Pacientu apmierinātības ar akūtu pēcoperācijas sāpju ārstēšanu pēc traumatoloģiskām un ortopēdiskām operācijām novērtējums Ziemeļkurzemes reģionālajā slimnīcā

Abstract

Šī darba mērķis ir apzināt, izvērtēt un aprakstīt pacientu apmierinātību ar pēcoperācijas sāpju ārstēšanu Ziemeļkurzemes reģionālās slimnīcas traumatoloģijas un ortopēdijas nodaļā. Pielietotā metodoloģija ir prospektīvs observācijas pētījums, kurš veikts Ziemeļkurzemes reģionālās slimnīcas Traumatoloģijas nodaļā laika posmā no 2023. gada 1. marta līdz 31. maijam. Pētījuma laikā veikta pacientu anketēšana, iegūto datu apkopošana un statistiskā analīze ar neparametriskām statistikas metodēm. Izmantota programma IBM SPSS Statistics 22. Zinātniskais darbs uzrakstīts latviešu valodā uz 38 lapām, tā sastāvā ir ievads, literatūras apskats, pētījuma materiālu un metodes apraksts, rezultāti, diskusijas sadaļa, kā arī secinājumi un izmantotās literatūras saraksts. Literatūras apskatā ir četras nodaļas, kurās aprakstīti algoloģijas pamatjautājumi, tajā skaitā, sāpju definīcija, anatomija, sāpju veidi un jēdziens par sāpju biopsihosociālo modeli, pēcoperācijas sāpju galvenie aspekti, kā arī izklāstīts multimodālās sāpju ārstēšanas jēdziens un būtība. Darba autors izskaidrojis pacientu apmierinātības aktualitāti gan pašam pacientam, gan medicīniskajam personālam, lielu nozīmi piešķirot saskarsmes jautājumiem, līdzestībai un izglītībai. Zinātniskā darba diskusiju secinājumu daļā aprakstīts, ka pēcoperācijas sāpju kontrole pacientiem pēc traumatoloģiskām un ortopēdiskām operācijām ir nepietiekama, dažkārt pēcoperācijas sāpes traucē veikt fiziskas aktivitātes, mazāk ietekmējot garastāvokli un emocijas. Neskatoties uz salīdzinoši intensīvām sāpēm, lielākā daļa pacientu ir apmierināti ar pēcoperācijas sāpju ārstēšanu pēc traumatoloģiskām un ortopēdiskām operācijām. Darba autors apņemas izstrādāt un ieviest praksē vienotas multimodālas rekomendācijas pēcoperācijas sāpju ārstēšanai, kas veicinās pacientu apmierinātību. Pievienots izmantotās literatūras saraksts.

The purpose of this work is to identify, evaluate and describe patient satisfaction with post-operative pain treatment in the traumatology and orthopaedics unit of North Kurzeme Regional hospital. The methodology applied is a prospective observational study carried out in the traumatology unit of North Kurzeme Regional hospital between 1 March and 31 May 2023. Patient questionnaires, data collection and statistical analysis with non-parametric statistical methods have been carried out during the study. IBM SPSS Statistics 22 has been used. The scientific work is written in Latvian on 38 pages, consisting of an introduction, a literature review, a description of the materials and method of the study, results, discussion section, as well as conclusions and a list of the literature used. The literature review contains four chapters describing basic issues of mercenary, including the definition of pain, anatomy, types of pain, and the concept of the biopsychosocial model of pain, key aspects of post-operative pain, and outlines the concept and nature of multi-modal pain treatment. The author explained the current state of patient satisfaction for both the patient himself and the medical staff, attaching great importance to issues of contact, equivalency and education. The discussion conclusions part of the scientific work describes that post-operative pain control in patients after traumatological and orthopaedic surgery is inadequate, sometimes post-operative pain disrupts physical activity, with less impact on mood and emotion. Despite relatively intense pain, most patients are satisfied with post-operative pain treatment after traumatological and orthopaedic surgery. The author commits to developing and putting into practice uniform multi-modal recommendations for the treatment of post-operative pain, which will boost patient satisfaction.
