Abstract: | Plaušu artērijas atrēzija ar intaktu ventrikuļu starpsienu ir reta arteriālā vada atkarīga iedzimta sirdskaite, kuras morfoloģiskie varianti var būt atšķirīgi, kas ietekmēs pacienta aprūpes iespējas un iznākumu. Pateicoties mūsdienu tehnoloģijām kā fetālai plaušu artērijas valvuloplastijai,kuras rezultātā iespējama biventrikulāra korekcija pēc dzimšanas, kas būtiski ietekmē ilgtermiņa rezultātus. Svarīgi precizēt kuri ehokardiogrāfijas kritēriji , izvērtējot augļa sirdskaites anatomiju, var liecināt par iespējamu valvuloplastijas pielietojumu antenatāli.
Šajā darbā demostrēju klīnisko gadījumu pacientam ar plaušu artērijas atrēziju , intaktu ventrikuļu starpsienu, koronāro artēriju un labā ventrikuļa savienojumiem. Mērķis bija izvērtēt literatūras datus par labāko aprūpes stratēģiju šādam pacientam, kā arī precizēt ehokardiogrāfiskos kritērijus, kuri ir svarīgi pacientu atlasei , lai veiktu invazīvu procedūru antenatāli. Pulmonary atresia with intact ventricular septum is a rare duct-dependent congenital cardiac disease that presents with different morphologies, which can affect the management options offered to patients. New advances have been made in the antenatal care, with fetal pulmonary valvuloplasty having increased positive results on rescuing borderline ventricles from univentricular circulations. In order to know who can benefit from this intervention, echocardiographic parameters are used to predict the type of circulation and who might need this procedure. In this work, I present a case report of a patient that was diagnosed with pulmonary atresia with intact ventricular septum and ventriculocoronary connections. The main goal was to review the literature in order to find what would be the best management strategy for this patient. The main focus is on the echocardiographic predictors of the type of circulation and if this patient would be a candidate for fetal cardiac intervention. |
