Psychological Impact of Work in Palliative and Hospice Care on Healthcare Staff

Abstract

Ievads. Paliatīvā un hospisa aprūpe ir medicīnas joma, kas cieši nodarbojas ar nāves un miršanas jautājumu, kas ir ļoti emocionāls temats daudziem cilvēkiem. Daudzi baidās nomirt paši un viņus dziļi satrauc tuvinieka nāve. Šajās jomās strādājošie īpaši bieži saskaras ar mirstošiem cilvēkiem, radot jautājumu, vai tas palielina risku saslimt ar nozīmīgām garīgās veselības slimībām. Mērķis. Šī darba mērķis ir noskaidrot, vai nodarbināšanai paliatīvajā un hospisa aprūpē ir kādas būtiskas negatīvas sekas uz garīgo veselību. Materiāli un metodes. Rakstisks literatūras apskats, kurā tika izmantotas esošās zinātniskās publikācijas un pētījumi, lai izvērtētu hipotēzi, vai darbs paliatīvajā veselības aprūpē un bieža saskare ar cilvēku nāvi hospisa aprūpē var ietekmēt cilvēka psihi. Tika izmantotas datu bāzes PubMed, ProQuest un SAGE Journals, kā arī Clinical Key un Access Medicine. Meklēšanas kritēriji ietvēra pētījumu rakstus, gadījumu ziņojumus un datu rakstus. Rezultāti. Medicīniskais personāls, kas strādā paliatīvajā un hospisa aprūpē, savas profesijas dēļ saskaras ar daudzām garīgās veselības problēmām. Visizplatītākie garīgās veselības traucējumu cēloņi ir strukturālas un organizatoriskas problēmas, kam seko stresa faktori, kas saistīti ar pacientu nāvi. Stress un citas garīgās veselības problēmas, kas saistītas ar pacientu nāvi, neskatoties uz biežāku iedarbību, paliatīvajā medicīnā un hospisa aprūpē nav lielākas, salīdzinot ar citām veselības aprūpes specialitātēm. Tas varētu būt saistīts ar faktu, ka cilvēki, kas strādā paliatīvajā un hospisa aprūpē, ir garīgi sagatavoti pacienta nāvei viņu aprūpē un tādējādi ir labāk psiholoģiski gatavi, lai tiktu galā ar zaudējumiem. Problēmas, ar kurām saskaras paliatīvās un hospisa aprūpes personāls, ir sākot no emocionāla izsīkuma un līdzjūtības noguruma līdz depresijai un pašnāvības domām. Tiek ziņots, ka pareizās vadības stratēģijas palīdz uzturēt garīgo labklājību un novērst smagas sekas darbiniekiem un pēc tam arī viņu pacientiem. Secinājumi. Kā gaidīts, psiholoģisko traucējumu līmenis paliatīvās medicīnas un hospisa aprūpes personāla vidū ir izrādījies diezgan augsts, un tas skar līdz pat pusei no visiem darbiniekiem. Taču iemesls garīgās veselības traucējumu augstajai izplatībai ir pretrunā ar iepriekš izvirzīto hipotēzi – paliatīvās un hospisa aprūpes personāla garīgo labsajūtu visvairāk ietekmē darba vide un organizācija, nevis bieža saskare ar pacientu nāvi.

Background. Palliative and hospice care is an area of medicine which deals closely with the matter of death and dying, a very emotional subject for many people. Many are afraid of dying themselves and are deeply disturbed by the death of a loved one. Employees working in these areas are confronted with dying people on particularly frequent basis, raising the question whether this increases the risk of suffering from significant mental health diseases. Aim. The aim of this paper is to examine whether being employed in palliative and hospice care has any significant negative consequences on the mental health. Material and methods. A written literature review where existing publications and research were utilized to test the hypothesis that working in palliative healthcare and being subjected to death so closely via hospice care can have a bearing on a person’s psyche. PubMed, ProQuest data bases and SAGE Journals, as well as Clinical Key and Access Medicine were utilized. Search criteria included research articles, case reports and data articles. Results. Medical personnel working in palliative and hospice care face many mental health challenges due to their profession. The most common causes of mental health issues are structural and organisational problems, followed by stressors related to patient deaths. Compared to other specialities in healthcare, stress, and other mental health problems due to dying patients are not increased in palliative medicine and hospice care, despite more frequent exposure. This could be due to the fact that people working in palliative and hospice care are mentally prepared for a patient to die under their care and are thus better equipped to deal with the loss than employees of other medical specialties. Issues palliative and hospice care personnel deals with ranges from emotional exhaustion and compassion fatigue to depression and suicidal ideation. Proper management strategies are reported to be helpful for the upkeep of mental well-being and prevention of severe consequences for employees and subsequently their patients. Conclusions. As expected, the rate of psychological disorders in palliative medicine and hospice care personnel has shown to be quite high, affecting up to half of all employees. However, the reason for the high prevalence of mental health disorders in palliative and hospice care employees, contradicts the hypothesis previously named; rather than the main causative factor being the frequent exposure to dying patients, it is factors pertaining the work environment which influence palliative and hospice care staff’s mental well-being the most.
