Šveices funkcionālo spēju novērtējuma instrumenta “Swiss FCE and Work hardening system” adaptēšana Latvijas sociālās rehabilitācijas ietvaros.

Abstract

Darba nosaukums: Šveices funkcionālo spēju novērtējuma instrumenta “Swiss FCE and Work hardening system” adaptēšana Latvijas sociālās rehabilitācijas ietvaros. Aktualitāte: Funkcionālo spēju novērtēšanas (Functional capacity evaluation, FCE) koncepts ir sistemātiska pieeja, ko izmanto darbinieka funkcionālo spēju novērtēšanai, ir kļuvusi par daļu no kopējās rehabilitācijas prakses pacientiem ar muskuloskeletālām saslimšanām vairākās pasaules valstīs. Latvijas sociālās rehabilitācijas ietvaros nav vienota funkcionālo spēju novērtēšanas instrumenta, kas palīdzētu sistematizēt novērtēšanas datus un sniegtu kopēju priekšstatu par cilvēku darbspējām. Darba mērķis: adaptēt Šveices funkcionālo spēju novērtējuma instrumentu “SWISS FCE and Work hardening system” latviešu valodā un izpētīt tā ticamības un pamatotības rādītājus. Pētījumā izmantotās metodes: Balstoties uz PVO izstrādātajām vadlīnijām, tika veikta instrumenta “Šveices funkcionālo spēju novērtēšanas un darbspēju nostiprināšanas sistēma” (“SWISS FCE and Work hardening system”) tulkošana latviešu valodā piesaistot trīs filologus un Sociālās integrācijas valsts aģentūras (SIVA) funkcionālos speciālistus. Pielietojot instrumenta tulkoto versiju, pilotpētījuma ietvaros tika novērtēti 12 SIVA klienti ar muskuloskeletālām saslimšanām, ar mērķi izprast vai instrumenta latviešu valodas versija ir izprotama mērķa grupai. Visi pētījumā iesaistītie dalībnieki aizpildīja arī jau Latvijā lietoto, Ministru kabineta, apstiprināto “Funkcionālo spēju pašnovērtējuma anketu”. Lai pārbaudītu “Šveices funkcionālo spēju novērtēšanas un darbspēju nostiprināšanas sistēmas” psihometriskās īpašības tika noteikta instrumenta iekšējā saskaņotība, saturiskā un paralēlā pamatotība. Rezultāti: Iegūtie rezultāti liecina, ka instrumentā iekļautie standartizētie testi funkcionālo spēju novērtēšanai uzrāda augstu iekšējo saskaņotību (Cronbach alfa α=0.978), kā arī subjektīvais novērtējums “Mugurkaula funkciju pašnovērtējuma tests” (Spinal function sort, SFS) uzrāda augstu iekšējo saskaņotību (α= 0.965). Saturiskā pamatotība veikta balstoties uz hipotēžu pārbaudi, tika atrastas vājas līdz vidēji ciešas korelācijas FCE testiem ar sāpēm, stipru korelāciju starp dzimumu un FCE slodzes izturības testiem un vidējas līdz ļoti stipras korelācijas FCE testiem ar SFS testa rezultātiem. Tika noteikta paralēlā pamatotība balstoties uz korelāciju starp SFS un “Funkcionālo spēju pašnovērtējuma anketu”, dati liecina, ka pastāv vidēji cieša korelācija (rs = -0.503), taču tā nav statistiski nozīmīga (p =.096). Secinājumi: “Šveices funkcionālo spēju novērtēšanas un darbspēju nostiprināšanas sistēma” ir augsti iekšējās saskaņotības un labi saturiskās pamatotības rādītāji, kas liecina par to, ka instruments ir uzticams un pamatots, lai to izmantotu Latvijas sociālās rehabilitācijas ietvaros.

Title: Adaptation of the Switzerland functional capacity evaluation tool "Swiss FCE and Work hardening system" in the context of Latvian social rehabilitation. Background: Functional capacity evaluation (FCE), as a systematic approach used to assess an employee's functional capacity, has become part of common rehabilitation practice for patients with musculoskeletal disorders in several countries around the world. In the Latvian social rehabilitation, there is no single functional capacity assessment tool that would help to systematise assessment data and provide an overall picture of people's working capacity. Research aim: To adapt the Switzerland functional abilities assessment tool “SWISS FCE and Work hardening system” into Latvian and examine reliability and validity the Latvian version of the “SWISS FCE”. Methods: Using the translated version of the tool, 12 SIVA clients with musculoskeletal disorders were assessed in a pilot study to understand whether the Latvian version of the tool was comprehensible to the target group. All participants also completed the "Functional Ability Self-Assessment Questionnaire" already approved by the Latvian Cabinet of Ministers. The internal consistency, content validity and concurrent validity of the instrument were determined in order to test the psychometric properties of the "Swiss FCE and Work hardening system". Results: The results show that the functional ability assessment of the instrument by standardised tests shows high internal consistency (Cronbach alpha α=0.978) and the subjective assessment "Spinal function sort (SFS)" shows high internal consistency (α= 0.965). Content validity was performed based on hypothesis testing, weak to moderate correlations were found for FCE tests with pain, strong correlations between gender and FCE exercise endurance tests, and moderate to very strong correlations for FCE tests with SFS test scores. Concurrent validity was established based on the correlation between SFS and the “Functional Capacity Self-Assessment Questionnaire”, the data suggest that there is a moderately strong correlation (rs = -0.503), but these are not statistically significant (p =.096). Conclusion: “Swiss FCE and Work hardening system” has high internal consistency and good content validity, indicating that the instrument is reliable and valid for use in the context of Latvian social rehabilitation system.
