Tuberkulozes pacientu izpratne par savu slimību un viņu pieredze komunikācijā ar ārstniecības personām terapijas gaitā

Abstract

Maģistra darba tēma ir “Tuberkulozes pacientu izpratne par savu slimību un viņu pieredze komunikācijā ar ārstniecības personām terapijas gaitā”. Tēmas aktualitāte: lai arī tuberkuloze (TB) ir profilaktiski novēršama un ārstējama slimība, tā joprojām ir nopietns globālās veselības apdraudējums. Latvijā saslimstība ar TB ir gandrīz divas reizes augstāka nekā vidēji Eiropas Savienībā. TB pacienti ārstēšanas procesā saskaras ar dažādiem sarežģījumiem, kas saistīti ar ilgstošo un komplicēto ārstēšanas kursu, sociālekonomiskajiem faktoriem, psihiskās veselības sarežģījumu riskiem un arī slimības stigmatizāciju. Pētījumi liecina, ka pacientu slimības izpratne ietekmē ar veselību saistīto uzvedību un arī rīcību veselības apdraudējuma gadījumā. Pacientu slimības izpratnes izzināšana sniedz iespēju identificēt grūtības, ar kurām pacienti var saskarties, lai pārvarētu slimību, kā arī uzlabot slimības izpratni, mazināt negatīvos priekšstatus un veicināt slimības pieņemšanu. Pētījuma mērķis ir noskaidrot, kā komunikācija ar ārstniecības personām sekmē vai kavē TB pacientu izpratni par slimību. Pētījumam izvēlēta kvalitatīva pētījuma pieeja, veicot daļēji strukturētas intervijas ar 15 TB pacientiem, kuri saņem ambulatoro terapiju Rīgā. Veicot interviju tematisko analīzi identificētas tēmas, kas raksturo TB pacientu slimības izpratni un komunikācijas pieredzi ar ārstniecības personām. 6 galvenās atklātās tēmas attiecas uz dažādajiem priekšstatiem par TB un izaicinājumiem TB slimības pieredzē; subjektīvajiem slimības skaidrojumiem un pacientu pieredzi līdz diagnozes noskaidrošanai; pacientu informācijas avotiem, lai gūtu zināšanas par TB, kā arī pacientiem svarīgākos aspektus terapijas gaitā un komunikācijas ar ārstniecības personām nozīmi. Šī pētījuma rezultāti apstiprina arī citos pētījumos atklātos secinājumus un izvirzītās problēmas par TB slimības stigmatizāciju, kas liecina par nepieciešamību uzlabot TB pacientu, viņu tuvinieku un arī sabiedrības zināšanas un izpratni par TB slimību. Efektīva pacientu un ārstniecības personu komunikācija un pacientu izglītošana var mazināt ar slimību saistītos negatīvos priekšstatus, pacientu bažas un nedrošības sajūtu, kā arī uzlabot zināšanas par slimības pārvaldīšanu un veicināt pacientu aktīvāku iesaisti veselības uzlabošanā.

The topic of the master thesis is “Tuberculosis patients' perceptions of their illness and their experiences of communication with health professionals during treatment”. Relevance of the topic: although tuberculosis (TB) is a preventable and treatable disease, it remains a serious global health threat. In Latvia, the incidence of TB is almost twice as high as the EU average. TB patients face several challenges during their treatment, related to the long and complex course of treatment, socio-economic factors, the risk of psychological health complications and the stigma associated with the disease. Research shows that patients' perceptions and beliefs about their illness have a major impact on their health-related behaviors, as well as their responses to health risks. Understanding patients' perceptions of illness provides an opportunity to identify the difficulties they may face in coping with their illness, improve patients` illness perceptions, reduce negative beliefs and promote acceptance of illness. The aim of this study is to investigate how communication with healthcare professionals facilitates or hinders TB patients' illness perceptions. A qualitative research approach was chosen for the study, conducting semi-structured interviews with 15 TB patients receiving outpatient treatment in Riga. Thematic analysis of the interviews identified themes describing TB patients’ illness perceptions and experiences of communication with healthcare professionals. The 6 main themes relate to different perceptions of TB and challenges in the TB illness experience; subjective explanations of the illness and patients experiences up to diagnosis; patients’ sources of information to gain knowledge about TB; the most important aspects for patients during treatment and the importance of communication with healthcare professionals. The results of this study confirm the findings and concerns of other studies on TB stigma, which point to the need for improvement in the knowledge and understanding of TB among TB patients, their families and society. Effective communication between patients and healthcare professionals and patient education can reduce negative perceptions related to the illness, patient anxiety and feelings of insecurity, as well as improve knowledge of illness management and promote more active involvement of patients in improving their health.
