dc.description.abstractMakulas cistveida tūska ir biežs iemesls pasliktinātai redzei pēc acu operācijām. Tā var izveidoties pēc jebkuras intraokulāras procedūras, tajā skaitā pēc kataraktas operācijas, vitrektomijas un glaukomas operācijas. Cistveida makulas tūska (CME), kas attīstās pēc kataraktas operācijas, tiek dēvēta par pseidofakisko cistveida makulas tūsku (PCME) vai arī par Irvine-Gass sindromu. Šī ir viena no izplatītākajām kataraktas operācijas komplikācijām, kā arī viens no galvenajiem iemesliem samazinātai redzei pēc kataraktas operācijas. CME biežums pēc kataraktas operācijas dažādos literatūras avotos atkarībā no pielietotās diagnostikas tehnikas, kritērijiem un operācijas tehnikas variē no 1% līdz 30%. Kaut arī ar minimāli invazīvu kataraktas operāciju attīstību tās incidence ir būtiski samazinājusis, tiek paredzēts, ka nākotnē riska faktoru pieaugšanas un populācijas vispārējās novecošanās dēļ, PCME gadījumi būs sastopami daudz biežāk. CME ir multifaktoriāla patoģenēze. Tomēr par galveno mehānismu makulas tūskas attīstībā tiek uzskatīta iekaisuma reakcija pēc operācijas. 95% gadījumu PCME ir komplikācija, kas spontāni uzsūcas 6 mēnešu laikā pēc operācijas. Reti makulas tūskas gadījumi tomēr neatrisinās un neizzūd spontāni. Šie gadījumi bieži pieprasa ilgu un sarežģītu ārstēšanu. Pretiekaisuma medikamenti, tādi kā kortikosteroīdi un nesteroīdie pretiekaisuma līdzekļi šobrīd ir primārā un galvenā izvēle makulas tūskas ārstēšanā. CME, kas nepakļaujas terapijai ar KS un/ vai NPL, var uzlaboties pēc daudz specifiskāku medikamentu, piemēram, anti-VEGF vai immuno-modulatoru, lietošanas. Arī ķirurģiskā ārstēšana ar lāzera vitreolīzi vai pars plana vitrektomiju var tikt veikta sevišķi smagos un nepakļāvīgos CME gadījumos, kas saistīti ar dažādām stiklveida ķermeņa-tīklenes patoloģijām. PCME hroniskās un nepakļāvīgās formas ārstēšana ir gara, lēni progresējoša un saistīta ar lielām papildus izmaksām. Turklāt, par spīti atbilstošai ārstēšanai, pastāv iespēja, ka redzes uzlabojums būs neliels vai pat bez uzlabojuma. Tamdēļ ārkārtīgi svarīga nozīme ir preventatīvajiem pasākumiem PCME novēršanā. Makulas tūskas novēršana sākas ar laicīgu kataraktas diagnosticēšanu, kas ir būtiski, lai mazinātu komplicētas operācijas iespējamību. Daudzi pētījumi ir atklājuši, ka CME attīstību var ievērojami mazināt, kontrolējot noteiktus riska faktorus. Ļoti liela nozīme ir arī vispiemērotākās kataraktas ekstrakcijas tehnikas izvēlei konkrētā gadījumā un profilaktiskai terapijai ar pretiekaisuma medikamentiem. Šobrīd vēl nav izstrādātas un plašāk pieejamas starptautiski atzītas vadlīnijas PCME ārstēšanai un profilaksei. Daudz dažādu pētījumu šajā jomā joprojām norit visā pasaulē. Šī literatūras apskata mērķis ir apkopot pieejamos literatūras avotus par CME biežumu pēc kataraktas operācijas, tās attīstības riska faktoriem un patoģenēzi, kā arī analizēt un apkopot rezultātus no dažādiem klīniskajiem gadījumiem un pētījumiem par visefektīvākajām PCME novēršanas un ārstēšanas iespējām.lv-LV
dc.description.abstractPostoperative cystoid macular edema is a common cause of deteriorated vision after eye surgery, which may develop following any intraocular procedure, including cataract surgery, vitrectomy, and glaucoma filtering surgery. Cystoid macular edema, that develops after cataract surgery is called pseudophakic cystoid macular edema (PCME), or Irvine- Gass syndrome, and is one of the most common complications and causes of vision worsening after cataract surgery. The incidence of PCME is reported to vary between 1% and 30% according to the used diagnostic technique, criteria and surgical technique. Although, with the develoment of less invasive cataract extraction surgeries the incidence of PCME has significantly reduced, however it is estimated, that in the future with growing risk factor numbers and overall ageing population, PCME cases will become more and more common. The pathogenesis of PCME is generally considered to be multifactorial, with the main contribution to post-surgical inflammation. In 95% of cases PCME is a self-limiting disorder, that resolves spontaneously within 6 months post-operatively. Rest of the cases do not always resolve by itself and require treatment, which often appears to be very complex and challenging. Anti-inflammatory medicines, including corticosteroids (CS) and non-steroidal anti-inflammatory drugs (NSAIDs), are the mainstay and primary choice in PCME treatment. PCME resistant to therapy with CS and/ or NSAIDs may benefit from other, more specific, pharmacological agents, such as anti-VEGF and immunomodulators. Surgical treatment, including laser vitreolysis or pars plana vitrectomy, can be considered in severe, refractory PCME cases, especially, if accompanied by vitreoretinal abnormalities. Nevertheless, the management of chronic and refractory PCME is often long and slowly progressing, associated with excessive expenses and, despite the treatment, risk of poor visual outcomes is high. This is why the role of preventative measures is crucially important and should not be underestimated. Prevention of PCME starts with an early diagnosis of cataract and preoperative to postoperative control of risk factors, followed by choosing the most appropriate surgical technique and by administration of the perioperative prophylactic therapy with an anti-inflammatory medication. Presently, there are no internationally accepted guidelines for the treatment and prophylaxis of PCME available. However, many studies in this field are continuously going on and various ophthalmic societies have created their own evidence- based recommendations. Aim of this review is to summarize currently available literature sources about the incidence of CME after cataract surgery, its risk factors and pathogenesis mechanisms, as well as to analyze and outline results of different clinical cases, recommendations and studies regarding most optimal and effective PCME prevention and treatment strategies.en-UK
