Vitiligo ārstēšana ar melanocītu transplantāciju un tās efektivitāte

Abstract

Vitiligo ir dermatoloģiska slimība, ko raksturo ādas pigmentācijas traucējumi, kuras izplatība visā pasaulē ir 0,5-2% iedzīvotāju. Klīniski slimība izpaužas kā skaidri norobežoti dažādas formas un lieluma balti plankumi, ko izraisa samazināts melanocītu skaits. Vitiligo nav infekcioza vai dzīvībai bīstama slimība, taču tā var izraisīt sociālās un psihoemocionālas problēmas un samazināt pacientu dzīves kvalitāti. Tagad ir pieejamas dažādas ārstēšanas metodes, kas palīdz uzlabot ādas izskatu. Ja visas pieejamās neinvazīvās ārstēšanas metodes ir neefektīvas, palīgā var nākt ķirurģiska ārstēšana, kas ir ļoti laba alternatīva grūti ārstējamu, bet stabilu vitiligo izmaiņu gadījumos. Jauno ārstēšanas metožu pētijumi un jau esošo ārstēšanas metožu pilnveidošana, izmantojot šūnu un audu transplantāciju, dod cerību daudziem pacientiem ar vitiligo visā pasaulē. Izvēlētās metodes efektivitāte ir atkarīga no tā, cik piemērota tā ir konkrētā gadījuma atkarībā no bojājuma īpatnībām. Katram ķirurģiskās iejaukšanās veidam ir savas priekšrocības un trūkumi, kas, jāņem vērā kopā ar hipopigmentācijas zonas atrašanās vietu un izmēru. Šī darba mērķis is aprakstīt dažādu ķirurģisku metožu pielietojumu vitiligo ārstēšanā, lai saprastu, kura no metodēm ir efektīvāka. Literatūras apskatā tika secināts, ka ķirurģiska ārstēšana ir viens no efektīvākajiem ārstēšanas veidiem stabilas vitiligo slimības gadījumā. Kā arī plānas ādas transplantācija ir metode, ar kuru vislielākajam ārstēto pacientu skaitam tiek sasniegta > 90 % repigmentācijas pakāpe. Melanocītu transplantācija ir efektīvs vitiligo ārstēšanas veids, kas salīdzinājumā ar citiem ārstēšanas veidiem, ļauj ievērojami atjaunot pigmentāciju depigmentētajās zonās un uzlabot kosmētisko izskatu.

Vitiligo is a dermatological disease characterised by pigmentation disorders of the skin with a worldwide prevalence of 0.5-2%. Clinically, the disease presents as clearly demarcated white patches of different shape and size caused by a reduced number of melanocytes. Vitiligo is not an infectious or life-threatening disease, but it can cause social and psychoemotional problems and reduce patients' quality of life. There are now various treatments available to help improve the appearance of the skin. If all available non-invasive treatments are ineffective, surgical treatment can be a very good alternative for difficult-to-treat but stable vitiligo changes. Research into new treatments and the development of existing treatments using cell and tissue transplantation offer hope to many vitiligo patients around the world. The effectiveness of the chosen method depends on how appropriate it is to the specific case, depending on the nature of the lesion. Each type of surgical intervention has its own advantages and disadvantages, which must be taken into account together with the location and size of the hypopigmentation area. The aim of this paper is to describe the use of different surgical techniques in the treatment of vitiligo in order to understand which technique is more effective. The literature review concluded that surgical treatment is one of the most effective treatments for stable vitiligo. Thin skin grafting is also the method that achieves >90% repigmentation in the highest number of patients treated. Melanocyte transplantation is an effective treatment for vitiligo, allowing significant restoration of pigmentation in depigmented areas and improvement of cosmetic appearance compared to other treatments.
