Ievads
Myasthenia Gravis (MG) ir autoīmūna neiromuskulāra saslimšana un aptuveni 90% pacientu serumā ir nosakāmas autoantivielas pret kādu no postsinaptiskās membrānas komponentiem. Plazmas apmaiņas (PA) terapija tiek pielietota ne tikai akūtu MG simptomu ārstēšanā, bet arī kā ilgtermiņa terapija, tādēļ ir nozīmīgi apkopot, kādi ir PA terapijas riski un ieguvumi, lai izvērtētu PA efektivitāti MG terapijā.
Metodoloģija
Literatūras apskatā iekļauti zinātniskie raksti no datu bāzēm – PubMed un ScienceDirect, kas publicēti laika posmā no 2013. līdz 2023. gadam.
Rezultāti
Datu bāzēs pēc noteiktajiem atslēgvārdiem “Myasthenia Gravis” un “Plasma exchange therapy” tika atlasīti 4887 zinātniskie raksti, pēc filtru pielietošanas tika iegūti 658 raksti. Raksti, kas atbilst pēc nosaukuma - 36, bet, izlasot anotāciju, tika atsijāti vēl 23. Rezultātā literatūras apskatā tika iekļauti 13 zinātniskie raksi.
Secinājumi
Pēc literatūras apskatā iekļautajām zinātniskajām publikācijām var secināt, ka PA terapija, neskatoties uz nevēlamo blakus parādību risku, ir efektīva ārstēšanas metode akūtas Myasthenia Gravis terapijā un pirms timektomijas veikšanas. Anti – Musk (antivielas pret muskuļspecifisku receptoru tirozīnkināzi) MG pacientu terapijā PA rezultējas ar labāku klīnisko atbildi, nekā IvIg terapija. Lai izvērtētu PA rezistentas MG terapijā, ir nepieciešami papildus pētījumi. ABSTRACT Introduction Myasthenia Gravis (MG) is an autoimmune neuromuscular disease and approximately 90% of patients have detectable serum autoantibodies to a component of the postsynaptic membrane. Plasma exchange (PEX) therapy is used not only for acute symptoms of MG but also as a long-term therapy, therefore it is important to summarise the risks and benefits of PEX therapy in order to evaluate the efficacy of PEX in the treatment of MG. Methodology The literature review included scientific articles from the PubMed and ScienceDirect databases published between 2013 and 2023. Results The databases were searched for 4887 scientific articles using the keywords "Myasthenia Gravis" and "Plasma exchange therapy", 658 articles were retrieved after applying filters. 36 articles matched the title, but after reading the annotation a further 23 were screened out. 13 scientific articles were included in the resulting literature review. Conclusions The literature review suggests that PEX therapy is an effective treatment for acute MG and before thymectomy, despite the risk of adverse events. In anti-Musk (MG patients, PEX therapy results in a better clinical response than intravenous immune globulin therapy. Further studies are needed to evaluate PA in resistant MG therapy. |
