Positive Radiological Sign Rate of Patella Dislocation at the Emergency Department in Primary Knee Assesment after Patella Dislocation

Abstract

Patellofemorālā nestabilitāte ir sarežģīta problēma, kas ir īpaši izplatīta starp atlētiskām pusaudzēm. Palielināts skaits atkārtotu dislokāciju tiek novērots starp jauniešiem, kuriem ir vai nu strukturālas patoloģijas, troheālā displāzijā, hiperflexitāte vai pārlieku stingras vai vaļīgas mediālās patellārās saites. Pētījumos ir ziņots par astoņas reizes augstāku atkārtošanās līmeni pēc primāras konservatīvas ārstēšanas, salīdzinot ar ķirurģisko ārstēšanu. Mediālās patellofemorālās saites rekonstrukcija uzrādīja labu prognozi, tikai vienam no 15 pacientiem attīstoties atkārtotai nestabilitātei.

Patellofemoral instability is a complex management problem, especially prevalent in athletic adolescent females. Increased dislocation recurrence is seen in young people with either structural abnormalities of trochlear dysplasia, hyperlaxity, deficient or lax medial patellar restraints or tight lateral patellar restraints. Studies have reported eight times higher recurrence rates after primary conservative management compared to surgical treatment. Medial Patellofemoral Ligament reconstruction showed good prognosis with one of 15 patients developing recurrent instability.
