Abstract: | Berkita limfoma ir ļoti agresīvs Ne-Hodžkina limfomas veids, kas klīniski var izpausties
kā limfoma un leikēmija. Leikēmiskā klīniskā prezentācija ir ļoti reta. Diagnozes noteikšana
balstās uz pathistoloģiskiem izmeklējumiem, PĶR un FISH metodes rezultāteim. Lielākā daļa
gadījumu ir sasitīta ar c-MYC onkogēna mutāciju. Leikēmiskā forma pieaugušo populācijā ir
sastopama ļoti reti. Dotais klīniskais gadījums ir reta Berkita limfomas forma, kas klīniski
prezentējās kā leikēmija un klīniski atgādina akūtu limfoblastu leikozi.
Apskatītajā klīniskajā gadījumā pirmie simptomi attīstījās nedēļu pēc vakcinācijas pret
ērču encefalītu. Pacientei parādījās nespecifiski simptomi kā subfebrila temperatūra, locītavu un
muskuļu sāpes, sāpes vēderā, vājums un nespēks kā arī petehiāli izsitumi. Pilnas asins ainas
izmeklējumos tika kostatēta smaga trombocitopēnija un blastu šūnas perifērajās asinīs. Veicot
datortomogrāfijas izmeklēšanu vēdera dobumam tika konstatēta intraperitoneāla un
retroperitoneāla limfadenopātija. Tika veikta arī plūsmas citometrija, kaulu smadzeņu citoloģiskā
izmeklēšana, kur konstatēja Berkita limfomai raksturīgu fenotipu un ģenētisku mutāciju.
Paciente saņēma plašu farmakoloģisku terapiju adekvātai atsāpināšanai, iepreikšesošo
hronisko saslimšanu ārstēšanai, tumorlīzes sindroma novēršanai plašu antimikrobiālu terapiju,
profilaktiskus antifungālus medikamentus un profilaktisko antivirālo terapiju iespējamo infekciju
prevencijai imūnsupresijas dēļ. Slimības ārstēšanai paciente saņēma ķīmijterapiju pēc DAEPOCH-R protokola. Pielietotā terapija tika izvēlēta atbilstoši Pasaulē noteiktiem standartiem un
pacientes veselības stāvoklim. Izvērtējot pacientes stāvokli pēc BL-IPI indeksa 3 gadu dzīvildze
sastādija 53%. Pēc ķīmijterapijas kursa saņēmšanas paciente tika veiksmīgi izrakstīta mājās. Pēc
trīs nedēļām ar nodaļu sazinājās pacientes vīrs un paziņoja, ka paciente nomira.
Apskatot un analizējot klīnisko gadījumu un literatūru par Berkita limfomu ar leikēmisko
prezentāciju var secināt, ka šāda klīniskā prezentācija pieaugušo populācijā ir reti aprakstīta
datubāzēs iekļautos zinātniskos darbos, izvērtēt pacientes prognozi ir sarežģīti, jo Pasaulē nav
pieņemta vienota prognozes izvērtēšanas sistēma. Burkitt lymphoma is a highly aggressive type of non-Hodgkin lymphoma that clinically can manifest as lymphoma and leukemia. The leukemic clinical presentation is very rare. Diagnosis is based on pathological examinations, PCR, and FISH method results. The majority of cases are associated with a c-MYC oncogene mutation. The leukemic form is very rarely found in the adult population. The presented clinical case is a rare form of Burkitt lymphoma, clinically presenting as leukemia and resembling acute lymphoblastic leukemia. In the reviewed clinical case, the first symptoms developed a week after vaccination against tick-borne encephalitis. The patient exhibited nonspecific symptoms such as subfebrile temperature, joint and muscle pain, abdominal pain, weakness, and petechial rash. Complete blood count revealed severe thrombocytopenia and blast cells in the peripheral blood. Abdominal CT scan showed intraperitoneal and retroperitoneal lymphadenopathy. Flow cytometry and bone marrow cytological examination were performed, revealing a characteristic phenotype and genetic mutation of Burkitt lymphoma. The patient received extensive pharmacological therapy for symptom relief, treatment of pre-existing chronic illnesses, prevention of tumor lysis syndrome, antimicrobial therapy, prophylactic antifungal medication, and prophylactic antiviral therapy to prevent possible infections due to immunosuppression. Chemotherapy was administered according to the DAEPOCH-R protocol for disease treatment. The therapy was chosen in accordance with established global standards and the patient's health condition. Evaluation of the patient's condition based on the BL-IPI index indicated a 53% 3-year survival rate. The patient was successfully discharged home after completing chemotherapy. Three weeks later, the patient's spouse contacted the department and informed that the patient had passed away. Reviewing and analyzing the clinical case and literature on Burkitt lymphoma with leukemic presentation, it can be concluded that such a clinical presentation in the adult population is rarely described in scientific literature databases. Evaluating the prognosis of the patient is challenging because there is no universally accepted prognostic evaluation system globally. |
