Abstract: Anotācija Ievads Ceļa pilna artroplastika kā ārstēšanas iespēja tika ieviesta 20. gadsimta 70. gados. Kopš tā laika ir veikta virkne uzlabojumu, bet ortopēdi joprojām nav vienisprātis par to, kā ārstēt patellofemorālas locītavas. Ķirurgi pašlaik tiek iedalīti trīs grupās atkarībā no viņu pieejas ceļa skriemeļu sāpju mazināšanai: ceļa skriemeļa pārklāšana visos gadījumos, selektīva ceļa skriemeļa pārklāšana vai ceļa skriemeļa nepārklāšana nevienā gadījumā. Šis sadalījums dažādās valstīs ir atšķirīgs, un šodien tas joprojām mainās, liecinot par to, ka trūkst skaidru vadlīniju, ar kuru palīdzību varētu prognozēt, kuriem pacientiem ir nepieciešama ceļa skriemeļa virsmas pārklāšana un kuriem tā nav nepieciešama. Mērķis un hipotēze Šajā pētījumā tiek apskatītas atjaunotas un neatjaunotas ceļa skriemeļa virsmas komplikācijas pēc ceļa pilnas artroplastikas, salīdzinot dažādas ķirurģiskās ārstēšanas metodes ceļa pilnas artroplastikas laikā. Tiek identificētas dažādas etioloģijas, kas rada patellofemorālas komplikācijas pēc ceļa artroplastikas, un salīdzinātas komplikācijas, kas pēc ceļa pilnas artroplasikas rodas aizstātam endoprotezitam un dabiskam endoprotezitam. Hipotēze ir tāda, ka ceļa skriemeļa virsmas selektīva atjaunošana ceļa pilnā artroplastikā ir piemērotāka nekā savstarpēji pretējās pieejas – ceļa skriemeļa virsmas pārklāšana visos gadījumos vai nevienā gadījumā. Metodes Šis pētnieciskais darbs ir balstīts uz selektīvas literatūras pārskatu, kurā analizēta mūsdienu literatūra, un pētījumiem, kas ir iekļauti promocijas darba beigās. Rezultāti Patellofemorālo locītavu komplikāciju cēloņus iedala trīs grupās: cēloņi, kas saistīti ar pacientu, ar ķirurgu un ar protēzi. Pēc ceļa vispārējās artroplastikas komplikāciju riska faktorus konstatē pacientiem ar lieko svaru, depresiju, jauniešiem un sievietēm. Ar protēzi saistītie faktori, kas agrāk izraisīja komplikācijas, ir pārāk sekla bloka rieva, izmantotā materiāla veids un neatbilstība starp ceļa skriemeļa implantu un augšstilba nodalījumu. Ķirurgam ceļa pilnas artroplastikas iznākumu nosaka tas, vai augšstilba kaula un lielā lielakaula nodalījuma forma un izmērs ir pareizs, atsevišķu nodalījumu izvietojums un savienojums starp lielā lielakaula femorālo locītavu un ceļa skriemeļa femorālo locītavu. Iespējamās komplikācijas, kas var rasties pēc ceļa locītavas pilnas artroplastikas, ir šādas: priekšējās ceļa sāpes, ceļa skriemeļa nepareiza kustība, ceļa skriemeļa krakšķēšana, ceļa skriemeļa avaskulāra nekroze un ceļa skriemeļa lūzumi. Salīdzinot komplikāciju biežumu atkarībā no ceļa skriemeļa ārstēšanas stratēģijas, statistiski nozīmīgas atšķirības nav. Atšķirība ir tā, ka dabiskā ceļa skriemeļa atkārtoti veicamo operāciju skaits ir lielāks, kas var būt saistīts ar iespēju veikt ceļa skriemeļa virsmas sekundāru pārklāšanu kā ārstēšanas opciju jebkuru komplikāciju gadījumā. Abstract Introduction Total knee arthroplasty got introduced as a treatment option during the 1970s. Since then more and more advantages have been made but the orthopedic society still does not have a clear consent on how to treat the patellofemoral joint. Surgeons are currently divided into three groups depending on their approach towards the patella: always, never or selectively resurfacing the patella. This distribution is from country to country different and is still changing today representing the lack of clear guidelines that could predict the patients that are in need for patella resurfacing and the ones that are not. Aim and hypothesis This study reviews the complications of resurfaced and non-resurfaced patellae after total knee arthroplasty by comparing the different treatment approaches of surgeons during total knee arthroplasty. To identify the different etiologies for arising patellofemoral complications after total knee arthroplasty and comparing the complications between resurfaced and native patella after total knee arthroplasty. The hypothesis is that the approach of selectively resurfacing the patella in total knee arthroplasty is the more appropriate approach compared to the opposing approaches of always or never resurfacing the patella. Methods This research paper is based upon a selective review of literature analyzing modern literature, and studies which can be found at the end of the thesis. Results The etiology for patellofemoral joint complications can be divided into patient, surgeon and prothesis associated. For patients overweight, depression, young age and female sex are identified risk factors for complications after total knee arthroplasty. The prothesis associated factors shown in the past to cause complications are a too shallow trochlear groove, the type of material used and an incongruence between the patella implant and the femoral compartment. For the surgeon the correct shape and size of the femoral and tibial component, the bone preparation, the alignment of the individual components, and the connection between the tibiofemoral joint and the patellofemoral joint are determining the outcome after the total knee arthroplasty. Possible complications that could arise after total knee arthroplasty are anterior knee pain, patella maltracking, patellar crepitus, avascular necrosis of the patella and patellar fracture. Comparing the complication rates depending on the treatment strategy of the patella no statistically significant difference is present. What differs is the reoperation rate in native patellae is higher, which can be associated with the possibility of secondary resurfacing the patella as a treatment option for any complications. Conclusion Analyzing the approaches of surgeons towards the patella it becomes evident that always or never resurfacing the patella creates a raffle, where subsequently some patellae aren’t resurfaced even though they need it and oppositely some that don’t need it are resu
