Abstract: | Šajā klīniskajā gadījumā pacientam ar stabilas stenokardijas klīnisko ainu tiek veikta
plānveida koronārā angiogrāfija. Nosakot vairākus kritiskus sašaurinājumus koronārajās
artērijās, ar visizteiktāko bojājumu kreisā priekšējā lejupejošā koronārajā artērijā (LAD), tiek
veikta perkutānā koronārā intervence (PCI). Lai raksturotu aterosklerotiskās pangas sastāvu tiek
veikta tuvās-infrasarkanās gaismas spektroskopija/intravaskulārā ultrasonogrāfija
(NIRS/IVUS). Pirms intervences, pēc pre-dilatācijas, pēc stenta implantācijas un pēc postdilatācijas tiek noteikts maksimālais lipīdu serdes indekss 4 mm segmentā (maxLCBI4mm)
kreisā priekšējā lejupejošā koronārā artērijā un maxLCBI4mm attiecīgi ir 594 → 492 → 388
→ 331. Intervences laikā divkārši samazinās lipīdu serdes indekss, savukārt minimālais lumena
laukums palielinās no 6.2 mm2
līdz 11.2 mm2
. Pēc literatūras datiem maxLCBI4mm >400
asociējas ar paaugstinātu nelabvēlīgu kardiovaskulāru notikumu risku, tomēr pacientam ir
neatbilstība starp klīnisko ainu un koronāro artēriju aniogrāfisko atradni. Šī neatbilstība liek
pārvērtēt klīnisko simptomu un diagnostisko izmeklējumu prognostisko nozīmi.
Neskatoties uz stabilo klīnisko ainu un ņemot vērā invazīvo izmeklējumu klīnisko
atradni, pacientam hospitāli attīstās akūts koronārs sindroms. Pacients tiek novērots intensīvās
terapijas nodaļā, bet sūdzības un miokarda bojājuma marķieri mazinās un divu dienu laikā tiek
izrakstīts no slimnīcas. Izrakstoties tiek papildus nozīmēta duāla antiagregantu terapija un
statīni maksimāli tolerējamā devā kombinācijā ar ezetimibu.
Pacienta apsekošanas nolūkos pēc 3 mēnešiem zema blīvuma lipoproteīna holesterīns no
sākotnējā līmeņa 3.37 mmol/l samazinājās līdz 0.78 mmol/l. Viens no būtiskākajiem statīnu
efektiem ir aterosklerotiskās pangas stabilizācija. Tieši šis elements varētu būt visbūtiskākais,
kāpēc pacientam pirms pirmreizējās stacionēšanas nebija nevēlami kardiovaskulāri notikumi.
Lai pārliecinātos par pacienta reziduālo kardiovaskulāro risku, kas varētu būt saistīts ar
iekaisumu, tiek veikta arī pozitronu emisijas tomogrāfija/datortomogrāfija, tomēr netiek
konstatētas hipermetabolas izmaiņas koronārajās artērijās. Līdz ar ko var spriest par
aterosklerotiskās plātnītnes stabilitāti. Tiek atkārtota arī invazīvā diagnostika un analizēts LAD
segments ar NIRS/IVUS. Sākotnēji maksimālais lipīdu serdes indekss (maxLCBI4mm 594)
praktiski ir mazinājies līdz nullei, savukārt LAD proksimālā daļā saglabājusies lipīdu serde,
neskatoties uz invazīvo un medikamentozo terapiju (maxLCBI4mm 314). In this case report, planned coronary angiography is performed on a patient with a stable angina pectoris clinical presentation. Several critical stenoses in the coronary arteries are identified, with the most pronounced lesions in the left anterior descending coronary artery (LAD), where percutaneous coronary intervention (PCI) is performed. Near-infrared spectroscopy/intravascular ultrasound (NIRS/IVUS) is performed to characterize the composition of the atherosclerotic plaque. Before intervention, after pre-dilation, after stent implantation, and after post-dilation, the maximum lipid core burden index in a 4 mm segment (maxLCBI4mm) is determined in the LAD, and the values are 594 → 492 → 388 → 331, respectively. During the intervention, the lipid core burden index is reduced by half, while the minimal luminal area increases from 6.2 mm2 to 11.2 mm2 . According to literature data, maxLCBI4mm >400 is associated with an increased risk of adverse cardiovascular events, but there is a discrepancy between the clinical presentation and the angiographic findings in the coronary arteries of the patient. This discrepancy requires a reassessment of the prognostic significance of clinical symptoms and diagnostic examinations. Despite the stable clinical presentation and considering the clinical findings of the invasive examination, the patient develops an acute coronary syndrome in the hospital. The patient is observed in the intensive care unit, but symptoms and myocardial damage markers decrease, and he is discharged from the hospital after two days. Upon discharge, dual antiplatelet therapy and statins are additionally prescribed at the maximally tolerable dose in combination with ezetimibe. For follow-up purposes, after three months, the low-density lipoprotein cholesterol is reduced from the initial level of 3.37 mmol/l to 0.78 mmol/l. One of the most significant effects of statins is the stabilization of atherosclerotic plaque, which could be the most important reason why the patient did not experience adverse cardiovascular events before admission. To evaluate the patient's residual cardiovascular risk, which could be associated with inflammation, positron emission tomography/computed tomography is also performed, but no hypermetabolic changes are detected in the coronary arteries. Therefore, it can be concluded that the atherosclerotic plaque is stable. Invasive diagnostics are repeated, and the LAD segment is analyzed using NIRS/IVUS. Initially, the maximum lipid core burden index (maxLCBI4mm 594) has practically decreased to zero, while the lipid core burden in the proximal part of the LAD is preserved despite the invasive and medical treatment. |
