Balss traucējumu novērtēšanas instrumentu satura un psihometrisko īpašību salīdzinājums: sistemātisks literatūras pārskats

Abstract

Darba nosaukums: Balss traucējumu novērtēšanas instrumentu satura un psihometrisko īpašību salīdzinājums: sistemātisks literatūras pārskats Pētāmā problēma: Šobrīd Latvijā tiek izmantoti daži novērtēšanas instrumenti, lai izvērtētu balss traucējumus, kas palīdz identificēt pacientus ar balss traucējumiem, taču nav pieejams vienots instruments un speciālistu darbības plāns šo pacientu turpmākai izvērtēšanai. Lai novērtētu, pacientu ar balss traucējumiem vajadzības, ir nepieciešams specifisks novērtēšanas instruments. Darba mērķis: Noskaidrot biežāk izmantotos balss traucējumu novērtēšanas instrumentus, salīdzināt to saturu, izmantojot Starptautisko funkcionēšanas, nespējas un veselības klasifikāciju (SFK) un psihometriskās īpašības. Pētījumu uzdevumi: Noteikt biežāk izmantotos balss traucējuma novērtēšanas instrumentus pacientiem ar balss traucējumiem, veikt atrasto novērtēšanas instrumentu elementu sasaisti ar SFK kategorijām vadoties pēc SFK sasaistes vadlīnijām un salīdzināt biežāk izmantoto balss traucējumu novērtēšanas instrumentu psihometriskās īpašības. Pētījumu rezultāti: Sistemātiskajā literatūras pārskatā iekļauti 91 pētījumi, kuros identificēti 57 balss novērtēšanas instrumenti. No identificētajiem instrumentiem tika analizēti 12 biežāk izmantotie (The Voice Handicap Index (VHI), Videostroboscopy evaluation, GRBAS scale , Acoustic analysis, Vocal self-assessment protocol VoiSS, Consensus Auditory-Perceptual Evaluation of Voice (CAPE-V), Endoscopy, Vocal self-assessment protocols (VTDS), Registration of voice range profile (VRP), Maximum phonation time (MPT) and type of breathing, , Laryngeal palpatory evaluation (LPE). Biežāk izmantotajiem instrumentiem tika veikta sasaiste ar 37 SFK kategorijām. Sasaistīto elementu pārsvars vērojams Voice Handicap Index, VoiSS un GRBAS scale. Ķermeņa funkciju kategoriju pārsvars identificēts Voice Handicap Index , VoiSS, GRBAS scale, ķermeņa struktūru domēnā VoiSS, Videostroboscopy, Endoscopy, aktivitātes un dalības kategorijās Voice Handicap Index, GRBAS scale un VoiSS, bet vides kategorijās VHI, VoiSS un V-RQOL instrumentiem. Psihometriskās īpašības tika vērtētas tādiem instrumentiem kā VHI, GRBAS scale, CAPE-V, VTDS, V-RQOL, MFT, Acoustic analysis, VoiSS un VRP. Uzticamība un pamatotība pierādīta VHI, GRBAS scale, VTDS, V-RQOL un VoiSS. Netika identificētas instrumentu Videostroboscopy evaluation, Endoscopy, Maximum phonation time (MPT) psihometriskās īpašības. Secinājumi: Darba ietvaros tika identificēti 12 biežāk izmantotie balss traucējumu novērtēšanas instrumenti, kas vērtē problēmas visās SFK komponentēs un daļa no tiem ir uzticami un pamatoti.

Title: Analysis on the content and psychometric properties of the assessment tools for voice disorders: systematic literature review. Surveyed problem: In Latvia there are only a few screening tools available that help identify patients with voice disorders. However, there is no joint tool available to help with future evaluations regarding these groups of patients. A specific assessment tool is imperative to evaluate the needs of patients with voice disorders. Aim: Find out the most commonly used assessment tools for voice disorders and compare their content using the International Classification of Functioning, Disability and Health (ICF) psychometric properties. Research tasks: To identify the most frequently used voice impairment assessment tools for patients with voice impairment, link the elements of assessment tools to ICF categories following the ICF linking guidelines, and to compare the psychometric properties of the most commonly used voice assessment tools. Results: 91 clinical studies which identified 57 voice disorder assessment tools were included in the systematic literature review. Further analysis was applied to 12 of the more common assessment tools The Voice Handicap Index (VHI), Videostroboscopy evaluation, GRBAS scale, Acoustic analysis, Vocal self-assessment protocol VoiSS, Consensus Auditory-Perceptual Evaluation of Voice (CAPE-V), Endoscopy, Vocal self-assessment protocols (VTDS), Registration of voice range profile (VRP), Maximum phonation time (MPT) and type of breathing, Laryngeal palpatory evaluation (LPE). Meaningful concepts which were identified were linked with 37 ICF categories. The predominance of linked elements can be observed in Voice Handicap Index, VoiSS and GRBAS scale. Predominance of bodily function categories identified in Voice Handicap Index , VoiSS, GRBAS scale , in the domain of body structures VoiSS, Videostroboscopy, Endoscopy, in activities and participation categories Voice Handicap Index, GRBAS scale and VoiSS, and environmental factors VHI, VoiSS and V-RQOL assessment tools. Psychometric properties were evaluated for assessment tools VHI, GRBAS scale, CAPE-V, VTDS, V-RQOL, MFT, Acoustic analysis, VoiSS and VRP. reliability and validity were proved in VHI, GRBAS scale, VTDS, V-RQOL and VoiSS. Psychometric properties for Videostroboscopy evaluation, Endoscopy, Maximum phonation time (MPT) were not identified. Conclusions: In this research, 12 of the more commonly used voice disorder assessment tools content were compared using the ICF and their psychometric qualities were identified.
