Daudzkritēriju lēmuma analīzes pielietošana reto zāļu farmakoekonomiskās novērtēšanas optimizācijai.

Abstract

Pieņemot lēmumus par jaunu veselības aprūpes tehnoloģiju cenu noteikšanu un kompensācijas apmēra piemērošanu, farmakoekonomiskie novērtējumi, piem., izmaksu efektivitātes analīze un ietekmes uz budžetu noteikšana, bieži vien, ir nepietiekami, lai noteiktu zāļu reti sastopamu slimību ārstēšanai un cita veida terapijas patieso vērtību. Gan dažādi sociālie un ētiskie apsvērumi, gan ierobežoto klīnisko pētījumu, kā arī augsto pētniecības un izstrādes izmaksu dēļ, klasiskais jeb ierastais farmakoekonomiskās novērtēšanas modelis ierobežo lēmuma pieņemšanas procesus pozitīva izmaksu lietderīguma iznākuma iegūšanā, rezultātā, kavējot ārstniecības terapijas pieejamību reto slimību (RS) pacientu grupām. Esošā situācija pamato nepieciešamību izveidot un iekļaut zāļu reti sastopamu slimību ārstēšanai farmakoekonomiskajā novērtējumā jaunu, diferencētu sistēmu, kas ietvertu sevī visaptverošu kritēriju kopumu, strukturētu lēmumu pieņemšanai. Pašlaik Latvijā nav izveidota vienota, unificēta DLA metodoloģija, kas būtu pielāgota reti sastopamu slimību ārstēšanai paredzēto zāļu farmakoekonomiskajai novērtēšanai. Pamatojoties uz tēmas aktualitāti, autors nosaka darba mērķi – izveidot daudzkritēriju lēmuma analīzes (DLA) modeli un noteikt tā potenciālo ietekmi zāļu reti sastopamu slimību ārstēšanai farmakoekonomiskās novērtēšanas optimizācijai. Darbam izvirzīti seši uzdevumi: izpētīt DLA teorētiskos aspektus un citu valsti praksi DLA nepieciešamo kritēriju atlasei; izveidot DLA modeli; noskaidrot un raksturot esošo situāciju zāļu reti sastopamu slimību ārstēšanai pieejamībā Latvijā; pielietojot autora izveidoto DLA modeli, veikt zāļu reti sastopamu slimību ārstēšanai farmakoekonomisko novērtējumu; salīdzināt savā starpā Zāļu valsts aģentūras atzinumus un DLA modeļa iegūtos novērtējuma rezultātus; sagatavot priekšlikumus DLA modeļa ieviešanai zāļu novērtēšanas kontekstā. Pētījuma rezultātā autors secina, ka DLA metodoloģija ir sarežģīts, taču papildinošs elements zāļu farmakoekonomiskajā novērtēšanā, lai izveidotu objektīvāku pierādījumu bāzi lēmuma pieņemšanai par konkrēto zāļu iekļaušanu valsts budžeta finansētajās programmās. Darbā izveidotā un sekmīgi aprobētā DLA metodika zāļu reti sastopamu slimību ārstēšanai farmakoekonomiskajai novērtēšanai sniedz praktiskus priekšlikumus, kas valstiskā līmenī varētu nodrošināt vienotus kritērijus zāļu novērtēšanā.

Pharmacoeconomic assessments, such as cost-effectiveness analysis and budget impact analysis, are often insufficient to determine the true value of medicines for rare diseases when deciding on the pricing and application of reimbursement rates for new healthcare technologies. Both for different social and ethical considerations, and due to limited clinical trials and high research and development costs, the usual model of pharmacoeconomic evaluation limits decision-making processes in obtaining a positive cost- effectiveness outcome, resulting in hindering access to therapeutic options for rare disease patient groups. The current situation justifies the need to develop and include rare disease medicinal products in the pharmacoeconomic evaluation of a new, differentiated system for structured decision-making, comprising a comprehensive set of criteria. Currently, Latvia does not have a single, unified MCDA methodology adapted to the pharmacoeconomic evaluation of drugs for the treatment of rare diseases. The aim of this study was to create a MCDA model and determine its potential impact on drugs for the treatment of rare diseases for the optimization of pharmacoeconomic evaluation. The thesis has six objectives: to study the theoretical aspects of MCDA and the practices of other countries for the selection of criteria necessary for MCDA; to create a MCDA model; to clarify and characterize the existing situation in the availability of drugs for the treatment of rare diseases in Latvia; using the MCDA model created by author, to carry out a pharmacoeconomic evaluation of drugs for the treatment of rare diseases; to compare among themselves the opinions of the State of Medicines agency and the results of the evaluation of the MCDA model; prepare proposals for the implementation of the MCDA model in the context of the evaluation of medicinal products. As a result of the study, the author concludes that the MCDA methodology is a complex but complementary element in the pharmacoeconomic evaluation of drugs to establish more objective evidence base for deciding on the inclusion of a particular drug in programs financed by the state budget. The MCDA methodology for the pharmacoeconomic evaluation of drugs for the treatment of rare diseases, established and successfully approved in the work, provides practical proposals that could provide uniform criteria for drug evaluation at the state level.
