Primāras ekstrakraniālas (sinonazālas) meningeomas klīniskā gadījuma analīze

Abstract

Paciente ir 73 gadus veca sieviete, kurai sūdzības par aizliktu degunu un izdalījumiem kopš 2023. gada februāra. Ap 2023. gada septembri pašsajūta esot stipri pasliktinājusies un 21.11.2023. vērsusies pēc palīdzības akūti PSKUS, kur pacienti stacionēja 15. nodaļā - otorinolaringoloģijā. Primāri bija aizdomas par polipozas dabas veidojumu. Diagnozes precizēšanai veikta biopsija no veidojuma, kā arī DT un MR galvai. Biopsijā iegūtais materiāls nosūtīts morfoloģiskai un mikrobioloģiskai izmeklēšanai. 27.11.2023. mikrobioloģiskā materiāla uzsējumā tika atrasts Staplhylococcus aureus, un uzsāka atbilstošo ārstēšanu ar Oksacilīns 12 g/dn IV, devu sadalot 4 - 6 ievadīšanas reizēs (ik 4 vai 6 stundas), kura rezultējās ar pacientes stāvokļa uzlabošanos. 30.11.2023. patohistoloģiskā izmeklējuma rezultāts: morfoloģiskā aina atbilst sinonazālai glomangiopericitomai (miopericitoma), labdabīgs tumors. 05.12.2023. tiek sasaukts onkokonsīlijs, kura ietvaros apspriež pacientes tālāko ārstēšanu un nolemj par audzēja intranazālās daļas rezekciju. Pacientei tiek sniegta informācija par tālāko taktiku. Ierodoties 18.12.2023. PSKUS, pacientei veikta operācija, kuras laikā iegūti preparāti histoloģiskai izmeklēšanai: veidojums no labās deguna ejas, no kreisās deguna ejas un no deguna starpsienas. Ņemot vērā morfoloģisko ainu un imūnhistoķīmiskās izmeklēšanas rezultātus, kā arī nosūtījumā norādīto veidojuma lokalizāciju, 05.01.2023. saņemtais histoloģiskais slēdziens - sinonazāla meningoteliāla meningioma (Grade 1 tumors pēc PVO klasifikācijas).

The patient is a 73-year-old woman who has been complaining of a blocked nose and nasal discharge since February 2023. Her condition worsened significantly around September 2023, and on November 21, 2023, she sought help at the PSKUS, where she was admitted to the 15th department - otolaryngology. There were primary suspicions of a polypoid nature formation. To further diagnose the condition, a biopsy was performed on the formation, as well as CT and MRI scans of the head. The material obtained from the biopsy was sent for morphological and microbiological examination. On November 7, 2023, Staphylococcus aureus was found in the microbiological culture, and appropriate treatment with Oxacillin 12 g/day IV, divided into 4 - 6 doses (every 4 or 6 hours), was initiated, resulting in improvement of the patient's condition. On November 30, 2023, the result of the pathological examination showed a morphological pattern consistent with sinonasal glomangiopericytoma (myopericytoma), a benign tumor. On December 5, 2023, an oncology consultation was convened to discuss the patient's further treatment, and it was decided to perform resection of the intranasal portion of the tumor. The patient was provided with information about the further treatment plan. Upon arrival at the PSKUS on December 18, 2023, the patient underwent surgery, during which preparations were obtained for histological examination: a formation from the right nasal passage, from the left nasal passage, and from the nasal septum. Considering the morphological appearance and the results of immunohistochemical examination, as well as the localization of the formation indicated in the referral, the histological conclusion received on January 5, 2023, was - sinonasal meningothelial meningioma (Grade 1 tumor according to the WHO classification).
