Abstract: | Pētījuma nosaukums: Sāpju neirozinātniskā skaidrojuma (no angļu valodas Pain Neuroscience
Education (PNE)) lietojums fizioterapeita praksē pacientiem ar hroniskām nespecifiskām
muguras lejasdaļas sāpēm.
Pētījuma mērķis: Izpētīt kā fizioterapeiti pacientiem ar hroniskām nespecifiskām muguras
lejasdaļas sāpēm izprot un lieto sāpju neirozinātnisko skaidrojumu (PNE).
Pētījuma metodes: Pētījums veidots pēc kvantitatīvās stratēģijas vienmomenta aprakstoši
analītiskā pētījuma dizaina. Pētījuma izlasi veidoja 59 dalībnieki. Lai iegūtu datus par
praktizējošu fizioterapeitu un ceturtā kursa studentu zināšanām sāpju neirofizioloģijā,
līdzšinējo prakses modeli, kā arī attieksmi un pārliecību darbā ar hronisku nespecifisku muguras
lejasdaļas sāpju pacientiem tika izstrādāts strukturēts aptaujas protokols, kas elektroniskā veidā
izplatīts internetā. Iegūtie dati apkopoti MS Excel un datu analīze veikta, izmantojot SPSS
programmatūru.
Pētījuma rezultāti un secinājumi: Pētītās izlases dalībnieki uzrādīja relatīvi zemas zināšanas
sāpju fizioloģijā, kas neatšķīrās apakšgrupās pēc līdzšinējās izglītības, kas norāda uz
nepieciešamību mācību programmā ieviest specifiskus algoloģijas kursus, tādējādi veidojot
kompetenci sāpju neirozinātniskā skaidrojuma lietojumam klīniskajā darbā. Pētījuma izlases
dalībnieku atbildes apstipināja, ka praksē izglītojot pacientus ar hroniskām nespecifiskām
muguras lejasdaļas sāpēm biežāk dominē biomedicīniskā pieeja, kas īpaši raksturīgi 4. kursa
studentiem un neatbilst esošajām vadlīnijām un klīniskās prakses rekomendācijām hronisku
muskuloskeletālu sāpju ārstēšanā, taču bakalaura un maģistra izglītību ieguvušiem
fizioterapeitiem novērojama prakses modeļa maiņa biopsihosociālā modeļa virzienā. Pētījuma
izlases dalībnieki, neatkarīgi no līdzšinējās izglītības, 86,5% gadījumos pauž pozitīvu attieksmi
un pārliecību par nepieciešamību izmantot sāpju re-konceptualizācijas pieeju, t.i., skaidrojot
hroniskas muguras sāpes, kā pārmērīgu uzbudināmības stāvokli un neiroplastiskas izmaiņas
perifērajā un centrālajā nervu sistēmā, tomēr tikai 6,8% jūtas pārliecināti par savām zināšanām
un prasmi praksē lietot sāpju neirozinātnisko skaidrojumu, kas kopā ar relatīvi zemām
zināšanām aktualizē būtisku barjeru sāpju neirozinātniskā skaidrojuma lietošanai klīniskajā
praksē. Pētījuma rezultāti apstiprināja realizētā prakses modeļa saistību ar attieksmi un
pārliecību, kas atkārtoti aktualizē mērķtiecīgus pasākumus gan līdzdiploma, gan pēcdiploma
fizioterapeitu izglītībā. Title: Application of Pain Neuroscience Education (PNE) in physiotherapy practice for patients with chronic non-specific low back pain. Objective: To investigate how physiotherapists in patients with chronic non-specific low back pain understand and use the pain neuroscience education (PNE). Methods: The study is based on the single-moment descriptive analytical research design of the quantitative strategy. The study sample consisted of 59 participants. To obtain data on the knowledge of practicing physiotherapists and fourth-year students about neurophysiology of pain, the current practice model, as well as the attitude and confidence in working with chronic nonspecific low back pain patients, a structured survey protocol was developed and distributed electronically on the internet. The data obtained were collected in MS Excel and data analysis was performed using SPSS software. Results and conclusion: The participants in the studied sample showed relatively low knowledge of pain physiology, which did not differ in subgroups according to previous education, which indicates the need to introduce specific algology courses into the curriculum, thus creating competence for the use of the pain neuroscience education in clinical work. The responses of the study sample participants confirmed that in practice, when educating patients with nonspecific chronic low back pain, the biomedical approach is more often dominant, which is especially characteristic of fourth-year students and does not comply with existing guidelines and clinical practice recommendations in the treatment of chronic musculoskeletal pain, but physiotherapists with bachelor's and master's degree can observe the change of practice model towards the biopsychosocial model. Participants in the research sample, regardless of previous education, in 86,5% of cases express a positive attitude and belief in the need to use a pain reconceptualisation approach, that is, to explain chronic back pain as a state of excessive excitability and neuroplastic changes in the peripheral and central nervous system; however, only 6,8% feel confident in their knowledge and ability to use the pain neuroscience education in practice, which, together with relatively low knowledge, raises a significant barrier to the use of the pain neuroscience education in clinical practice. The results of the study confirmed the relationship of the implemented practice model with attitude and confidence, which repeatedly brings to the fore the importance of targeted initiatives in the undergraduate and postraduate education of physiotherapists. |
