un plaukstas nespēja” īsās versijas latviešu valodā psihometriskos rādītājus pacientiem / klientiem ar karpālā kanāla sindromu, lai ergoterapeiti varētu pielietot to savā praksē kā skrīninga instrumentu, kas taupīs laiku un identificēs rokas funkcijas grūtības. Pētījuma mēŗkis: Pārbaudīt novērtēšanas instrumenta “Rokas, pleca un plaukstas nespēja” īsās versijas latviešu valodā psihometriskos rādītājus (iekšējo saskaņotību, testa – atkārtota testa pārbaudi) pacientiem / klientiem ar karpālā kanāla sindromu. Pētījuma metodes: kvantitatīvs, neeksperimentāls pētījums, aprakstošā statistika. Lai noskaidrotu psihometriskās īpašības, tika noteikta iekšējā saskaņotība ar Kronbaha α koeficientu (Cronbach’s α) un testa-atkārtota testa ticamība, izmantojot starpklašu korelācijas koeficientu (intraclass correlation coefficient - ICC). Dalībnieki: pacienti / klienti ar apstiprinātu diagnozi “karpālā kanāla sindroms” (G56.0 pēc SSK-10 klasifikācijas) un kuri saņem rehabilitācijas pakalpojumus sabiedrībā ar ierobežotu atbildību "SANARE-KRC JAUNĶEMERI", sociālās rehabilitācijas pakalpojumus Sociālās integrācijas valsts aģenturā bez dzimuma ierobežojuma, vismaz 18 gadus veci. Saprot un runā latviešu valodā (brīvi pārvalda latviešu valodu), piekrīt dalībai pētījumā. Rezultāti: Novērtēšanas instrumenta “Rokas, pleca un plaukstas nespēja” kopējā iekšējā saskaņotība ir ļoti laba (Kronbaha α = 0,968). Kopējais starpklašu korelācijas koeficients (ICC) ir 0,921, kas norāda uz ļoti labu testa-atkārtota testa ticamību. Arī 95% ticamības intervāls ir robežās no 0,881 līdz 0,949, kas norāda uz labu - teicamu ticamību. Secinājumi: Novērtēšanas instruments ir sagatavots latviešu valodā. Visi iegūtie rezultāti norāda, ka instrumenta īsajai versijai latviešu valodā ir augsti ticamības (ICC = 0,921) un iekšējās saskaņotības rezultāti (Kronbaha α = 0,968). Tika secināts, ka jāturpina veikt psihometrisko īpašību pārbaudi lielākam personu skaitam, kā arī personām ar citām augšējo ekstremitāšu saslimšanām / stāvokļiem.

ABSTRACT Title:Psychometric properties of Latvian version of “Disabilities of the Arm, Shoulder and Hand” (TheQuick DASH) short version in patients / clients with carpal tunnel syndrome. Introduction: Impaired hand functions affect the performance of daily activities and also reduce the quality of life. Assessment of patients with various illnesses and conditions in rehabilitation is a very important part of setting therapy goals and determining effectiveness. Various illnesses and conditions can cause impairment of hand function. Carpal tunnel syndrome (CTS) is the most common compression neuropathy. Approximately 90% of patients diagnosed with neuropathy are diagnosed with carpal tunnel syndrome. Since the incidence rates of CTS are increasing every year in Latvia, it is worth checking the psychometric properties of the assessment tool "Disabilities of the Arm, Shoulder and Hand" (TheQuick DASH) short version so that occupational therapists can use it in their practice as a screening tool, which will save time and identify hand function difficulties. The aim of the research: To evaluate the psychometric properties (internal consistency reliability, test-retest reliability) of the short Latvian version of the assessment tool "Disabilities of the Arm, Shoulder and Hand” (TheQuick DASH) in patients/ clients with carpal tunnel syndrome. Research methods: Quantitative, non-experimental study, descriptive study. To clarify the psychometric properties, internal coherence with the Cronbach α coefficient and the reliability of the test-retest method using the intraclass correlation coefficient (ICC) was established. Participants: patients/ clients diagnosed “carpal tunnel syndrome" (G56.0 ISC-10 classification) and who receive rehabilitation services at the Rehabilitation center "Jaunķemeri", social rehabilitation services at the Social Integration State Agency. Participants are at least 18 years old, and able to understand and talk in Latvian, without gender restriction. Results: The internal consistency of the self-assessment questionnaire is excellent (Cronbach α = 0.968). The total interclass correlation coefficient (ICC) was 0.921, indicating excellent test-retest reliability. Conclusions: The obtained results indicate that the Latvian short version of “Disabilities of the Arm, Shoulder and Hand” (TheQuick DASH) has high reliability and internal coherence results, but it is necessary to continue testing the psychometric properties of a larger number of patients / clients with other diseases/conditions of the upper extremities too.
