Fugl- Meyer apakšējās ekstremitātes novērtējuma daļas ticamības pārbaude fizioterapeitu praksē

Abstract

Nosaukums: Fugl – Meyer apakšējās ekstremitātes novērtējuma daļas latviešu valodas versijas ticamības pārbaude fizioterapeitu praksē. Ievads: Fugl – Meyer novērtējums ir viens no visplašāk izmantotajiem novērtējumiem ar augstu ticamības līmeni, kas ļauj novērtēt sensorimotoro traucējumu smaguma pakāpi personām pēc saslimšanas ar insultu. Šobrīd Latvijā ir validētas Fugl – Meyer augšējo ekstremitāšu novērtējuma B un C daļas, tulkotas Fugl – Meyer augšējās ekstremitātes novērtējuma A, D, H, I un J daļas, ko ergoterapeiti pielieto savā praksē. Pēc Fugl – Meyer apakšējo ekstremitāšu novērtējuma E, F, H, I un J daļas latviešu valodas versijas izveides, fizioterapeitiem būs iespēja izmantot standartizētu, validētu mērījumu latviešu valodā. Pētījuma mērķis: Pārbaudīt Fugl – Meyer apakšējās ekstremitātes novērtējuma E, F, H, I un J daļas latviešu valodas versijas ticamību fizioterapeitu praksē. Pētījuma metodes: Kvantitatīvs, neeksperimentāls, aprakstošs pētījums. Pētījuma procesa laikā tika veikta Fugl – Meyer apakšējās ekstremitātes novērtējuma E, F, H, I un J daļas sagatavošana latviešu valodā, fizioterapeitu iepazīstināšana un apmācība par novērtējuma pielietošanu praksē. Novērtējuma ticamības pārbaudei tika izmantots tests – atkārtots tests, iekšējā vērtētāja ticamība pēc starpklašu korelācijas koeficienta, iekšējā saskaņotība, izmantojot Kronbaha alfa koeficientu un starpvērtētāju ticamība pēc Cohen’s Kappa koeficienta. Dalībnieki: Pētījumā tika iesaistītas divas pētījuma dalībnieku grupas: 14 pacienti pēc pirmreizējas saslimšanas ar insultu un seši praktizējoši fizioterapeiti, ar vismaz viena gada pieredzi strādājot ar personām pēc saslimšanas ar insultu. Novērtēšanas instrumenti: Fugl – Meyer apakšējās ekstremitātes novērtējuma E, F, H, I un J daļas latviešu valodas versija, kas izveidota pēc tulka un pētnieka veikto tulkojumu versiju sintezēšanas procesa. Rezultāti: Fugl – Meyer apakšējās ekstremitātes novērtējuma E, F, H, I un J daļām ir augsts ticamības līmenis. Novērtējuma tests – atkārtots tests un iekšējā vērtētāja ticamība pēc ICC variēja no 0,955 līdz 0,999, iekšējā saskaņotība pēc Kronbaha alfa koeficienta variēja no 0,63 līdz 0,75, starpvērtētāju ticamība pēc Cohen’s Kappa koeficienta no 0,89 līdz 1,00. Secinājumi: “Fugl – Meyer apakšējās ekstremitātes novērtējuma E, F, H, I un J daļas” latviešu valodas versija uzrāda augstus psihometriskos rādītājus. Novērtējumu var pielietot fizioterapeita praksē Latvijā personām pēc saslimšanas ar insultu.

Title: The Fugl – Meyer assessment of lower extremity reliability testing of the Latvian language version in the practice of physiotherapists. Introduction: The Fugl-Meyer assessment is one of the most widely used assessments with high reliability for assessing the severity of sensorimotor impairment in individuals who have had stroke. Currently, parts B and C of the Fugl - Meyer upper limb assessment has been validated, parts A, D, H, I and J has been translated in Latvian. After the development of the Latvian version of the Fugl-Meyer lower limb assessment parts E, F, H, I and J, physiotherapists will be able to use a standardised and validated measurement in Latvian. Objective of the study: To verify The Fugl – Meyer assessment lower extremity parts for reliability testing of the Latvian language version in the practice of physiotherapists. Research methodology: Quantitative, non-experimental, descriptive study. During the research process, the Latvian language version of The Fugl-Meyer assessment lower extremity parts were prepared, physiotherapists were introduced and trained about the assessment and how to use it in practice. Test-retest, intra-rater reliability by interclass correlation coefficient, internal consistency by Cronbach's alpha and inter-rater reliability by Cohen's Kappa coefficient were used to test the reliability of the assessment. Participants: In the study were involved two groups of participants: 14 patients after a first stroke and six practising physiotherapists with at least one year of experience working with people after a stroke. Assessment tools: The Latvian version of The Fugl - Meyer assessment lower extremity E, F, H, I and J parts who has created after a process of synthesising the translated versions by the translator and the researcher. Results: The Fugl - Meyer assessment lower extremity E, F, H, I and J parts rating has a high level of reliability. Assessment tool test - retest and intra – rater reliability to the ICC ranged from 0.955 to 0.999, internal consistency according to the Cronbach's alpha coefficient ranged from 0.63 to 0.75, inter – rater reliability to the Cohen's Kappa coefficient ranged from 0.89 to 1.00. Conclusion: The Latvian version of The Fugl - Meyer assessment lower extremity E, F, H, I and J parts shows high psychometric scores. The assessment can be used in physiotherapy practice in Latvia for people after stroke.
