Totālas un subtotālas paratireoīdektomijas rezultāti pacientiem ar sekundāru un terciāru hiperparatireozi

Abstract

Pacientiem ar hronisku nieru slimību renālā hiperparatireoze ir izplatīta komplikācija, kurai raksturīgs paaugstināts parathormona līmenis kalcija, fosfora un D vitamīna homeostāzes traucējumu rezultātā. Sekundāras un terciāras hiperparatireozes gadījumā ir iespējama konvencionāla un ķirurģiska ārstēšana. Ķirurģiskās ārstēšanas pamatā ir divas biežāk izmantotās operācijas metodes: totāla paratireoīdektomija ar autotransplantāciju vai subtotāla paratireoīdektomija. Uz doto brīdi nav konkrētu vadlīniju par izvēles ķirurģisko metodi hiperparatireozes ārstēšanai pacientiem ar hronisku nieru slimību. Operatīvās tehnikas izvēle pamatā balstās uz ķirurga izvēli un to, vai pacientam turpmāk tiek plānota nieru transplantācija. Darba mērķis bija analizēt totālas paratireoīdektomijas ar autotransplantāciju un subtotālas paratireoīdektomijas rezultātus pacientiem ar sekundāru un terciāru hiperparatireozi. Analizējot pētījuma rezultātā iegūtos datus par pacientu grupu raksturojumu un laboratorisko rādītāju dinamiku, izvērtēt abu metožu efektivitāti, kā arī salīdzināt tās savā starpā. Salīdzināt iegūtos datus ar literatūrā pieejamiem datiem. Pētījums veikts retrospektīvi, apkopojot Paula Stradiņa klīniskās universitātes slimnīcas ķirurģijas klīnikas un nefroloģijas centra datus par 28 pacientiem ar sekundāru un terciāru hiperparatireozi, kuriem laika posmā no 2015. līdz 2020. gadam tika veikta totāla paratireoīdektomija ar autotransplantāciju vai subtotāla paratireoīdektomija. Tika analizēti pacientu hroniskās nieru slimības raksturojumi un pirms operācijas rādītāji abās grupās un to ietekme uz operatīvās metodes izvēli. Tika salīdzināti parathormona un kalcija dinamiskās izmaiņas, operāciju ilgumi, pēcoperācijas komplikācijas un reoperāciju nepieciešamība starp grupām. Iegūtie rezultāti tika salīdzināti ar literatūrā pieejamiem datiem. Pētījuma rezultāti apstiprināja izvirzīto hipotēzi, ka ārstēšanas rezultāti neatšķiras subtotālas un totālas paratireoīdektomijas gadījumā. Tika secināts, ka starp grupām netika novērotas statistiski ticamas atšķirības starp hroniskās nieru slimības stadiju, pielietotās dialīzes veidu, nieru transplantācijas anamnēzi, parathormona un kalcija līmeni pirms operācijas. Būtiskas atšķirības nepastāvēja, salīdzinot parathormona un kalcija pēcoperācijas dinamiku dažādos novērošanas laika posmos kā arī komplikāciju skaitu un operatīvās intervences ilgumus abu metožu gadījumā, Hipotēze par ķirurģiskām ārstēšanas indikācijām renālas hiperparatireozes gadījumā apstiprinājās.

In chronic kidney disease patients renal hyperparathyroidism is a common complication characterized by elevated parathyroid hormone levels as a result of changes in the homeostasis of calcium, phosphate, and vitamin D. In case of secondary and tertiary hyperparathyroidism, both conservative and surgical treatment are possible. Surgical treatment is based on the two most commonly used surgical methods: total parathyroidectomy with autograft or subtotal parathyroidectomy. There are currently no specific guidelines for the surgical method of choice for the treatment of hyperparathyroidism in patients with chronic kidney disease. The choice of surgical technique is mainly based on the choice of the surgeon and whether the patient is planning a kidney transplant in the future. The aim was to analyse results of subtotal parathyroidectomy and total parathyroidectomy with autotransplantation in patients with secondary and tertiary hyperparathyroidism. Analysing obtained data about characteristics of patient groups and dynamics of laboratory parameters, to evaluate effectiveness of both methods, as well as to compare them with each other. Compare obtained data with data available in the literature The study was performed retrospectively, summarising data of Pauls Stradins Clinical University Hospital Surgery Clinic and Nephrology Centre about 28 patients with secondary and tertiary hyperparathyroidism, who underwent total parathyroidectomy with autograft or subtotal parathyroidectomy from 2015. to 2020. The characteristics of patients with chronic kidney disease and preoperative parameters in both groups and their influence on the choice of surgical method were analysed. Dynamic changes in parathyroid hormone and calcium, duration of operations, postoperative complications, and the need for reoperation between groups were compared. The obtained results were compared with the data available in the literature. The results of the study confirmed the hypothesis that the treatment results did not differ in the case of subtotal and total parathyroidectomy with autotransplantation. It was concluded that no statistically significant differences were observed between groups between the chronic kidney disease stages, the type of dialysis used, the history of kidney transplantation, parathormone and calcium levels before surgery. There were no significant differences between the postoperative dynamics of parathormone and calcium at different follow - up periods, as well as the number of complications and the duration of surgical intervention for both methods. The hypothesis of surgical indications for renal hyperparathyroidism was confirmed.
