Dažādu A vitamīna atvasinājumu lietošanas iespējas un blakusparādības ādas problēmu ārstēšanā .

Abstract

A vitamīna un to atvasinājumu jeb retinoīdu izpēte dermatoloģijā iesākās vēl pagājušā gadsimta sākumā, un kopš tā brīža interese un informācijas daudzums par tiem turpina strauji augt. Retinoīdiem tiek dota priekšroka ādas problēmu ārstēšanā pateicoties to spējai ietekmēt dažādus šūnu procesus, piemēram, veicināt šūnu proliferāciju, diferenciāciju un kolagēna sintēzi, kas padara retinoīdus par neaizstājamiem līdzekļiem aknes, fotonovecošanās, ekzēmas, rozācijas, psoriāzes un citu ādas slimību ārstēšanā. Taču, neskatoties uz retinoīdu labvēlīgām īpašībām, to blakusparādību spektrs ir diezgan plašs, sākot no neliela ādas kairinājuma un sausuma līdz pat retinoīdu izraisītām dermatītam, kas apgrūtina to pielietošanu. Šī iemesla dēļ šobrīd tiek izstrādāti jaunāki retinoīdi ar mērķi sniegt lielāku efektu ar mazāku blakusparādību iespējamību. Šī pētījuma mērķis ir, apkopojot uz pierādījumiem balstīto literatūru, salīdzināt pašreiz ādas ārstēšanas praksē pielietojamus retinoīdus, kā arī noteikt, kurš vai kuri no tiem būtu efektīvāki plašāka darbības spektra ziņā ar pēc iespējas mazāku blakusparādību iespējamību vai smaguma pakāpi. Veicot zinātniskās literatūras analīzi tika noteikts, ka izotretinoīns jeb 13-cis-retīnskābe var tikt pielietots daudz plašākam indikāciju skaitam, salīdzinot ar citiem retinoīdiem, turklāt retinaldehīds jeb retināls ir drošākais retinoīds, pateicoties tā pakāpeniskai maigai iedarbībai un vismazākajām blakusparādību daudzumam un smagumam.

The study of vitamin A and its derivatives or retinoids in dermatology began at the beginning of the last century, and since then the interest and amount of information about them continues to grow rapidly. Retinoids are preferred in the treatment of skin problems due to their ability to influence various cellular processes, such as promoting cell proliferation, differentiation and collagen synthesis, which makes retinoids indispensable in the treatment of acne, photoaging, eczema, rosacea, psoriasis and other skin diseases. However, despite the beneficial properties of retinoids, their spectrum of side effects is also quite wide, ranging from minor skin irritation and dryness to retinoid-induced dermatitis, which complicates their use. For this reason, newer retinoids are currently being developed with the aim of providing a greater effect with less potential for side effects. The aim of the study (The possibilities and side effects of usage of different vitamin A derivatives in treatment of skin problems) is to compare retinoids, that are currently used in skin treatment practices by gathering evidence-based literature, as well as to determine which ones of them would be more effective in terms of a wider spectrum of action with the least possibility and severity of side effects. By analyzing the scientific literature, it was determined that isotretinoin or 13-cis-retinoic acid can be used for a much wider number of indications compared to other retinoids, moreover, retinaldehyde or retinal turned out to be the safest retinoid due to its gradual gentle effect and the least amount and severity of side effects.
