Strukturēts fizioterapijas pakalpojums primārās veselības aprūpes līmenī pacientiem ar hroniskām primārām muskuloskeletālām sāpēm: veidošanas process un sākotnējie rezultāti

Abstract

Darba nosaukums: Strukturēts fizioterapijas pakalpojums primārās veselības aprūpes līmenī pacientiem ar hroniskām primārām muskuloskeletālām sāpēm: veidošanas process un sākotnējie rezultāti Darba mērķis: atspoguļot strukturēta kompleksa fizioterapijas pakalpojuma (programmas) veidošanas procesu un sākotnējos rezultātus pacientiem ar hroniskām primārām muskuloskeletālās sāpēm primārās veselības aprūpes līmenī. Pētījuma metodes: Koprades pieejā tika vākti un analizēti kvalitatīvie dati no fokusgrupu diskusijām. Sākotnēji pierādījumu izpētē I tika meklēti un apkopoti dati par citās valstīs ieviestajām un realizētajām fizioterapijas programmām. Programmu saturiskais apkopojums tika ņemts par pamatu, lai izveidotu pirmās fokusgrupas diskusijas tēmas. Pierādījumu izpēte II ietvēra aktuālo vadlīniju kartēšanu, kā arī papildus pēdējos divos gados publicēto pētījumu apkopošanu. Ieviešanas fokusa pieejā dati tika apkopoti PRISM ietvarā izceļot četrus kontekstuālos domēnus, datu ieguvei tika izmantoti statistikas dati, normatīvie regulējumi un līdzšinējās izpētes rezultāti. Datu sasaistē satura kodi no pirmās fokusa grupas tika saistīti ar pierādījumiem no kartēšanas datiem. tika izveidoti sasaistītie kodi, papildus atziņas un nesasaistītie kodi iegūtie sasaistes rezultāti tika analizēti saistībā ar kontekstuālajiem domēniem PRISM ietvarā. Otrās fokusgrupas atziņas tika saistītas un lietotas prototipa pilnveidei. Pētījuma rezultāti un secinājumi: Pakalpojuma veidošanas procesa atspoguļojums, ļauj ieraudzīt kombinētās pieejas komplekso ieguvumu, integrējot uz pierādījumiem balstītu praksi ar radošo un iteratīvo dizaina domāšanu, virzot caur lokālā konteksta prizmu, tādejādi saglabājot fokusu uz ieviešanu. Izveidotā FT-PVA-LV pakalpojuma struktūra ietver četrus pamata elementus, no kuriem divi saistāmi ar FT pakalpojuma integrācijas stratēģiju, balstoties uz Varavīksnes modeli, bet divi specifiski un detalizēti aprakstīta pakalpojuma sniegšanas veidu.

Title: Model of complex physiotherapy interventions in primary care settings for patients with chronic primary musculoskeletal pain: development and initial results Aim of the study: to illustrate the process and initial outcomes of developing a structured complex physiotherapy intervention (programme) for patients with chronic primary musculoskeletal pain at primary care level. Methods: The co-creation approach involved collecting and analysing qualitative data from focus group discussions. Initially, Evidence Study I searched for and collected data on physiotherapy programmes introduced and implemented in other countries. The content of the programmes was used as a basis for the first focus group discussion topics. Evidence exploration II included a mapping of current guidelines as well as additional research published in the last two years. In the implementation focus approach, data were collected within the PRISM framework highlighting four contextual domains, statistical data, normative frameworks and results of previous research were used for data extraction. In the data linkage, content codes from the first focus group were linked to evidence from the mapping data. Linked codes were created, additional insights and unlinked codes were extracted and the results of the linking were analysed in relation to the contextual domains in the PRISM framework. The insights from the second focus group were linked and used to refine the prototype. Results and conclusion: The service design process is illustrated, allowing us to see the complex benefits of a combined approach, integrating evidence-based practice with creative and iterative design thinking, guided through the prism of the local context, thus keeping the focus on implementation. The developed FT-PVA-LV service structure includes four core elements, two of which relate to the FT service integration strategy based on the Rainbow model, and two of which are specific and describe in detail the way the service is delivered.
