Ekstramammāra Pedžeta slimība saistībā ar invazīvu taisnās zarnas adenokarcinomu

Abstract

Dotajā darbā autore sniedz aprakstu neparastai divu slimību vienlaicīgai kombinācijai - Ekstramamāra Paget (Pedžeta) slimība, kas manifistējās ar vulvas un anusa apvidus intraepiteliālu adenokarcinomu kombinācijā ar taisnās zarnas ampulārās daļas invazīvu adenokarcinomu, un sniedz aprakstu šo slimību netipiskām izpausmēm. Dotā pētnieciskā darba saturs iekļaus konkrēta pacienta raksturojumu, klīniskās atradnes, pielietoto diagnostisko metožu un terapeitiskās iejaukšanas analīzi, ziņojumu par slimību uzraudzības pasākumiem un iznākumu konkrētam reprezentētam gadījumam. Pētnieciska darba iegūstams rezultāts paredz to, ka tiks izanalizēts klīniskais gadījums ar diviem dažādiem audzējiem, kur tāda audzēju kombinācija literatūrā ir reti aprakstīta. Turklāt nav pieejama kāda liela apjoma konkrēta izlase ar šo slimību kombināciju. Dotajā klīniskajā gadījumā tiks reprezentēta Gerontoloģijas klīnikas Paliatīvās aprūpes nodaļas paciente ar pamatdiagnozi: C20 – taisnās zarnas adenokarcinoma uz anālā kanāla, starpenes ādas Pedžeta slimības fona, IV stadija. Pacientes pamatdiagnozes sarežģījumi: multiplas metastāzes aknās; stāvoklis pēc paliatīvās ķīmijterapijas; hepatomegālija; metastāzes abdominālajos limfmezglos, cirkšņa limfmezglos; metastāzes Th12, L4 līmenī; sāpju sindroms. Augstāk minēta diagnosticēto slimību kombinācija ir ļoti reta. 2021. gada oktobrī pacientei tika veikta diagnostiska manipulācija - biopsija un patologs ir sniedzis mikroskopisko aprakstu. Pirmais audu fragments bija ar izteiktu elektrotermisku bijājumu un epitēlija struktūras nebija izvērtējamas. Otrā materiāla ādas audu fragments klāts ar hiperplazētu un akantotisku epiteliju; tā bazālā un vidējā slānī multiplu liela izmēra šūnu proliferāti, kuri iesniedzas epidermas videjā daļā, šo šūnu citoplazma pozitīvi reaģē ar PAS (Perjodiska skābes Schiff reakcija). Jātzīmē, ka pacientei pirms tam vairāku gadu garumā tika veiktas biopsijas no starpenes un anālā apvidus epidermas, kur arī tika diagnosticēta Pedžeta slimība. Veiktajā imūnhistoķīmiskajā izmeklējumā šīs šūnas izrādījās CK20 pozitīvas, CK7 retas pozitīvas šūnas un p16 negatīvas. Tika sniegts sekojošs patohistoloģiskais atzinums: Morfoloģiska un imūnhistoķīmiskā aina atbilst Pedžeta slimībai. Saskaņā ar SSK-10 pacientei atklāta C51 diagnoze – Sieviešu ārējo dzimumorgānu ļaundabīgs audzējs. Balstoties uz augstāk minētiem imūnhistoķīmiskiem izmeklējuma rezultātiem var domāt, ka dotajā klīniskajā gadījumā Pedžeta slimība pacientei varētu būt sekundāra, attīstījusies uz taisnās zarnas ampularās daļas invazīvās adenokarcinomas fona. Pielietojot attēldiagnostiku, kļuva skaidrs, ka pacientes taisnās zarnas adenokarcinoma uz anālā kanāla, starpenes ādas Pedžeta slimības fona progresēja līdz metastātiskai stadijai ar attālām metastāzēm aknās.

The author of the given paper describes an unusual combination of two diseases: extramammary Paget's disease manifested by intraepithelial adenocarcinoma of the vulva and anus combined with invasive adenocarcinoma of the ampullary part of the rectum, and describes the atypical manifestations of these diseases. The content of this research paper includes a description of the patient, an analysis of the clinical picture, diagnostic methods and therapeutic interventions used, a report of the following disease, and the result of the presented case. The result of the research work is the analysis of a clinical case with two different tumours, where such combination of tumours is rarely described in the literature. Moreover, no large specific sample with this combination of diseases is available. The given case report describes a patient of the Palliative Care Unit of the Gerontology Clinic with a primary diagnosis of C20 - rectal adenocarcinoma in the background of the anal canal, perineal skin Paget's disease, stage IV. The presented complications of the patient's primary diagnosis are: multiple metastases in the liver; status post palliative chemotherapy; hepatomegaly; metastases to abdominal lymph nodes, inguinal lymph nodes; metastases at Th12, L4 level; pain syndrome. The presented above combination of diagnosed diseases is very rare. In October 2021, it was performed diagnostic manipulation: biopsy and the pathologist has provided a microscopic description. The first tissue fragment had a pronounced electrothermal lesion and the epithelial structures were not evaluable. The second skin tissue fragment was covered with hyperplastic and acanthotic epithelium; its basal and middle layers contained multiple large cell proliferates extending into the medial epidermis, the cytoplasm of these cells reacted positively with PAS (Periodic Acid Schiff reaction). It needs to be noted that the patient had previously had several years of biopsies from the perineal and anal epidermis, where a Paget's disease had also been diagnosed. The performed Immunohistochemistry showed these cells to be CK20 positive, CK7 rare positive and p16 negative. The following pathohistological findings were made: morphological and immunohistochemical picture is consistent with Peget's disease. According to the ICD-10, the patient was diagnosed with C51 malignant neoplasm of the female external genitalia. Based on the above immunohistochemical findings, it is possible to suggest that in this clinical case the patient's Paget's disease might be secondary to invasive adenocarcinoma of the ampullary part of the rectum. Using imaging diagnostics, it became clear that the patient's rectal adenocarcinoma had progressed to metastatic stage with distant liver metastases in the background of anal canal, perineal skin and Paget's disease.
