Abstract: | Darba nosaukums: Novērtēšanas instrumentu satura un psihometrisko īpašību salīdzinājums bērniem ar obstruktīvo miega apnoju (OMA): sistemātisks literatūras pārskats
Pētāmā problēma: Atsevišķās slimnīcās un veselības aprūpes iestādēs Latvijā šobrīd ir pieejams polisomnogrāfijas (PSG) izmeklējums obstruktīvās miega apnojas (OMA) diagnosticēšanā, kas ir dārga un sarežģīta procedūra. Vienoti un validēti skrīninga instrumenti OMA izvērtēšanā Latvijā netiek pielietoti. Lai diagnosticētu bērnus ar OMA, un novērtētu šo pacientu vajadzības, ir nepieciešams specifisks un vienots novērtēšanas instruments. Pediatrisko pacientu vidū, kuriem ir paaugstināts OMA risks, ir bērni ar kraniofaciāliem traucējumiem, iedzimtām patoloģijām, adipozitāti, ar neiromuskulārām saslimšanām, kardiomiopātiju, lūpu/aukslēju šķeltnēm, hronisku nieru slimību (MacLean, 2019).
Darba mērķis: Noskaidrot biežāk izmantotos obstruktīvās miega apnojas novērtēšanas instrumentus bērniem vecumā līdz 18 gadiem, salīdzināt to saturu, izmantojot Starptautisko funkcionēšanas, nespējas un veselības klasifikāciju – Bērnu un jauniešu versiju (SFK-BJ), un salīdzināt to psihometriskās īpašības.
Pētījuma metodes: Sistemātisks literatūras pārskats, pētījumos biežāk izmantoto obstruktīvās miega apnojas novērtēšanas instrumentu identificēšana. Atlasīto instrumentu jēgpilno elementu sasaiste ar SFK-BJ kategorijām, un šo instrumentu psihometrisko īpašību analīze.
Pētījuma rezultāti: Sistemātiskajā literatūras pārskatā tika iekļauti 77 pētījumi, kas tika identificēti un atlasīti saskaņā ar iekļaušanas un izslēgšanas kritērijiem. Pētījumos identificēti 26 obstruktīvās miega apnojas novērtēšanas instrumenti bērnu populācijā. Biežāk izmantotie instrumenti (Polysomnography (PSG), Pediatric Sleep questionnaire (PSQ), ESS-CHAD - Epworth Sleepiness Scale for Children and Adolescents, Obstructive Sleep Apnea 18 (OSA-18), Pediatric Quality of Life Inventory (PedsQL)) tika iekļauti tālākā analīzē. Identificējot jēgpilnos konceptus, biežāk izmantotajiem novērtēšanas instrumentiem sasaiste ar SFK-BJ tika veikta ar 65 kategorijām. Sasaistīto konceptu pārsvars vērojams PSQ novērtēšanas instrumentam. Ķermeņa funkciju kategoriju pārsvars instrumentos PSQ un OSA-18 ir vienādā skaitā, šajā domēnā sasaiste veikta arī PSG, ESS-CHAD, PedsQL. Ar vienu ķermeņa struktūru sasaiste veikta PSG un OSA-18. Sasaiste ar aktivitātes un dalība kategorijām veikta visiem instrumentim. Vides kategorijās sasaiste nav veikta tikai PSG. Visiem biežāk izmantotajiem novērtēšanas instrumentiem ir pierādīta uzticamība un pamatotība, atsaucība uz izmaiņām ir pierādīta tikai OSA-18. Thesis title: Comparison of assessment tools content and psychometric properties in children with obstructive sleep apnea (OSA): a systematic review of the literature Research problem: Polysomnography (PSG) as diagnostic tool of obstructive sleep apnea (OSA) is currently available in some hospitals and health care institutions in Latvia, which is an expensive and complicated procedure. Uniform and validated screening tools are not used in OSA evaluation in Latvia. A specific and common assessment tool is needed to diagnose children with OSA and to assess their needs. Pediatric patients at increased risk of OSA includes children with craniofacial disorders, congenital abnormalities, adiposity, neuromuscular disorders, cardiomyopathy, cleft lip and palate, and chronic kidney disease. (MacLean, 2019) The purpose of this study was to determine most commonly used tools for assessing obstructive sleep apnea in children under 18 years of age, to compare their content using the International Classification of Functioning, Disability and Health for Children and Youth (ICF-CY), and to compare their psychometric characteristics. Methodology: Systematic review of the literature, identification of the most commonly used assessment tools for obstructive sleep apnea in research. Linking the meaningful concepts of the selected instruments with the ICF-CY categories, and the analysis of the psychometric properties of these instruments. Findings: The systematic literature review included 77 studies that were identified and selected according to inclusion and exclusion criteria. The studies identified 26 tools for assessing obstructive sleep apnea in the pediatric population. Commonly used instruments (Polysomnography (PSG), Pediatric Sleep questionnaire (PSQ), ESS-CHAD - Epworth Sleepiness Scale for Children and Adolescents, Obstructive Sleep Apnea 18 (OSA-18), Pediatric Quality of Life Inventory (PedsQL)) were included in the further analysis. In order to identify meaningful concepts, the most frequently used assessment tools were linked to the ICF-CY with 65 categories. The predominance of linked concepts is observed in the PSQ assessment tool. The predominance of categories in component body functions in the instruments PSQ and OSA-18 is equal, PSG, ESS-CHAD, PedsQL instruments were also linked in this domain. Linking to one body structure was performed with PSG and OSA-18. Linking to categories of activities and participation was done for all instruments. Categories of environment are not linked only to PSG. All commonly used assessment tools have been shown to be reliable and valid, while responsiveness to change has only been demonstrated in OSA-18. |
