Abstract: | Zarnu invaginācija ir stāvoklis, pie kura zarnu siena ieliecās lumenā, kas noved pie zarnu
obstrukcijas. Visbiežāk šis stāvoklis ir novērojams resnajās un tievajās zarnās. Tas ir biežs akūtā
vēdera iemesls, it īpaši mazajiem bērniem, un nereti noved pie smagām komplikācijām. Šis
stāvoklis var tikt ārstēts konservatīvi, neoperatīvi ar pneimatisko vai hidrostatisko redukciju vai
operatīvi ar zarnu rezekciju vai manuālo redukciju, kuru veic laparotomiski vai laparoskopiski.
Šī literatūras apskata uzmanības centrā ir pētījumi, kuros ir apskatītas dažādas zarnu invagināciju
ārstēšanas metodes bērniem. Dati ir iegūti no dažādām datubāzēm un apstrādāti. Statistiskie dati ir
salīdzināti, it īpaši dažādu metožu sekmīgums un to iemesli. Kopumā ir analizēti 669 pētījumi, 106
apskatīti, un analīzē ir iekļauti dati no 24 pētījumiem.
Neoperatīva zarnu invaginācijas ārstēšana bērniem ir droša un efektīva pirmās līnijas metode
idiopātiskai invaginācijai. Ķirurģija ir pielietojama akūta vēdera vai radioloģisko patoloģisko
pazīmju gadījumos. Vislabākie iznākumi operativai ārstēšanai ir Eiropā, bet neoperatīvai ārstēšanai
Ziemeļāfrikā vai Tuvajos Austrumos. Laika skalas datu analīze rāda apšaubāmus rezultātus un
nerāda veiksmīgas ārstēšanas metodes periodā no 1976. līdz 2019. gadam. Neoperatīvās ārstēšanas
metodes bija uzlabotas pēdējo divu desmitgadu laikā, bet operatīvās ārstēšanas rezultāti ir
pasliktinājušies. Šī statistika var nebūt uzticama neekvivalenta datu apjoma dēļ. Intussusception is a condition in which intestine folds inside itself, leading to bowel obstruction. It usually occurs in large or small bowel and is a relatively common cause of acute abdomen especially in toddlers and may lead to severe complications. The condition may be treated conservatively by follow-up, non-operatively with pneumatic or hydrostatic reduction or surgically by bowel removal or manual reduction performed in laparotomy or laparoscopy. This literature review focuses on studies that have gathered data of different intussusception treatment methods with children. The data is gathered from different databases and then evaluated. The statistical data is put together and the differences and successfulness between different methods are compared and reasons for differences are questioned. Total of 669 studies were evaluated, 106 studies screened and the final review analysis contains data from 24 studies. Non-operative management of intussusception in children is useful and safe technique as first-line treatment for idiopathic intussusception. Surgery should be considered for patients with acute abdominal symptoms or pathological leading points found during radiological studies. Operative management is most likely successful in Europe and non-operative reduction succeeds most likely in North-Africa or Middle east. Timeline data analysis gives questionable results and shows no significant chances on successfulness of different treatment methods during 1976 and 2019. Nonoperative management seems to have improved during last two decades while surgical data shows decreasing successfulness. These statistics may not be reliable due to non-equivalent amount of data. |
