Staged orthopedic treatment of a patient with rheumatoid arthritis

Abstract

Ievads: Reimatoīdais artrīts (RA) ir invaliditāti izraisoša slimība, kas prasa agrīnu diagnostiku un agresīvu ārstēšanu ar slimību modificējošiem pretreimatisma līdzekļiem (DMARD), lai iegūtu vislabāko funkcionālo rezultātu. Šajā klīniskā gadījuma pārskatā ir uzsvērta medikamentozās terapijas nozīme RA pacientiem saskaņā ar jaunākajām vadlīnijām un traumatoloģijas un ortopēdijas specialitātes nozīme pacientiem ar smagām locītavu deformācijam neskatoties uz medikamentozo terapiju. Klīniskais gadījums: 54 gadus veca sieviete, ar sarežģītu 28 gadu vēsturi ar ortopēdiskām operācijam saistībā ar smagām RA locītavu deformācijām, jo diagnozes noteikšanas brīdī terapijas vadlīnijas un iespējas bija ierobežotas, kā arī suboptimālu slimības aktivitātes mazināšanos uz pašreizējās ārstēšanas režīma fona. Secinājumi: Reimatoīdā artrīta arstēšanā ārkārtīgi svarīga ir multidisciplināra komanda, ko vada reimatologs, lai efektīvi palēninātu slimības progresēšanu un atvieglotu simptomus, pirms tie noved pie invaliditātes. Ortopēdiskā ķirurģija kopā ar reimatoloģisku bāzes terpaiju un fizioterapiju sniegs vislabāko iespēju pacientiem ar jau esošām sinoviālo locītavu deformācijām, kas ietekmē viņu ikdienas dzīvi, ierobežojot kustību amplitūdu (ROM) un radot sāpes slimajās locītavās. Tas ļauj arī pacientiem ar izmainītām locītavām dzīvot salīdzinoši normālu dzīvi, izvēloties veikt funkcionalitāti ierobežojošās locītavas nomaiņu pret endoprotezi vai artrodēzi.

Introduction: Rheumatoid arthritis (RA) is a severely disabling disease that requires early recognition and aggressive treatment with disease-modifying antirheumatic drugs (DMARDs) for the best functional outcome. The following case report highlights the importance of drug therapy in RA patients according to the latest guidelines and the field of orthopedics in managing joint involvement in patients with unsatisfactory responses to drug therapy. Case Report: We present a case of a 54-year-old female with a complex 28-year history of orthopedic interventions for RA-related joint deformities due to a lack of available treatment options at the time of diagnosis and poor response to the current treatment regimen. Conclusion: In the management of RA, a multidisciplinary team led by a rheumatologist is of utmost importance to effectively slow disease progression and alleviate the symptoms before they lead to disability. Orthopedic management in conjunction with rheumatological treatment and physiotherapy will offer the best opportunity for patients with already established synovial joint deformities affecting their everyday lives, through limitation of range of motion (ROM) and pain in the diseased joints, to lead relatively normal lives by performing arthroplasty or arthrodesis for the functionality limiting joint.
