Ārstu rakstiskās rekomendācijās izmantotās valodas un tās vēstījuma izpratne pacientu komunikatīvās veselībpratības kontekstā

Abstract

Maģistra darba tēma ir “Ārstu rakstiskās rekomendācijās izmantotās valodas un tās vēstījuma izpratne pacientu komunikatīvās veselībpratības kontekstā.” Tēmas nozīmību nosaka Berila institūta 2020.gada pētījuma dati par pacientu pieredzi un vēlmi, kā arī Pikera institūta uz cilvēku centrētas veselības aprūpes 1 no 8 noteiktajiem principiem, saņemt informāciju skaidrā un saprotamā valodā. Viens no Latvijas veselibas politikas pamatprincipiem ir uz cilvēku centrēta veselības aprūpe, kas ietver skaidras un labi saprotamas informācijas pieejamību. Veselībpratība ir pacienta spējas atrast, saprast un izmantot informāciju, kas ietekmē viņa spējas izprast sniegto informāciju. Darba mērķis ir novērtēt Latvijas ārstu komunikācijā izmantotās tipiskās informācijas vienības (medicīnas terminus, frāzes), izveidot un testēt komunikatīvās veselībpratības novērtēšanas rīka prototipu, kas palīdz novērtēt, vai ārstu rakstiskās rekomendācijas ir cilvēkam izprotamas. Teorētisko bāzi veido literatūras apskats par pratības ideoloģisko modeli, veselībpratības definīcijas, tās koncepta attīstība, tās nozīmi veselības aprūpē, komunikācijā un uzvedības maiņā, kā ārī ārstu valodas nozīmi komunikācijā ar pacientu. Darbā izmantota jaukta pētījuma stratēģija. Pirmajā posmā tika veikta atlasīto pilngadīgu personu stacionāru ārstu rekomendāciju kvalitatīva un kvantitatīva kontentanalīze. Otrajā posmā veiktas ekspertu intervijas un to tematiska analīze, iegūtu padziļinātu izpratni par ārstu-pacientu komunikāciju ietekmējošiem faktoriem, tās uzlabošanas iespējām. Trešājā posmā tika izveidots un testēts komunikatīvās veselībpratības rīks pacientu izpratnes par ārstu rekomendācijām novērtēšanai. Darba galvenais secinājums – Latvijas ārstu rakstiskās rekomendācijas ir viegli lasāmas, bet to saturs neatbilst uz cilvēku centrētas veselības aprūpes principiem, neatbilstību ietekmē ārstu laika un komunikācijas prasmju trūkums jeb organizācijas veselībpratība, Latvijas veselības aprūpes organizācijas īpatnības un papildus drošticamas informācijas resursu trūkums jeb valsts kā organizācijas veselībpratība. Lai uzlabotu komunikācijas, tostarp rakstisko rekomendāciju, kvalitāti, veselības aprūpē Latvijā, nepieciešams uzlabot izpratni par komunikācijas nozīmi tajā un veidot atbalsta sistēmu gan veselības aprūpes sniedzējiem, gan saņēmējiem.

ABSTRACT The topic of the master thesis is "Comprehension of the language used and the message it contained in doctors' written recommendations in the context of patient's communicative health literacy". The relevance of the topic is determined by the data from the 2020 Beryl Institute study on patient experience and preferences, as well as by 1of 8 principles of person-centred healthcare, as defined by the Picker Institute, to receive information in clear and understandable language. One of the key principles of Latvian health policy is person-centred healthcare, which includes the availability of clear and well-understood informationHealth literacy is the patient's ability to find, understand and use information, which affects their ability to understand the information provided. The aim of the work is to assess the typical units of information (medical terms, phrases) used in Latvian doctors' communication, to create and test a prototype of a tool for assessing communicative health literacy, which helps to assess whether doctors' written recommendations are comprehensible to people. The theoretical basis consists of a literature review on the ideological model of literacy, the definition of health literacy, the development of its concept, its role in healthcare, communication and behaviour change, and the role of physicians' language in communication with patients. The thesis uses a mixed research strategy. In the first stage, a qualitative and quantitative content analysis of the recommendations of hospital physicians of adults was selected. In the second stage, expert interviews and their thematic analysis were conducted, gaining a deeper understanding of the factors influencing doctor-patient communication, the possibilities of its improvement. The main conclusion of the paper is that the written recommendations of Latvian doctors are easy to read, but their content does not comply with the principles of person-centred health care, and that the mismatch is influenced by the lack of time and communication skills of doctors, or organisational health literacy, the peculiarities of the Latvian health care organisation, and the lack of additional reliable information resources, or the health literacy of the state as an organisation. To improve the quality of communication, including written recommendations, in Latvian healthcare, it is necessary to raise awareness of the importance of communication and to develop a support system for both healthcare providers and recipients.
