Abstract: | Parkinsona slimību (PS) joprojām galvenokārt diagnosticē, izmantojot klīniskos testus un reizēm pozitronu emisijas tomogrāfiju (PET) vai viena fotona emisijas datortomogrāfiju (SPECT). Nesenie sasniegumi magnētiskās rezonanses jomā (MR) var izrādīties noderīgi šīs PS diagnostikā vai prognostiskajā darbā. Kā PS biomarķieri tiek izmantotas difūzijas attēlveidošanas modalitātes (DTI), jo īpaši ar difūziju gar perivaskulārās telpas (ALPS) indeksu un glimfātisko sistēmu. Lai apstiprinātu to izmantošanu diagnostikas vai prognostiskas nolūkiem, ir jāveic, vērienīgāki un pētījumi. Tiem jānovērtē arī attēlu korekcija saistībā ar brīvā ūdens ietekmi uz vidējiem DTI mērījumiem. PS diagnostikā miera stāvokļa funkcionālā MR (RS-fMRI) precizitāte dziļā neironu tīkla analīzē 96,4%. Lai gan trūkst ilgtermiņa pētījumu, tika konstatēts, ka zemas frekvences svārstību reģionālajam viendabīgumam un daļējai amplitūdai ir spēja novērtēt iespējamo turpmāko komplikāciju risku. Quantitative susceptibility mapping (QSM) bija pēdējā svarīgā novērtētā modalitāte, kas nozīmē, ka substantia nigra un globus pallidus ir saistīti ar PS. Kā redzams “HybraPD” atlasā, labākos rezultātus uzrādīja QSM apvienošana ar citām attēlveidošanas metodēm. Šis literatūras pārskats apkopo informāciju par difūzijas, funkcionālajiem un vielmaiņas biomarķieriem un to, kā tos var klīniski izmantot PS ārstēšanā Parkinson’s disease (PD) is to date still mainly diagnosed using clinical tests and sometimes with the support of positron emission tomography (PET) or single photon emission computed tomography (SPECT). Recent advancements made in magnetic resonance imaging (MRI) could show use of this imaging modality in the diagnostic or prognostic work-up of PD. Diffusion imaging modalities (DTI) have shown use especially with diffusion along perivascular space (ALPS) index and glymphatic system as biomarkers for PD. Larger, longitudinal studies must be conducted to confirm the use of these for diagnostic or prognostic purposes. These should also evaluate the effect of free water correction on average DTI measures. Resting state functional MRI (RS-fMRI) in a deep neural network analysis was able to achieve 96.4% accuracy in diagnosing PD, which should be reproduced but is a promising approach. Regional homogeneity and fractional amplitude of low frequency fluctuation were found to have good ability to assess risk of developing future complications although longitudinal studies were lacking. Quantitative susceptibility mapping (QSM) was the last important modality evaluated, implications the substantia nigra and globus pallidus to be associated with PD. Combining QSM with other imaging modalities showed the best results as seen in the HybraPD atlas. This review gives an overview about diffusion, functional, and metabolic biomarkers and how they can be applied to PD management clinically. |
