Abstract

Maģistra darba nosaukums: Dzemdes kakla vēža skrīninga programmas analīze un pilnveide Latvijā. Tēmas aktualitāte: dzemdes kakla vēzis ir viens no visizplatītākajiem vēža gadījumiem sievietēm (Paulauskiene, et al., 2019). Pēdējo 10 gadu laikā Latvijā saslimstība ar dzemdes kakla vēzi ir pieaugusi par 20% (SPKC, 2022). Vairumu dzemdes kakla vēža un ar to saistīto nāves gadījumu var novērst, atklājot pirmsvēža izmaiņas dzemdes kaklā, veicot uz iedzīvotājiem balstītu citoloģisko skrīninga programmu (SPKC, 2022). Latvija organizē valsts apmaksātu dzemdes kakla vēža skrīninga programmu sākot ar 2009.gadu (NVD, 2022b), taču neskatoties uz ievērojamiem uzlabojumiem dzemdes kakla vēža skrīninga programmā Latvijā rādītāji joprojām ir zemi - tikai 40 % latviešu sievietes vecumā no 20 līdz 69 gadiem 2019. gadā veica dzemdes kakla vēža pārbaudi, salīdzinot ar vidēji 58% ES. Pētījuma mērķis: izpētīt Latvijā organizēta dzemdes kakla vēža skrīninga programmas organizāciju un izstrādāt priekšlikumus tas pilnveidošanai. Pētījuma mērķa sasniegšanai autore galvenie uzdevumi ir noskaidrot dzemdes kakla vēža skrīninga programmas organizācijas teorētiskos aspektus un konceptuālās pieejas starptautiskajā praksē, izpētīt dzemdes kakla vēža skrīninga programmas organizāciju un atsaucības rādītājus Latvijā, noskaidrot dzemdes kakla vēža skrīninga organizāciju un pieejamību, izdarīt secinājumus un izstrādāt rekomendācijas dzemdes kakla vēža skrīninga programmas modeļa pilnveidošanai. Zinātniskā darba ietvaros galvenie pētījuma jautājumi ir: kāds ir dzemdes kakla vēža skrīninga programmas organizācijas raksturojums Latvijā, kādi ir dzemdes kakla vēža skrīninga programmas sniegumu ietekmējošie organizatoriskie faktori un kādi uzlabojumi ir nepieciešami dzemdes kakla vēža skrīninga programmas organizācijā Latvijā? Uz visiem pētījuma jautājumiem autore pētījuma ietvaros atbildēja. Pētījuma rezultātā tika izveidots uz PVO rekomendācijam balstīts konceptuālais modelis, kurš sastāv no septiņiem skrīninga novērtēšanas posmiem katrā posmā ar saviem snieguma radītājiem, pēc kura tika analizēta dzemdes kakla vēža skrīninga programmas organizācija Latvijā. Katrs rādītājs rika novertēts pēc realizācija līmeņa no 0 līdz 2. Secinājumi: dzemdes kakla vēža skrīninga programmas organizācija Latvijā nav līdz galam attīstīta un tai nepieciešami nopietni uzlabojumi. Iegūtie darba rezultāti turpmāk var tik izmantoti dzemdes kakla vēža skrīninga valsts programmas attīstībai nākotnē. Title: Analysis and enhancement of cervical cancer screening programme in Latvia. The relevance of the topic: cervical cancer is one of the most common cancers in women (Aulause, et al., 2019). Over the past 10 years, in Latvia, cervical cancer has grown by 20% (SPKC, 2022). The incidence of cervical cancer and associated deaths can be prevented by detecting pre-cancer changes in the cervix by a population-based cytological screening program (SPKC, 2022). Latvia organizes a state-paid cervical cancer screening program from 2009 (NVD, 2022b), but despite significant improvements in the cervical cancer screening program in Latvia, rates remain low, with only 40% of Latvian women aged 20 to 69 in 2019 conducting a cervical cancer test, compared to an average of 58% in the EU. The purpose of the study: explore the organization of a cervical cancer screening program organized in Latvia and develop proposals for improving it. To achieve the purpose of the study, the author's main tasks are to clarify the theoretical aspects of the cervical cancer screening program organization and conceptual approaches in international practice, to explore the organization and responsiveness of the cervical cancer screening program in Latvia, find out the organization and availability of cervical cancer screening, draw conclusions and develop recomendations for improving the pattern of cervical cancer screening programs. As part of the scientific work, the study questions are: what is the characteristics of the organization of the cervical cancer screening program in Latvia, what are the organizational factors influencing the performance of the cervical cancer screening program and what improvements are needed in the cervical cancer screening program organization in Latvia? To all the questions in the study, the author answered as part of the study. The study resulted in the creation of a conceptual model based on the WHO Recipient, which consists of seven stages of screening assessment at each stage with its performance creators, after which the organization of the cervical cancer screening program in Latvia was analyzed. Each indicator is rotated after realization level from 0 to 2. Conclusions: The organization of the cervical cancer screening program in Latvia is not fully developed and needs serious improvements. The results of the work obtained can continue to be used so in the future for the development of the cervical cancer screening national program.
