Abstract: | Atsaucoties uz vadlīnijām, PFO slēgšana tiek rekomendēta sekundāra insulta profilaksei pacientiem ar kriptogēnu insultu. Insulta mehānisms PFO gadījumā tiek aprakstīts kā pardoksiāls embols dēļ PFO mediēta šunta no labās un kreiso pusi. Šī pētījuma mērķis ir apsekot kriptogēna insulta pacientus pēc PFO slēgšanas procedūras un noteikt šīs procedūras ilgtermiņa efektivitāti atkārtota insulta riska samazināšanā.
Pacienti tika iesaistīti retrospektīvi – prospektīvā pētījumā un apsekoti pa telefonu līdz pat pieciem gadiem pēc PFO slēgšanas. Standartilizēta aptauja tika veikta par pacienta labsajūtu, atkārtotiem cerebrovaskulāriem notikumiem un lietotajiem medikamentiem. Pacientiem arī tika izvērtēti reziduāli šunti ar eho-kg palīdzību 24h, 30 dienas, 1 un 2 gadus pēc PFO slēgšanas. Patogēnais išēmiskā insulta apakštips tika noteikts izmantojot CCS (Causative Classification System for Ischemic Stroke).
No 103 pacientiem - 56,3% (n = 58) bija sievietes. Vidējais vecums bija 44 ± 13 (18-75). Saskaņā ar CSS 66,7% (n = 58) pacientu bija iespējams paradoksiāls embols, bet 19,5% (n = 17) bija acīmredzama mazo artēriju oklūzija. Pirms PFO slēgšanas 91,9% (n = 91) pacientu bija piedzīvojuši vismaz vienu insultu vai tranzitori išēmisku lēkmi, un piecu gadu laikā pēc PFO slēgšanas atkārtots insults vai tranzitori išēmiska lēkme bija notikusi tikai 5,1% (n = 5) pacientu, šī atšķirība ir statistiski nozīmīga (p <0,001).
PFO ir iespējams kriptogēna insulta riska faktors pieaugušajiem jaunākiem par 60 gadiem. PFO slēgšana ir efektīva, lai mazinātu atkārtotus cerebrovaskulārus notikumus. According to guidelines PFO closure is recommended for secondary stroke prevention in patients with cryptogenic stroke. Paradoxial embolism from PFO mediated right to left shunt or cardioembolism has been described as the mechanism of stroke in these cases. The aim of the study is to follow-up stroke patients after PFO closure and determine the long-term effectiveness on recurrent stroke risk reduction. A total of 103 patients were enrolled in a retrospective - prospective study and followed-up by phone up to five years after PFO closure. Standardized survey was conducted about their well-being, recurrent cerebrovascular events, and the use of prescribed medication. Patients were also followed up for residul shunts 24h, 30 days, 1 and 2 years after PFO closure by a cardiologist using ECHO-KG.The pathogenic ischemic stroke subtypes are determined using CCS (Causative Classification System for Ischemic Stroke). Of 103 patients included - 56,3% (n=58) were female. The mean age was 44±13 (18-75). According to CSS 66,7% (n=58) of patients had possible cardio-aortic embolism (or paradoxical embolism) and 19,5% (n=17) had evident small artery occlusion. Before PFO closure 91,9% (n=91) of patients had at least one stroke or transient ischemic attack and in five-year time after PFO closure recurrent stroke or transient ischemic attack was reported in only 5,1% (n=5) of patients, this difference is statistically relevant (p<0.001). PFO is a possible risk factor for cryptogenic stroke in young adults. PFO closure is effective in reducing recurrent cerebrovascular events. |
