Abstract
Ievads.
Pie varavīksnenes fiksējamo iris claw veida lēcu implantācija acs priekšējā kamerā ir bieža ārstēšanas metode primāri ievietotu intraokulāru lēcu dislokāciju gadījumā. Operācijas dēļ radušais astigmātisms var ietekmēt redzes asumu un radusies zīlīšu ovalizācija var veidot zīlītes asimetriju. Pētījuma mērķis ir izvērtēt saistību starp intraokulārās iris claw veida lēcas novietojumu acs priekšējā kamerā ar minētajām acs pārmaiņām pēc operācijas.
Materiāli un metodes.
Mērījumi uz pacientu acu priekšējo kameru fotouzņēmumiem tika izmantoti, lai nolasītu ievietoto iris claw veida lēcu fizikālo asu novietojumu grādos attiecībā pret horizontālu asi. Zīlīšu augstums un platums tika mērīti, lai izrēķinātu zīlīšu ovalizācijas attiecību – ja tā ir lielāka par 1,00, tad tas norāda uz zīlītes ovalizāciju. Lai izvērtētu operēto acu astigmātisma parametrus, tika izmantoti postoperatīvie autorefraktometra dati. Visi ievāktie dati tika apkopoti un analizēti, izmantojot Microsoft Excel un IMB SPSS Statistics 22 programmas.
Rezultāti.
Tika izvērtētas desmit pacientu (5 vīrieši, 5 sievietes, vidējais vecums 76,9 gadi) desmit acis. Vidējais postoperatīvais astigmātisms bija 2,10 D ± 1,01 (SD), vidējais astigmātisma ass leņķis bija 66,7° ± 57,9 (SD), vidējais iris claw veida lēcas leņķiskais novietojums bija 6,6° ± 3,3 (SD). Korelācijas koeficients starp iris claw veida lēcas novietojumu grādos un astigmātisma ass grādiem bija - 0,599 (p=0,117). Zīlīšu parametru attiecība bija sākot no 1,00 līdz 1,24 ar vidējo parametru 1,11 ± 0,09 (SD). Korelācijas koeficients starp iris claw veida lēcas novietojumu grādos un zīlītes ovalizāciju bija 0,540 (p=0,133).
Secinājumi.
1. Ir novērojama mērena korelācija starp iris claw veida lēcas leņķisko novietojumu un postoperatīvā astigmātisma ass leņķi.
2. Rezultāti norāda uz iris claw veida lēcas leņķiskā novietojuma ietekmi uz pacienta acu postoperatīvajām pārmaiņām – uz astigmātisma paramteriem ar klīnisku nozīmi un uz zīlīšu ovalizāciju ar gan klīniski, gan estētisku nozīmi.
Objectives. Iris claw type lens implantation in an anterior chamber is a common treatment method for dislocation of primary inserted intraocular lens. Surgery induced astigmatism can affect visual acuity and pupil ovalisation can cause pupilar asymmetry. The aim of the study is to evaluate the correlation between iris claw lens position in the anterior chamber with postoperative astigmatism and pupil ovalisation. Materials and methods. Measurements on patients eyes anterior chamber photos were used to note iris claw lenses physical axis localisation in degrees in regard to a horizontal axis. The width and height of the pupil was measured to calculate an ovalisation ratio, which was used to evaluate the pupil ovalisation where ratio greater than 1,00 showed ovalisation. Patients postoperative autorefractometry data were used to evaluate the operated eyes astigmatism. All acquired data were analysed using Microsoft Excel and IMB SPSS Statistics 22. Results. Ten eyes of ten patients (5 male, 5 female, average age 76,9 yr.) were evaluated. The postoperative mean astigmatism value was 2,10 D ± 1,01 (SD), mean axis angle - 66,7° ± 57,9 (SD), mean iris claw lens angle - 6,6° ± 3,3 (SD). The correlation coefficient of the location of iris claw lens in degrees and astigmatism axis degrees was - 0,599 (p=0,117). Pupils ovalisation ratios were ranging from 1,00 to 1,24 with the mean of 1,11 ± 0,09 (SD). The correlation coefficient of iris claw lens location in degrees and pupil ovalisation ratio was 0,540 (p=0,133). Conclusions. 1. A trend of moderate correlation between the iris claw lens angle and the postoperative astigmatism axis angle was noted. 2. The results show the impact of intraocular iris claw lens position to the postoperative changes in patients’ eye – as astigmatism with clinical and pupil ovalisation with both aesthetical and clinical significance.
