Immune Reconstitution Inflammatory Syndrome (IRIS): Problems and Solutions

Abstract

Imūnās atjaunošanas iekaisuma sindroms (IRIS) ir sarežģīta parādība, kas var rasties cilvēkiem ar novājinātu imūnsistēmu, piemēram, tiem, kam ir HIV infekcija, orgānu transplantācija vai vēža ķīmijterapija. IRIS sastopamības biežums atšķiras atkarībā no pamatslimības, augstāku biežumu novērojot personām ar progresējošu HIV slimību vai tiem, kam tiek veikta orgānu transplantācija. Ir divi IRIS veidi: paradoksālais un atmaskojošais. Paradoksāla IRIS rodas, ja iepriekš zināma oportūnistiska infekcija pirms pretretrovīrusu terapijas (ART) sākšanas izraisa simptomu pasliktināšanos, neskatoties uz HIV infekcijas atveseļošanos. Paradoksālā IRIS pārstāv lielāko daļu IRIS. IRIS atmaskojošais veids rodas, kad terapijas laikā tiek atklāta jauna oportūnistiska infekcija. Autore aplūko dažādus IRIS aspektus un tās izpausmes. Ar šo viņš norāda uz svarīgākajām problēmām, kuras viņš varētu atrast, pārskatot literatūru. Viņš pārskatīja IRIS attiecībā uz tās sastopamību, diagnostikas vienprātību, IRIS izpausmēm, kas nav saistītas ar HIV, patofizioloģiju, mirstību un saslimstību un, visbeidzot, ārstēšanu. Pēc katras nodaļas ar izklāstītajām problēmām viņš norāda iespējamos risinājumus katrai no šīm problēmām. Noslēgumā viņš apkopo pārskatu un sniedz ieskatu par pašreizējo pētījumu stāvokli IRIS.

Immune Reconstitution Inflammatory Syndrome (IRIS) is a complex phenomenon that can occur in individuals with weakened immune systems, such as those with HIV infection, organ transplantation, or cancer chemotherapy. The incidence of IRIS varies depending on the underlying condition, with higher rates observed in individuals with advanced HIV disease or those who undergo organ transplantation. There are two types of IRIS: The paradoxical and the unmasking type. Paradoxical IRIS occurs when a previously known opportunistic infection prior to initiation of antiretroviral therapy (ART) leads to worsened symptoms despite recovery of the HIV infection. Paradoxical IRIS represents the majority of IRIS. The unmasking type of IRIS occurs when during therapy a new opportunistic infection is discovered. The author takes a look at different aspects of IRIS and its manifestations. Hereby he points out the most important problems which he could find by reviewing the literature. He reviewed IRIS regarding its incidence, diagnostic consensus, non – HIV manifestations of IRIS, pathophysiology, mortality and morbidity and, lastly, the treatment. After each chapter with the presented problems he points out the possible solutions to each of those problems. In the conclusion he summarizes the review and gives an outlook on the current state of research in IRIS.
