Abstract: Barības vada atrēzija ir reta iedzimta slimība, kurai raksturīga barības vada pārtraukšanās un nespēja savienot rīkli ar kuņģi. Apmēram desmit procentos barības vada atrēzijas pacientiem novēro garu vai ļoti garu attālumu jeb distanci starp atrētiskajiem galiem. Barības vads ir 18-26 cm cilindrisks cauruļveida orgāns, kam raksturīga anizotropiska uzvedība un kas histoloģiski sastāv no četriem slāņiem, proti, gļotādas, zemgļotādas (spēcīgākais no mehāniskā viedokļa), muskuļslāņa un serozas. Gadījumos, kad attālums starp atrētiskajiem barības vada galiem ir liels, atlikta primārā anastomoze tiek uzskatīta par zelta standartu. Starp potenciālajām jaunajām ārstēšanas metodēm par perspektīvu tiek uzskatīta audu inženierija. Metodes parasti iedala scaffolds (matrices), kas realizētas ar parastām un ātrām prototipēšanas metodēm, un scaffolds-free (bez matrices) kas realizētas, izmantojot Kenzan metodi, šūnu loksnēs un audu inženierijas organoģenēzē.
Esophageal atresia is a rare congenital disease, characterized by esophagus discontinuity, and inability to connect the pharynx to the stomach. Ten percent of esophageal atresia are named long-gap esophageal atresia based on the length of the gap, despite no clear definition currently exists. The esophagus is a cylindrical conduit of 18-26 cm, characterized by anisotropic behaviour and, histologically composed of four layers, namely mucosa, submucosa (strongest from mechanical perspective), muscularis propria, and adventitia. Concerning the treatment of long gap esophageal atresia, delayed primary anastomosis is considered the gold standard. Among the existing novel therapies, tissue engineering represents the most important. The techniques are generally divided into scaffolds realised by conventional and rapid prototyping techniques and, scaffold-free esophagi realized via Kenzan method, cell sheet and tissue engineered organogenesis.
