Abstract: | Ievads: Sifiliss ir seksuāli transmisīva slimība, kuru izraisa spiroheta Treponema
pallidum. Ar to var inficēties galvenokārt seksuāla kontakta ceļā. Visbiežāk sifiliss ir
atpazīstams tieši ādas izpausmju dēļ, bet infekcijai attīstoties, tā izpausmes var būt mainīgas.
Lai arī slimība šķiet bieža un atpazīstama, ne reti ir sastopamas slimības atipiskas formas
ar netipiskām slimības izpausmēm.
Mērķis: Apkopot biežākās atipiskās sifilisa izpausmes izmantojot brīvpiekļuves
datubāzēs publicētos klīniskos gadījumus.
Metodes: Klīniskie gadījumi tika atlasīti PubMed un Scopus elektroniskajās datu
bāzēs. Pētījumam tika atlasīti klīniskie gadījumi, kas publicēti laika posmā no 2013.gada
janvāra līdz 2023.gada martam.
Rezultāti: Literatūras apskatā tika iekļauti 71 klīniskie gadījumi, kas tika sadalīti pēc
slimības stadijas un ietekmētās orgānu sistēmas. Pētījumi, kuros aprakstīta atipiska primāra
sifilisa norise bija pieci, sekundāra sifilisa atipiska norise – 62 klīniskie gadījumi, terciāra
sifilisa atipiska norise – četri pētījumi. Primāra sifilisa gadījumā infekcijas atipiskas
izpausmes varēja novērot lokāli uz ādas. Sekundāra sifilisa gadījumā visbiežāk atipisku
norisi varēja novērot uz ādas un centrālajā nervu sistēmā. Terciāra sifilisa gadījumā
visbiežākā atipiskā pazīme bija gummu veidošanās atipiskās lokācijās. Visbiežāk izmainīta
norise sastopama sekundāra sifilisa gadījumā, jo slimība ir izplatījusies audos un katru no
orgānu sistēmām ietekmē citādāk.
Secinājumi: Atipiskām sifilisa slimības formām ir saistība ar blakus saslimšanām.
Visbiežākā blakus saslimšana pacientu vidū bija HIV. Introduction: Syphilis is a sexually transmitted disease caused by the spirochete Treponema pallidum. It can be transmitted almost exclusively through sexual contact. Syphilis is most often recognized due to skin manifestations, but its manifestations can be variable as the infection develops. Although the disease appears to be common and recognizable, atypical forms of the disease with atypical disease manifestations are not uncommon. Objective: To summarize the most common atypical manifestations of syphilis using clinical cases published in open-access databases. Methods: Clinical cases were selected from PubMed and Scopus electronic databases. Clinical cases published between January 2013 and March 2023 were selected for the study. Results: The literature review included 71 clinical cases divided by disease stage and affected organ system. There were five studies describing the atypical course of primary syphilis, 62 case studies describing the atypical course of secondary syphilis, and four studies describing the atypical course of tertiary syphilis. In the case of primary syphilis, atypical manifestations of the infection could be observed locally on the skin. In the case of secondary syphilis, the most common atypical course could be observed on the skin and in the central nervous system. In tertiary syphilis, the most common atypical sign was gumma formation in atypical locations. The most often altered course occurs in secondary syphilis because the disease has spread systematically and affects each of the organ systems differently. Conclusions: Atypical syphilis forms of the disease are associated with comorbidities. The most common comorbidity among patients was HIV. |
