Philosophy of Out-of-Body Life Support – Extracorporeal membrane oxygenation (ECMO) (literature review)

Abstract

Ievads. Ekstrakorporālā membrānu oksigenācija (EKMO) ir ierīce īslaicīgai plaušu, sirds vai abu funkciju uzturēšanai. EKMO pielietojums un interese par to ir ievērojumi augusi kopš tās pirmssākumiem pagājušajā gadsimtā. EKMO var sniegt nozīmīgu atbalstu tādos stāvokļos, kā akūta respiratora nepietiekamība un kardiogēns šoks, tomēr joprojām aktuāli ir jautājumi saistībā ar ieguvumiem attiecībā pret iespējamām komplikācijām.

Mērķis. Literatūras apskata mērķis ir gūt ieskatu aktuālajās EKMO indikācijās, kontaindikācijās un iespējamo komplikāciju riskos. Tāpat arī aplūkot ētiskos jautājumus, kas rodas ar šīs dzīvības atbalsta ierīces lietošanu.

Materiāls un metode. Literatūras apskats veikts izmantojot Pubmed, EBSCO un ScienceDirect datubāzes, kā arī “Extracorporeal Life Support Organization” izstrādātās vadlīnijas. Darbā apskatīti raksti angļu valodā, kas galvenokārt iznākuši 2005.-2021. gadam, iekļauti 22 pētījumi un 34 apskati. Lielākā daļa patreizējo pētījumu ir retrospektīvi vai aprakstoši. Tika atrasti divi mūsdienu randomizēti kontrolēti pētījumi, kas apskata VV EKMO pielietojumu akūta respiratora distresa sindroma gadījumā un viens šobrīd notiekošs pētījums par VA EKMO.

Secinājumi. Šī literatūras apskata rezultāti ļāva sniegt ieskatu šī brīza EKMO indikācijas, kontraindikācijās, komplikācijās un ētiskajās problēmās, kas saistītas ar šīs ierīces lietošanu. Tāpat arī tika secināts, ka ir nepieciešami turmpmāki, plašāki pētījumi.

Background. Extracorporeal membrane oxygenation (ECMO) is a temporary mechanical life-support technology, that can be used to support the respiratory function, the cardiac function or both. The usage of ECMO and interest surrounding it has greatly evolved since the first attempts during the previous century. ECMO increases the chances of survival for serious, life-threatening conditions, such as acute respiratory failure and cardiogenic shock. However, questions about the benefits versus risks of potential severe complications still remain.

Objective. The aim of this literature review is to gain insight into the current, research-supported indications, benefits, contraindications and complications of VV ECMO and VA ECMO, and lastly the ethical questions, that arise with the use of this evolving life-support technology.

Materials and methods. The research was done using the PubMed (Medline), EBSCO and ScienceDirect databases and guidelines from the Extracorporeal Life Support Organization. Keywords included in the search were extracorporeal membrane oxygenation, ECMO, venovenous ECMO, venoarterial ECMO, ECMO complications. The search was limited to papers in English. Excluding the chapter describing the historical development of ECMO, where historical papers were also included, the main search was limited to recent papers published 2005-2021, 22 studies and 34 reviews were included. Most current studies are observational or retrospective. Two modern day randomized controlled trials (RCT) evaluating VV ECMO in ARDS patients were found. Another RCT concerning VA ECMO was identified as currently ongoing.

Conclusions. The results of this literature review allowed to gain an insight in the current indications, contraindications, complications and ethical issues related to the use of ECMO. Moreover, it underlined the need for more research in this area, that would allow for the development of more precise criteria for ECMO use and even more so for the withdrawal of it.
