Selective laser trabeculoplasty(SLT) as alternative treatment for patients with glaucoma

Abstract

Mērķis - Novērtēt SLT lāzera procedūru kā efektīvu spiedienu samazinošo terapiju, kas varētu būt kā primāra ārstēšanas metode pacientiem ar glaukomu vai kā papildu terapiju, pacientiem ar primāru atvērta kakta glaukomu vai okulāru hipertensiju. Materiāli un metodes – Šis retrospektīvais pētījums ietver datus no 23 medicīniskajām dokumentācijām, kas tika ievāktas Dr. Solomatina acu centrā laika posmā no 2022. gada jūnija līdz decembrim 2022. gada. Pacienti tika rūpīgi atlasīti, iekļaušanas kritēriji : bija – pacienti ar primāro atvērta kakta glaukomu (POAG) vai okulāro hipertensiju (OHT), glaukomas diagnoze atbilstoši EGS (European Glaucoma Society) vadlīnijām, un tika veikta gonioskopija. Katram pacientam acu spiediens tika mērīts pirms SLT lāzera procedūras, 1 nedēļu-1 mēnesi un 3-6 mēnešus pēc SLT lāzera procedūras. Pacieti tika iedalīti trīs grupas, atbilstoši spiediena mērījumiem. Dati tika analizēti izmantojot MS Excel un IBM SPSS 27 programmu. IOS izmaiņas starp grupām tika analizēti, izmantojot Atkarīgo izlašu T testu (Paired Samples T test) un Zīmju testu (Sign test). Rezultāti - Kopumā pētījumā tika iesaistīti 23 glaukomas pacienti un 35 acis. 52% (n=12) bija vīrieši un 48% (n=11) bija sievietes. Vidējais vecums bija 65 (S 13,5) gadi, no 40 līdz 86 gadiem. No iegūtiem rezultātiem tika secināts, ka bija statistiski nozīmīga acu spiediena atšķirība (p < 0.001) starp grupām – Acu spiediens pirms SLT procedūras un Acu spiediens 1 nedēļu-1 mēnesi pēc SLT lāzera procedūras. Kā arī efekta lielums bija vidējais (Cohen’s d=0.715). Ka arī statistiski nozīmīgas atšķirības (p = 0.009) tika atrastas starp grupām - Acu spiediens 1 nedēļu-1 mēnesi un 3-6 mēnešus pēc SLT lāzera procedūras. Pētījums parāda, ka 22 acīm (63%) acu spiediens ir samazinājies attiecībā uz sākotnējo acu spiedienu pirms SLT lāzera procedūras. Savukārt, 7 acīm (20%) acu spiediens pēc SLT lāzera procedūras ir palielinājies un 6 acīm (17%) acu spiediena izmaiņas pēc SLT lāzera procedūras netika novērotas. Acu spiediena vidējā vērtība samazinājās par 2.6 mmHg jeb par 14,17%. Secinājumi - SLT lāzera procedūra dod pietiekamu IOS samazinājumu. SLT lāzera procedūru var ieteikt kā alternatīvu ārstēšanas metodi glaukomas medikamentozās terapijas neefektivitātes gaidījumos. Šī procedura ir ieteicama pacietiem ar glaukomas slimību, lai samazinātu IOS līmeni un paleninātu tās progresiju.

Objectives. The aim of this study was to evaluate the effectiveness of selective laser trabeculoplasty (SLT) as initial or adjuvant treatment in patients with primary open-angle glaucoma or ocular hypertension. Materials and methods. This retrospective study included 23 medical records of patient’s eyes who visited ophthalmologist in Dr. Solomatin eye center in the period from June 2022 to December 2022. Inclusion criteria were patients with diagnosed POAG or OH, patients with clear glaucoma diagnosis (based on European Glaucoma society guidelines) and performed gonioscopy. Each patient underwent IOP measurement before SLT, 1 week to 1 month after SLT and 3 to 6 months after SLT procedure and were divided into 3 groups according to eye pressure measurements. Data were analyzed by MS Excel and IBM SPSS 27 program. Difference between groups were evaluated by Paired Samples T – Test and Sign test. Results. Data were obtained for 35 eyes from 23 patients. 52% of patients (n=12) were male and 48% of patients (n=11) were female. The mean age was 65 (S 13,5) years with range from 40 to 86. There was highly statistically significant difference (p < 0.001) in IOP between two groups - IOP before SLT procedure and IOP 1 week to 1 month after SLT procedure. Effect size was medium (Cohen’s d=0.71). Also, statistically significant difference (p = 0.009) was found between IOP after 1 week to 1 month and 3 to 6 months after SLT. Study have demonstrated the IOP reduction in 22 eyes (63 %), in 7 eyes eye pressure increased (20%) and in 6 eyes eye pressure did not demonstrate change. Study resulted with IOP reduction of 2.4 mmHg or 14,17%. Conclusions. Significant results were observed in IOP reduction after SLT procedure. Selective laser trabeculoplasty is an effective alternative to drops used in glaucoma treatment. This procedure can be offered for patients with glaucoma to lower IOP and reduce further visual field progression and optic nerve damage.
