Fizioterapeitu pielietotās ārstēšanas tehnoloģijas pacientiem ar deģeneratīviem rotatoru aproces muskuļu bojājumiem: jomas pārskats un fizioterapeitu klīniskās prakses raksturojums.

Abstract

Nosaukums: Fizioterapeitu pielietotās ārstēšanas tehnoloģijas pacientiem ar deģeneratīviem rotatoru aproces muskuļu bojājumiem: darbības jomas pārskats un fizioterapeitu klīniskās prakses raksturojums Latvijā. Mērķis: 1. Identificēt un sistematizēt datus par fizioterapeitu izmantotajām fizioterapeitiskajām tehnoloģijām ambulatorā ārstēšanas pieejā pacientiem ar deģeneratīviem rotatoru aproces bojājumiem no jaunākās zinātniskās literatūras, veicot darbības jomas pārskatu (scoping review). 2. Izpētīt kādas fizioterapeitiskās tehnoloģijas savā praksē pielieto Latvijā praktizējoši fizioterapeiti ambulatorā ārstēšanas pieejā pacientiem ar deģeneratīviem rotatoru aproces bojājumiem. Metodes: Darbības jomas pārskats (scoping review) ar tam sekojošu kvantitatīvu šķērsgriezuma pētījumu. Pirmajā daļā tiek veikta literatūras izpēte un apkopošana, lai izveidotu jomas pārskatu, pamatojoties uz Joanna Briggs Institute's (JBI) vadlīnijām. Otrajā daļā iegūta atļauja no autora anketas “Rotator cuff disorders: a survey of current UK physiotherapy practice” tulkošanai un adaptēšanai latviešu valodā. Ar anketas palīdzību tiek veikta kvantitatīvo datu vākšana no Latvijā praktizējošiem fizioterapeitiem, lai noskaidrotu viņu pielietotās fizioterapijas tehnoloģijas strādājot ar pacientiem pēc deģeneratīviem rotatoru aproces bojājumiem. Iegūtie dati tiek salīdzināti ar jomas pārskata rezultātiem. Rezultāti: Galvenās terapeitiskās tehnoloģijas, kas tika identificētas veidojot darbības jomas pārskatu, sakrita ar terapijas tehnoloģijām ārstējot pacientus ar deģeneratīviem rotatoru aproces bojājumiem, ko rekomendēja aptaujātie Latvijā praktizējoši fizioterapeiti: pacienta izglītošana/informēšana (85,9%); kustību apjoma izstrāde (25,7%); ārstniecisko vingrojumu pielietošana (62%); manipulācijas/mobilizācijas (36,6%); masāža (53,5), fizikālā terapijas (22,5%) un injekcijas (14,1%). Galvenās atšķirības, kas tika novērotas Latvijas fizioterapeitu un citu valstu fizioterapeitu rekomendācijās, bija par akupunktūras, kortikosteroīdu injekciju pielietošanu, papildus radioloģisko izmeklēšanu un papildus nosūtījumu pie ķirurga/ortopēda. Fizioterapeiti, kam ir 11 un vairāk gadu pieredze sniedz atšķirīgas rekomendācijas par akupunktūras (p=0,014) un kortikosteroīdu injekciju (p=0,018) pielietošanu pacientiem ar deģeneratīviem rotatoru aproces plīsumiem, salīdzinot ar fizioterapeitiem, kam darba pieredze ir mazāka. Secinājumi: Lielākā daļa terapijas tehnoloģiju, ko rekomendēja Latvijā praktizējoši fizioterapeiti, sakrita ar darbības jomas pārskatā identificētajām tehnoloģijām, ārstējot pacientus ar deģeneratīviem rotatoru aproces bojājumiem.

Title: Treatment technologies used by physiotherapists in patients with degenerative rotator cuff injuries: a scoping review and characteristics of physiotherapist’s clinical practice in Latvia. Objective: 1. To identify and systematize data on physiotherapeutic technologies used by physiotherapists in outpatient treatment for patients with degenerative rotator cuff injuries from the latest scientific literature by conducting a scoping review. 2. To explore the physiotherapeutic technologies used in their practice by physiotherapists practicing in Latvia in outpatient treatment for patients with degenerative rotator cuff injuries. Methods: A scoping review followed by a quantitative cross-sectional study. The first part involves literature research and compilation to create a scoping review based on the guidelines of the Joanna Briggs Institute (JBI). In the second part, permission was obtained to translate and adapt the questionnaire "Rotator cuff disorders: a survey of current UK physiotherapy practice" into Latvian. Using the questionnaire, quantitative data is collected from physiotherapists practicing in Latvia to determine the physiotherapy technologies they use when working with patients with degenerative rotator cuff injuries. The obtained data is compared with the results of the scoping review. Results: The main therapeutic technologies identified during the scoping review coincided with the therapeutic technologies recommended by the surveyed physiotherapists practicing in Latvia for treating patients with degenerative rotator cuff injuries: patient education/information (85.9%); range of motion development (25.7%); therapeutic exercises (62%); manipulations/mobilizations (36.6%); massage (53.5%); physical therapy (22.5%); and injections (14.1%). The main differences observed in the recommendations of Latvian physiotherapists compared to those from other countries were regarding the use of acupuncture, corticosteroid injections, additional radiological examinations, and additional referrals to a surgeon/orthopedist. Physiotherapists with 11 or more years of experience provided different recommendations on the use of acupuncture (p=0.014) and corticosteroid injections (p=0.018) for patients with degenerative rotator cuff tears compared to physiotherapists with less experience. Conclusions: Most of the therapeutic technologies recommended by physiotherapists practicing in Latvia coincided with the technologies identified in the scoping review for treating patients with degenerative rotator cuff injuries.
