Temporomandibularas locītavas artroskopija

Abstract

Temporomandibulāras locītavas (TML) disfunkciju un sāpju etioloģija ir ļoti multifaktoriāla un to ārstēšana ir liels izaicinājums dažādu specialitāšu ārstiem. TML slimību ārstēšanai ir pieejamas dažādas metodes: artrocentēze, artroskopija, artrotomija, un medikamentoza terapija. Pētnieciskā darba mērķis ir apkopot informāciju par artroskopijas pielietošanu pacientiem ar temporomandibulāras locītavas saslimšanām; aprakstīt artroskopijas tehniku, aprīkojumu un klīnisko pielietojumu. Analizējot uz pieradījumiem balstīto literatūru, aprakstīt šīs metodes priekšrocības, trūkumus un salīdzināt ar citām ārstēšanas iespējam. Tika veikta literatūras atlase angļu valodā, izmantojot datu bāzes PubMed, EBSCO, Dyna Med, ka arī veicot manuālu meklēšanu RSU bibliotēkas pieejamos zinātniskus resursos. Pēc literatūras atlases pētījumā tika iekļauti 39 raksti un 5 grāmatas. TML artroskopiju var pielietot pacientiem ar deģeneratīvam locītavu saslimšanam sakumstadijās, locītavas diska dislokāciju, sāpju disfunkcijas sindromu, sinovītu ka arī intraartikulāru saaugumu gadījumos. Diagnostiskā artroskopija kā vienīgā manipulācija mūsdienas netiek rekomendēta, to ieteicams kombinēt ar terapeitisko artroskopiju vai locītavas lavāžu. Locītavas artroskopija kā jebkura cita invazīva metode var sekmēt komplikāciju attīstību. Literatūrā visbiežāk tiek pieminētas sekojošas komplikācijas: pēcoperācijas tūska, hematoma, nervus facialis bojājums, instrumenta salūšana, infekcija, otoloģiskas un retāk kardioloģiskas komplikācijas. Locītavu artroskopiskās procedūras pacientiem ar TML traucējumiem ir drošas un efektīvas. Izmantojot šo metodi, samazinās pēcoperāciju komplikāciju risks un pacienta atveseļošanās periods

The etiology of temporomandibular joint (TMJ) dysfunction and pain is multifactorial, and treatment of these issues is a considerable challenge for doctors of various specialities. There are several methods for treating TMJ disorders, namely arthrocentesis, arthroscopy, arthrotomy and pharmacotherapy. The aim of the research is: to compile information on the use of arthroscopy in treatment of temporomandibular joint disorders; to describe the technique of this method, the necessary equipment, and its clinical use; to analyse evidence-based literature; to describe how this method compares with others; to examine its advantages and disadvantages. The sources were selected in English using PubMed, EBSCO, and Dyna Med databases, as well as via manual search of scientific sources available at the RSU library. A total of 39 articles and 5 books were selected to be included in the research. TMJ artroscopy can be used to treat patients with early-stage degenerative joint disease, with joint disc displacement, pain dysfunction syndrome and with synovitis, as well as in cases of intra-articular adhesions. Today diagnostic arthroscopy is not recommended to be used as a sole procedure; rather, it should be combined with therapeutic arthroscopy or joint lavage. As any invasive procedure, joint arthroscopy can contribute to the development of complications. Complications mentioned in the literature most frequently are as follows: postoperative oedema, haematoma, facial nerve injury, breakage of instruments, infection and ontological or (less commonly) cardiac complications. Arthroscopic procedures are safe and effective for patients with TMJ disorders as it reduces the risk of postoperative complications and shortens recovery time.
