The doctor-patient relationship, health literacy and treatment adherence

Abstract

Mērķis: Šī pētniecības darba mērķis ir izpētīt saikni starp ārsta un pacienta attiecībām, veselības stāvokļa izpratni un terapijas ievērošanu. Metodes: Pētījums veikts kā literatūras pārskats, meklējot klīniskos pētījumus izmantojot PubMed un Sage Journals datu bāzes. Izmantotie atslēgvārdi: “ārsta un pacienta attiecības”, “hroniska slimība”, “psihiatrs”, “pacienta ārsta saziņa”, “ārsta un ārsta attiecības”, “ārsts kā pacients”, “veselības pratība”, “pacienta ārsta saziņa”, “sakaru pacients”, “pacientu neuzticība”, “pacientu ārstu attiecības”, “dzīves kvalitāte”, “pacienta atbilstība”, “terapeitiskā alianse” un “pieturēšanās ārstēšanai”. Iekļaušanas kritēriji ietver pētījumus, kas publicēti pēdējo desmit gadu laikā, un ietver vismaz 100 dalībniekus. Netika iekļauti pētījumi, kas vecāki par desmit gadiem un/vai kuros piedalījās mazāk par 100 dalībniekiem. Kopumā sākotnējā meklēšanā tika iegūti 55 pētījumi, bet izskatīšanai tika atlasīti 32 raksti pēc iekļaušanas un izslēgšanas kritēriju piemērošanas. Secinājums: Ārsta un pacienta attiecības ietekmē terapijas ievērošanu. Gan tieši kā komunikācija starp pacientu un ārstu, gan netieši arī ar faktoriem ārpus klīnikas apmeklējumiem, piemēram, internetā atrodamā informācija un pacienta zināšanas par veselību. Ārstam ir svarīgi atzīt visus pacienta un ārsta attiecību aspektus, lai varētu nodrošināt atbilstošu pakalpojumu un uzlabot.

Aim: The aim of this research work is to explore the connection of doctor-patient relationship, health literacy and treatment adherence. Methods: Research was performed as a literature review by searching for clinical trials using PubMed and Sage Journals databases. Keywords used: “Doctor-patient relationship”, “chronic disease”,” psychiatry”, “doctor patient communication”, ”Doctor-doctor relationship”, “Doctor as patient”,” health literacy”, “patient doctor communication”, “communication patient”,” patient mistrust”,” patient doctor relationship”, “quality of life”, “patient compliance”, “therapeutic alliance” and “treatment adherence”. Inclusion criteria includes studies being published within the past ten years and include a minimum of 100 participants. Studies older than ten years and/or having less than 100 participants were excluded. A total of 55 studies were obtained in the initial search while 32 articles were selected for review after applying inclusion and exclusion criteria. Conclusion: The doctor-patient relationship does influence treatment adherence. Both directly as the communication between patient and doctor but also indirectly by factors outside the clinical meeting such as information found on the internet and the health literacy of the patient. It is important for the doctor to acknowledge all aspects of the patient-doctor relationship to be able to provide adequate service and to enhance the patients’ adherence to treatment.
