Abstract

Maģistra darba tēma ir “Farmaceitiskās aprūpes loma un nozīme pacientvērstas veselības aprūpes īstenošanā”. Pieaugot sabiedrības novecošanās tendencēm, palielinās arī neracionālas medikamentu lietošanas problēmas aktualitāte. Tas piešķir ar vien lielāku nozīmi pacientvērstas veselības aprūpes modeļa attīstības nepieciešamībai. Ņemot vērā, ka farmaceits ir pieejamākais veselības aprūpes speciālists, šāda veselības aprūpes koncepta attīstībā arī ikdienas aptiekas praksē var palīdzēt sasniegt pacientu zāļu terapijas mērķus un uzlabot pacientu līdzestību terapijai un veselībpratību kopumā. Līdz ar to šī pētījuma mērķis ir novērtēt farmaceitiskās aprūpes integrācijas iespējas pacientvērstas aprūpes konceptā, ņemot vērā pacientu apmierinātību ar esošo farmaceitisko aprūpi un nepieciešamību nodrošināt pacientiem informāciju par pareizu un drošu medikamentu lietošanu. Pētījuma ietvaros tiek apskatīti iespējamie risinājumi farmaceitiskās aprūpes uzlabošanai, lai tā atbilstu pacientvērstas aprūpes konceptam, noteikta šī brīža apmierinātība ar farmaceitisko aprūpi un saņemto informāciju par medikamentu lietošanu. Balstoties uz literatūras apskatu un iegūtajiem rezultātiem, tiek izvērtēta nepieciešamība pēc jauniem aptieku sniegtiem pakalpojumiem, lai uzlabotu pacientu apmierinātības līmeni. Tā kā pacientvērstas aprūpes modelis paredz sadarbību starp veselības aprūpes speciālistiem, tiek identificēti šķēršļi tās veicināšanai starp farmaceitiem un ģimenes ārstiem. Pacientu anketēšanas rezultātā tika noteikts augsts apmierinātības līmenis ar aptiekas sniegumu un kopējo farmaceitisko aprūpi, bet ir identificēta vidēja apmierinātība ar saņemto informāciju par pareizu un drošu medikamentu lietošanu. Tās uzlabošanai kā piemērotākais veids, kā attīstīt farmaceitisko aprūpi, ir zāļu terapijas pārskata ieviešana, kas nodrošinātu arī pacientvērstas aprūpes mērķu sasniegšanu. Intervijās ar farmaceitiem un ģimenes ārstiem tiek rastas iespējas sadarbības uzlabošanai, kā arī šobrīd esošie šķēršļi zāļu terapijas pārskata attīstībai. Paidere, Madara. The theme of the master’s thesis is “The role and importance of pharmaceutical care in the implementation of patient – centered health care”. As the aging of the population grows, so does the urgency of the problem of irrational use of medications. This attaches increasing importance to the need to develop a patient – centered healthcare model. Given that the pharmacist is the most accessible healthcare professional, the development of such a healthcare concept can also help to achieve the goals of the patient’s medication treatment and improve medication adherence and health literacy in general. Therefore, this study aims to evaluate the possibilities of integrating pharmaceutical care into the concept of patient – centered care, taking into account patients’ satisfaction with existing pharmaceutical care and the need to provide patients with information on the appropriate and safe use of medicines. Within the framework of the research, possible solutions for the improvement of pharmaceutical care are considered to comply with the concept of patient-based care, the current satisfaction with pharmaceutical care and the received information on the use of medicines has been determined. Based on the literature review and the results obtained, the need for new services provided by pharmacies to improve patient satisfaction is assessed. As the patient – centered care model involves collaboration between healthcare professionals, barriers to its promotion are identified between pharmacists and GPs. As a result of the patient survey, a high level of satisfaction with performance of the pharmacy and overall pharmaceutical care was determined, but a medium level of satisfaction with the received information on the appropriate and safe use of medicines was identified. To improve it, the most appropriate way to develop pharmaceutical care is to introduce a medication use review, which would also ensure the achievement of the goals of patient – centered care.
