Abstract: | Hemofagocītiska limfohistiocitoze (HLH) ir reta un dzīvību apdraudoša imūnas sistēmas saslimšana. To raksturo pārāk aktīva īmūnās sistēmas atbilde, kas rezultējās pārmērīgā T limfocītu un makrofāgu aktivācijā un proliferācijā, kas noved pie hemafagocitozes- asins šūnu destrukcijas kaulu smadzenēs, aknās un liesā.
HLH var būt gan primāra, gan sekundāra. Primāra saistās ar iedzimtiem ģenētiskiem imūnās sistēmas šūnu un/vai funkciju defektiem, bet sekundāra ar infekciju, malignitāti vai citu pavadošo stāvokli vai slimību. HLH ir bieži fatāla un tai ir īpaši svarīga agrīna diagnostika un ārstēšana. Ebšteina- Barra vīrusa (EBV) asociēta HLH ir reta, bet biežākais virālās HLH iemesls bērnu populācijā.
Šajā darbā ir paskaidrots klīniskais gadījums par 12 gadus vecu meiteni, kas iestājās stacionārā ar sūdzībām par drudzi 8 dienas ilgi, augšējās elpceļu infekcijas pazīmēm, sāpēm vēderā un mugurā. Fiziskā izmeklēšanā konstatēja kakla limfadenopātiju, drudzi un hepatosplenomegāliju, kamēr laboratori: limfocitozi, trombocitopēniju, paaugstinātus aknu enzīmus, hiperferitinēmiju un hipertrigliceridēmiju. Serumā tika konstatēts augsts EBV DNS kopiju skaits un kaula smadzeņu izmeklējumā hemofagocītiska limfohistiocitoze un makrofāgu aktivācija, bez pazīmēm par malignitāti. Tika uzsākta augstu devu kortikosteroīdu terapija, kas nedeva vēlamo uzlabojumu. Otrās līnijas medikaments Anakinra sniedza nelielu klīnisku uzlabojumu, bet pēc 4 devām Rituksimaba, klīniskais stāvoklis ievērojami uzlabojās un paciente tika izrakstīta ambulatorai terapijai.
Agrīna diagnostika un specifiskas terapijas uzsākšana sniedza iespēju veiksmīgi ārstēt pacientu ar sekundāru HLH. Hemophagocytic lymphohistiocytosis (HLH) is a rare, life-threatening disorder of the immune system. It is characterized by an overactive immune response resulting in excessive activation and proliferation of T lymphocytes and macrophages leading to hemophagocytosis, which is the destruction of blood cells within the bone marrow, liver, and spleen. HLH can be either primary or secondary, with the primary form being hereditary and the secondary form arising as a result of an underlying medical condition or infection. Early diagnosis and treatment are crucial for improving the outcome of this severe and often fatal disease. Epstein Barr virus (EBV) related HLH is the most common type of viral HLH in childhood. This case is about a previously healthy 12-year-old girl that presented to our hospital with 8 days of fever, signs of acute upper respiratory tract infection and a pain in the back and abdomen. Physical investigation revealed cervical lymphadenopathy, high fever, hepatosplenomegaly, while laboratory tests revealed lymphocytosis, trombocytopenia, increased liver enzymes, hyperferritenemia, hypertrygliceridemia. High EBV DNS copy count correlated with hemophagocytic lymphohistiocytosis and macrophage activation in bone marrow biopsy without signs of malignancy. High dose steroids were initiated without any improvement, second line treatment with Anakinra gived a slight improvement, but only after 4 doses of Rituksimab patients general condition significantly improved and she was discharched for an outpatient treatment. High clinical suspicion and prompt evaluation were critical to early treatment and decreased morbidity. |
