Modificēta mīksto audu estētikas koeficienta pielietošana autotransplantētu trešo molāru estētikas novērtēšanai

Abstract

Estētikas kritēriji zobārstniecībā visbiežāk tiek izmantoti frontālā rajona ārstēšanas rezultātu izvērtēšanai un salīdzināšanai. Dažreiz estētikas novērtēšana sānu rajonā tiek uzskatīta par mazāk nozīmīgu, taču bieži vien tieši izmainīti estētiskie rādītāji var tieši vai netieši norādīt uz potenciālām funkcionālām un bioloģiskām problēmām. Tādi kritēriji kā vājš kontaktpunkts, nepareiza zoba pozīcija zoba rindā vai nepilnīga oklūzija, var pasliktināt periodonta stāvokli un potenciāli novest pie zoba zaudēšanas. Darbā tiek pētīta modificēta mīksto audu estētikā koeficienta pielietošana autotransplantētu molāru rajonā. Metodes. Tiek analizētas fotogrāfijas autotransplantētiem trešiem molāriem, lai novērtētu to pozīciju zobu rindā un mīksto audu salīdzinājumu ar blakus zobiem. Mīkstos audus izvērtēja divi speciālisti, rezultāti tika apkopoti tabulās un savstarpēji salīdzināti. Rezultāti. Salīdzinot estētisko novērtējumu starp mērītājiem, un viena mērītāja veiktajiem novērtējumiem, iegūta augta ticamība (ICC > 0,9). Par estētiski pieņemamu rezultātu, kur nebūtu nepieciešami estētiski vai funkcionāli uzlabojumi pēc abu mērījumu veicēju domām būtu pacienti ar rezultātu virs 7 punktiem. Rezultātiem ar 6 un 7 punktiem varētu būt nepieciešami nelieli uzlabojumi. 1-5 punkti būtu nepieciešama ortodontiska vai protezēšanas ārstēšana lai uzlabotu autotransplantētā zoba pozīciju, estētiku un funkciju. Secinājumi. Metode ir izmantojama estētisko kritēriju novērtēšanai autotransplantētiem zobiem sānu rajonā. Būtu nepieciešams iesaistīt vairāk izmeklējamos kritērijus , lai novērtētu ne tikai estētiku bet arī estētikas iespējamo ietekmi uz funkciju un autotransplantēta zoba ilgmūžību.

Esthetic criteria in dentistry are most often used to assess and compare the results of frontal are treatment. Sometimes the assessment of esthetics in the side area is considered less significant, but often directly altered esthetic indicators can directly or indirectly indicate potential functional and biological problems. Criteria such as a weak contact point, incorrect tooth position on the tooth line or incomplete occlusion can impair periodontal condition and potentially lead to tooth loss. Research is carried out application of modified soft tissue aesthetics coefficient in autotransplanted molar region. Methods. Photographs of autotransplanted teeths are analysed to assess comparison of their position in the row of teeth and soft tissues with neighbouring teeth. The soft tissues were assessed by two specialists; the results were tabulated and compared. Results. All researchers were consistent in repeated measurements, and ICCs ranged between 0.97 and 0.94. High correlation was found for inter-examiner repeatability (ICC 0.92). An esthetically acceptable result, where esthetic or functional improvements would not be required according to both measurement participants, would be patients with a result above 7 points. Results with 6 and 7 points might require some improvement. 1-5 points would require orthodontic or prosthetic treatment to improve the position, esthetic and function of the autotransplanted tooth. Conclusions. The method can be used for the assessment of aesthetic criteria for autologous teeth in the distal region. It would be necessary to involve more criteria under investigation to assess not only aesthetics but also the potential impact of aesthetics on function and the longevity of an autologous tooth.
