Šneidera membrānas perforācija atvērtās sinus lift operācijas laikā

Abstract

Mērķis: Šneidera membrānas perforācija ir galvenā deguna blakusdobuma pacelšanas operācijas komplikācija. Atkarībā no perforācijas lieluma nepieciešams veikt atbilstošu ārstēšanu. Ārstēšanas iespējas ir: perforētās membrānas šūšana, fibrīnlīmes lietošana, resorbējamās kolagēna membrānas ievietošana vai Šneidera membrānas mobilizācijas paplašināšana. Pēc tam audu pārpalikums noved pie spontānas defekta slēgšanās. Šajā pētnieciskajā rakstā tiek pētītas šīs dažādās metodes, pati operācija, komplikācijas un sastopamības biežums, kā arī vairāki pētījumi par implantu izdzīvošanu kombinācijā ar perforāciju.

Metodes: Lai identificētu visus iespējamos atbilstošos publicētos rakstus, tika izmantota šāda meklēšanas stratēģija: EBSCOhost, ScienceDirect, Wiley Online Library, PubMed un ProQuest. Tika izmantotie meklēšanas vienumi: ''Šneidera membrānas perforācija'', ''Sinusa augšžokļa anatomija'', ''Sinusa pacelšana'', ''Kaula palielināšana'', ''atvērta sinusa pacelšana'', ''Implanta izdzīvošana''. ; membrānas perforācija”, “Klasifikācija Šneidera membrānas perforācija”, “Sānu pieeja”, “Šneidera membrānas perforācijas vadība” un “Komplikācijas sinusa pacelšana”. Tika izmantotas papildu atsauces, kas norādītas atsauces sadaļā, kas atrodama vācu bibliotēkā Bonnā.

Rezultāti: Šis pētnieciskais darbs identificēja vairākus pētījumus par to, kā rīkoties Šneidera membrānas perforācijas gadījumā. Perforācijas visbiežāk rodās, kad membrāna tiek atdalīta no blakusdobuma kaula sienām. Perforācijas tiek klasificētas pēc izmēra un novietojuma. Pēc šo faktoru noteikšanas seko pareiza ārstēšana. Nav būtiskas atšķirības implantu izdzīvošanas rādītājos, kas tika ievietoti zem membrānām kuras ir ārstētas pēc perforācijas ar implantiem, kas tika ievietoti zem veselīgas membrānas.

Objective: Schneiderian membrane perforation is the main complication of the sinus lift procedure. Depending on the size of the perforation an adequate treatment will follow. Treatment possibilities are: suturing of the perforated membrane, the use of Fibringlue, the placement of a resorbable collagen membrane, or extending the mobilization of the Schneiderian membrane. The excess tissue then leads to closure of the defect. This research paper explores these different techniques, the surgery itself, complications and incidences and several studies about the survival of implants in combination with the perforation.

Methods: The following search strategy was used to identify all possible relevant published articles: EBSCOhost, ScienceDirect, Wiley Online Library, PubMed and ProQuest. Search terms that were used were: ‘’Schneiderian membrane perforation’’, ‘’Anatomy of sinus Maxillaris’’, ‘’Sinus lift’’, ‘’Bone augmentation’’, ‘’open sinus lift’’, ‘’Implant survival; membrane perforation’’, ‘’Classification Schneiderian membrane perforation’’, ‘’ lateral approach’’, ‘’Management Schneiderian membrane perforation’’ and ‘’Complications sinus lift’’. Additional references were used indicated in the reference section found in a German library in Bonn.

Results: This research paper identified several studies on how to manage the perforation of the Schneiderian membrane. It happens mostly during its dissection and reflection from the sinus bone walls. Perforations are classified according to size, perforation, and position. The right treatment will follow after identifying these factors. The individual patient cases need to be studied beforehand. There is no significant difference in implant survival rates which were placed below perforated and repaired membranes with implants which were placed underneath a healthy membrane.
