Abstract: Ķeizargriezienu definē kā augļa piegādi ar ķirurģisku iegriezumu, to veic caur vēdera sienu un dzemdes sienu. Galvenie ķeizargrieziena indikācijas cēloņi ir; atkārtota ķeizargrieziena piegāde, bridža prezentācija, distocija un augļa distress. Adhēzija notiek vairāk nekā 50% no visām intraabdominālajām operācijām, ieskaitot ķeizargriezienu. Adhēzija parasti atsevišķiem orgāniem piestiprinās viens otram un rada vairākas sekas, ieskaitot tievās zarnas obstrukciju. Šajā pētījumā galvenais mērķis bija izpētīt, kādi ir ķeizargrieziena izraisītie adhēzijas riski, sekas un profilakses metodes. Šajā pētījumā tika izmantotas vairākas datu bāzes, piemēram, NCBI, EBSCO, Elsevier, ScienceDirect, PubMed un PLOSMEDICINE. Rezultāts parādīja, ka pēc ķeizargrieziena ir pierādīts saaugumu pieaugums un ka turpmākajai ķeizargrieziena operācijai palielinās saķeres risks un smagums. Saķeres klātbūtne izraisa ilgāku operācijas laiku nākamajā ķeizargrieziena operācijā, kas palielina tādu komplikāciju risku kā inficēta limfa, ķirurģiskas vietas infekcijas un nieru infekcijas. Šajā pētījumā iekļautie pētījumi pierādīja, ka modificētās cietes ar peritoneālo aizdari ir labākā ķirurģiskā metode, lai novērstu adhēziju. Labi rezultāti adhēzijas novēršanā bija tādi šķēršļi kā modificēts hitozāns, seprafilma un oksidētā celuloze. Lietojot farmakoloģiskos līdzekļus, ieskaitot nimesulīdu, bija arī ievērojama adhēzijas samazināšanās. Turpmākās profilakses metodes, piemēram, gēnu terapija, tika novērtētas šajā pētījumā, un rezultāts pierādīja, ka tādu gēnu kā SPK1 un htPA gēnu piegāde samazināja adhēzijas veidošanos. Lai gan šī pētījuma uzmanības centrā bija saķere pēc ķeizargrieziena, apspriestās profilakses metodes varēja izmantot citās intraabdominālajās operācijās.
Caesarean section is defined as the delivery of foetus through surgical incision, this is made through the abdominal wall and the uterine wall. The leading causes of indication for caesarean section are; repeated caesarean delivery, breech presentation, dystocia and foetal distress. Adhesion occurs in more than 50% of all intra-abdominal surgeries, including caesarean section. Adhesion causes the normally separate organs to attach to each other and cause multiple consequences including small bowel obstruction. In this research paper the main aim was to examine what the risks, consequences and prevention methods of adhesion caused by the caesarean sections are. Several databases were used in this research paper such as NCBI, EBSCO, Elsevier, ScienceDirect, PubMed and PLOSMEDICINE. The result showed that there is a proven increase in adhesions after caesarean section and that the risk and severity of adhesion increases for subsequent caesarean delivery. The presence of adhesion leads to prolonged operation time in subsequent caesarean delivery which increases the risk of complications such as infected lymph, surgical site infections and renal infections. The studies included in this research paper proved that modified starks with peritoneal closure is the best surgical method to prevent adhesion. Barriers such as modified chitosan, seprafilm and oxidised cellulose all had good results in preventing adhesion. There was also a marked decrease in adhesion using pharmacological agents including nimesulide. Future prevention methods like gene therapy were assessed in this research paper and the result proved that gene delivery of genes like SPK1 and htPA resulted in decreased adhesion formation. Although the focus of this study was adhesion following caesarean section the prevention methods discussed could be used in other intraabdominal surgeries.
