SFK pamatkopa pacientiem ar krūts vēzi - pacientu pēc mastektomijas subjektīvs funkcionēšanas vērtējums

Abstract

Nosaukums: SFK pamatkopa pacientiem ar krūts vēzi - pacientu pēc mastektomijas subjektīvs funkcionēšanas vērtējums.

Aktualitāte: Latvijā 2017. gadā pirmreizēji diagnosticēts ļaundabīgs krūts audzējs tika 1133 sievietēm un 10 vīriešiem, gada beigās uzskaitē ar diagnosticētu krūts vēzi bija 13356 sievietes, 64,8% no uzskaitē esošajām sievietēm kopš diagnozes noteikšanas nodzīvojušas ilgāk par 5 gadiem. 2018. gadā no krūts vēža mira 418 sievietes. Lai sniegtu mērķtiecīgu ārstniecības rezultātu, tai skaitā rehabilitācijas pakalpojumu, ir svarīgi identificēt problēmas un šķēršļus, kas ietekmē darbību no pacienta perspektīvas.

Pētījuma mērķis: Analizēt funkcionēšanas traucējumus pacientiem ar krūts vēzi, izpētot uztvertās problēmas, to sasaisti ar “SFK pamatkopu pacientiem ar krūts vēzi”.

Pētījuma dizains: Kvalitatīvs pētījums, ar tematisku satura analīzi.

Pētījuma dalībnieki: Pētījumā tika iekļautas 7 sievietes pēc krūts vēža operācijām, neatkarīgi no saņemtās papildus terapijas.

Instrumenti: Daļēji strukturētas intervijas.

Rezultāti: Kopumā no visiem analizētajiem transkriptiem tika savienotas 215 vienības, kas atbilst kādai no SFK pamatkopas pacientiem ar krūts vēzi kategorijai. “Ķermeņa funkcijas” (94 vienības, kas savienoja septiņas kategorijas), “Ķermeņa struktūras” (31 vienības, kas savienoja četras kategorijas), “Aktivitātes un dalība” (57 vienība, kas savienoja deviņas kategorijas); “Vides faktori” (33 vienības, kas savienoja sešas kategorijas). Visas vienības tika kodētas balsoties uz SFK veidotajiem kodiem (otrā līmeņa kodiem), kas ļauj izvērtēt pētījuma dalībnieka funkcionēšanas līmeni un viņu saskatītās problēmas saskaroties ar konkrēto diagnozi. Papildus tika savienota viena kategorija (b144 Atmiņas funkcijas), kas šobrīd nav iekļauta SFK pamatkopā pacientiem ar krūts vēzi. Kopumā tika savienotas 26 kategorijas no 80, kas ietilpst SFK pamatkopā pacientiem ar krūts vēzi.

Secinājumi: Krūts vēža pacientes pēc mastektomijas saskaras ar funkcionēšanas traucējumiem visos SFK sadaļu līmeņos, problēmu kopums veidojas mijiedarbībā ķermeņa funkcijām, struktūrām ar aktivitātēm un dalību kā arī vides un apkārtējās vides faktoriem. Pētīto pacientu subjektīvais funkcionēšanas viedoklis pēc mastektomijas atspoguļojās “SFK pamatkopā pacientiem ar krūts vēzi” savienojot 26 kategorijas no 80), šo pamatkopu papildinot ar jaunām kategorijām un iespējams reducējot izslēdzot, kādas kategorijas var pielietot, lai veiktu pacientu pēc krūts vēža operācijām funkcionēšanas ierobežojumu novērtēšanu, tādējādi iegūstot strukturētu informāciju par pacientu.

Title: ICF Core Set for breast cancer - subjective assessment of patients after mastectomy.

Topicality: In Latvia, 2017, 1133 women and 10 men were diagnosed with malignant breast cancer for the first time, at the end of the year there were 13,356 women registered with breast cancer, 64.8% of the registered women had lived for more than 5 years since the diagnosis. In 2018, 418 women died of breast cancer. In order to provide a targeted treatment outcome, including rehabilitation services, it is important to identify issues and barriers that affect patient limitations from the patient's perspective.

Aim: Analysed the limitation of functioning in patients with breast cancer by studying the perceived problems and their relevance with the “ICF Core set for Breast cancer”.

Study design: Qualitative research with thematic content analysis.

Participants: The research included 7 women after breast cancer surgery, regardless of the additional treatment they received.

Methods: Semi-structured interviews.

Results: 215 units from all analysed transcripts were linked, corresponding to one of the ICF Core set for Breast cancer. “Body functions” (94 units that linked seven categories), “Body structures” (31 units that linked four categories), “Activities and participation” (57 units that linked nine categories); “Environmental factors” (33 units linked six categories). All units were coded based on the codes created by the ICF (second level codes), which allows to assess the level of functioning of the research participant and the problems they perceived in the face of the specific diagnosis. In addition, one category (b144 Memory functions) was merged, which is not currently included in the ICF Core set for Breast cancer patients. A total of 26 of the 80 categories in the ICF Core set for Breast cancer patients were linked.

Conclusions: Breast cancer patients after mastectomy experiencing functional problems at all levels of the ICF section, a set of problems is formed by the interaction of body functions and structures with activities and participation and environmental factors. The subjective opinion of the research patients after mastectomy was reflected in the “ICF Core set for Breast cancer ” linking 26 categories out of 80), adding new categories to this core group and possibly reducing to exclude which categories can be used to assess the limitations of breast cancer patients obtaining structured information about the patient.
