Sistēmisko psoriāzes ārstēšanas iespēju pārskats pediatriskiem pacientiem

Abstract

Šī darba mērķis ir apskatīt sistēmiskās psoriāzes ārstēšanas iespējas pediatriskiem pacientiem – to efektivitāti, terapijas vadību, pacienta uzraudzību un iespējamās blaknes, liekot uzsvaru uz pēdējo gadu pētījumu rezultātiem un Eiropas Zāļu aģentūrā (EMA) apstiprinātiem medikamentiem. Darba uzdevumi ir: 1. noskaidrot kāda ir psoriāzes ietekme uz cilvēka veselību pacientiem, kas slimo bērnībā. 2. noskaidrot kāda ir fototerapijas loma psoriāzes ārstēšanā pediatriskiem pacientiem. 3. noskaidrot, vai konvencionālā sistēmiskā terapija psoriāzes ārstēšanā bērniem ir pietiekami droša un efektīva. 4. noskaidrot, kāda ir bioloģisko medikamentu loma psoriāzes ārstēšanā bērniem. 5. Ar kādām ārstēsanas līdzekļiem tiek rekomendēts uzsakt sistēmisku terapiju pediatriskiem psoriāzes pacientiem.

Apskatā izmantotās literatūras atlase veikta, izmantojot ExLibris PRIMO vienoto informācijas meklēšanas rīku (elektroniskās platformas PubMed, Key Elsevier, EBSCO, UpToDate uc). Tika lietoti atslēgvārdi 'pediatric psoriasis' kombinējot ar vārdiem 'treatment', 'guidelines', 'phototherapy', 'topical', ‘systemics’, ’biologics’ un atsevišķu medikamentu nosaukumiem. Tika atlasīti darbi angļu valodā, kuros atspoguļotas viena vai vairākas psoriāzes ārstēsanas iespējas pediatriskiem pacientiem, pētījumi izmantoti liekot uzsvaru uz tiem, kas publicēti pēc 2020. gada. Tāpat tika izmantotas Amerikas dermatoloģijas akadēmijā un Nīderlandē izstrādātās vadlīnijas, kā arī autorei iepriekš zināmas publikācijas, kurās iekļauta vērtīga informācija šī darba izstrādei. Literatūras apskata veidošanai kopumā izmantots 68 avoti - literatūras apskati, klīnisko gadījumu sērijas, klīnisko pētījumu pārskati, ārstēšanas vadlīnijas uc. Visus darbā izmantotos avotus – tekstu un tabulas ir tulkojusi autore. Darbā apskatītās terapijas metodes grupētas 3 grupās. Fototerapija – ārstēšanas metodes kurās tiek izmantota redzamā un ultravioletā gaisma. Tradicionālā sistēmiskā terapija – ārstēšanas metodes kurās tiek izmantoti medikamenti – metotreksāts, ciklosporīns, acitretīns. Bioloģiskā terapija – ārstēšanas metodes kurās izmanto bioloģiskos medikamentus – TNF- alfa inhibitorus, IL-12/23 inhibitorus un IL-17A inhibitorus. Darbā aprakstīta katras grupas ārstēšanas metožu efektivitāte, faktori, kas jāņem vērā konkrēto terapiju uzsākot, pacienta uzraudzība un iespējamās blaknes terapijas laikā un pēc tās.

The objective of this paper is to review the systemic treatment options for psoriasis in pediatric patients - efficacy, therapy management, patient monitoring and potential adverse effects, with an emphasis on the results of recent studies and on drugs approved by the European Medicines Agency (EMA).

The tasks are: 1. to find out the impact of psoriasis on human health in patients with childhood psoriasis. 2. to understand the role of phototherapy in the treatment of psoriasis in pediatric patients. 3. to find out whether conventional systemic therapy for psoriasis in children is safe and effective enough. 4. to investigate the role of biologic drugs in the treatment of psoriasis in children. 5. to clarify which treatments are recommended as a first choice for systemic therapy in pediatric psoriasis patients.

The literature review was conducted using the ExLibris PRIMO unified information search tool (electronic platforms PubMed, Key Elsevier, EBSCO and UpToDate). Keywords 'pediatric psoriasis' were combined with words 'treatment,' 'guidelines,' 'phototherapy,' 'topical,' 'systemics,' 'biologics,' and individual drug names. Works in English were selected, reflecting one or more treatment options for pediatric patients, with an emphasis on those published after 2020. American and Dutch guidelines were also used, along with previously known publications containing valuable information for the development of this work. Literature review included a total of 68 references - literature reviews, case series, clinical study reviews, treatment guidelines and others. Therapeutic methods discussed in the paper are grouped into 3 groups. Phototherapy - treatment methods using visible and ultraviolet light. Traditional systemic therapy - treatment methods using drugs such as methotrexate, cyclosporine and acitretin. Biological therapies - treatments using biological drugs such as TNF-alpha inhibitors, IL-12/23 inhibitors and IL-17A inhibitors. The paper describes the efficacy of each group of treatments, factors to consider when initiating a particular therapy, patient monitoring and possible side effects during and after therapy.
